5 PROFESSIONAL SERVICES Basic Professional Fees. Professional consultation with licensed personnel, including expert advice on options to meet your needs...$695 Professional Funeral Director s Services. These services include the personal coordination and supervision by the licenced funeral director of all the arrangements on behalf of the family, including the coordination of all rites and ceremonies, before, during and after they have been provided...$585 Visitation Staff. Staff to coordinate and oversee the visitation at our premises for a period up to 4 hours...$385 Funeral Support Staff. Staff to coordinate and oversee the ceremony or memorial at our premises...$300 If the ceremony or memorial takes place at an off-site location, additional charge of $300 per ceremony fee will apply and Facilities for Ceremony will not be charged. Funeral Director s Attendance at cemetery or crematorium for witnessing/committal next day after the funeral service...$585 If funeral coach required for transfer to cemetery or crematorium next day after church service there is an additional fee of $395...$980 Standard Embalming. Embalming is the process of sanitation, restoration and temporary preservation. Embalming is not legally required, but may be recommended to preserve the body between the time of death and the visitation or disposition of the deceased. Our professional funeral directors will make a recommendation based on the condition of the body and expected service needs...$450 Basic Care. The preparation of the body for placement into the casket, including bathing, dressing, use of a vinyl enclosure and the setting of facial features for identification or viewing without embalming...$400 Documentation. Completion and filing of all documents necessary to carry out the services and supplies requested, including, but not limited to, death registration, burial permit, coroner s certificate for cremation. As part of this service, we will provide you with 10 Proof of Death Certificates. Such certificates are usually accepted for most estate settlement purposes, but sometimes a Provincial Death Certificate will be necessary and is available from the Office of the Registrar General via Service Ontario website...$400 FACILITIES AND EQUIPMENT Basic Use of Facilities. Use of our general and common areas, parking lot, public washrooms, cloakrooms, heating, lighting and maintenance...$340 Facilities for Visitation & Setup (requires Visitation Staff). Designated facilities used for visitation for up to 4 hours...$580 Facilities for Ceremony (requires Funeral Support Staff). Designated facilities at our Funeral Home used for the ceremony or memorial service...$300 Preparation or Basic Care Facilities (requires Standard Embalming or Basic Care). Facilities used to prepare, embalm and/or shelter the body; this charge applies whenever the body is in our facility for any purpose...$400 TRANSPORTATION NEEDS Transfer of Recipient. Transportation of the remains from one place to another, for example the hospital to our premises or from our premises to the cemetery or crematorium using a transfer vehicle, for distances not exceeding 30km...$300 2-person Transfer...$395

6 General Administrative Vehicle. A general-purpose vehicle for the accomplishment of various funeral related tasks and duties. May be used as a lead/clergy vehicle in a funeral procession if required... $195 Funeral Coach. A specialized vehicle used to transport the casket or container from the funeral home to the cemetery/crematorium, for transportation to a church or another location for a funeral ceremony (3 hours)... $395 Limousine. The use of this vehicle is insured for up to nine (9) passengers and is used to transfer family members (if desired) on the day of the funeral (3 hours)... $475 Additional mileage travelled beyond 30km from our facility will be charged from $2.00 to $2.25 per kilometer one way only. All vehicles are for a time period of 3 hours. After which livery overtime hours will apply. OTHER AVAILABLE SERVICES AND EQUIPMENT Receptions. Use of facilities for a reception following the ceremony or memorial service, which includes the set up and the use of the facilities and amenities... $600 Off-site Reception. Extended attendance at off-site reception (2 staff & vehicle)... $195/hr Equipment Charge. Use of Funeral Home equipment such as the Casket Carriage, Flower Stands, Lecturns, Candles, Religious Equipment, Picture Easels... $750 Additional Staff Services for Visitation (requires Additional Facilities). Staff to coordinate and oversee the visitation for a period which exceeds the service plan selected... $100/hr Removal of Pacemaker or Implant. The removal of a pacemaker or implant will be carried out by a licensed professional... $45 Professional Restorative Procedures. In some circumstances, additional restorative efforts beyond those captured in the standard embalming procedure are necessary to enhance the appearance of the deceased. In such cases, the funeral director will discuss the available options and their cost... from $495 Environmental Service Surcharge. A charge for biohazard materials and disposal costs$10 Staff Surcharge for Sunday Funeral. This is a surcharge that is imposed when a funeral ceremony is conducted on a Sunday or a statutory holiday... $650 Shipping Administration. If remains are being shipped outside of North America, we will arrange for transport and all necessary paperwork... $450 plus all shipping costs. Audio Visual. Should a family require our audio visual equipment during the ceremony or visitation... $100 Flower Vehicles. This vehicle will take the floral arrangements to the cemetery... $225-$250 DISBURSEMENTS Clergy Honorarium. This is an amount paid to the Clergy for their services... from $300 Newspaper Notices. This is a fee for death notices placed in newspapers... from $250 Coroner s Cremation Certificate. This is a fee charged by the Coroner for issuing a certificate that allows cremation to take place or to transfer out of the province/country... $75 Municipal Registration Fee. This is a fee charged by the local municipality to register the death. The fee is set by the municipality... $50
