2 Pre-Arrangements If something were to happen to you today, would your family know what to do? Do they know your wishes and would your final arrangements be exactly as you wish? A host of responsibilities accompanies the loss of a loved one. Pre-arrangements can help in numerous ways: Remove decision-making burden from your loved ones by making personal choices well in advance. Ensures services and merchandise chosen now will be guaranteed for the same price no matter what the cost is in the future. Earned growth adds to the final benefit, with excess refunded to your beneficiaries, if applicable. Flexible payment plans available. Transferable plan if you move out of the area. First-Day Coverage or Guaranteed-Issue plans are available depending on individual health. Preserves the option to obtain government assistance in the future, by making plan irrevocable. Provides risk protection during the payment period. Transportation Benefit (additional $500 if death occurs 250 or more miles away from principal residence and basic plan exceeds $2000. Protection for your grand children available for a nominal premium at the time of application. We honor most plans, regardless of original Funeral Home. After Care After the funeral, there will be a number of things that require attention. Most of them can be postponed for a reasonable length of time, until you feel ready. Bereavement Programs Grief is a normal response to any loss, affecting the grieving person physically, emotionally, and spiritually. Time is necessary to the healing process, but it is only one aspect of effective grieving. In addition to taking time, grief requires intentional work by the bereaved in order to achieve a healthy outcome from the process. In your time of grief, counseling is important in helping you to come to terms with the passing of a loved one. Hughes Mortuary staff can recommend and connect families with trusting and caring grief counseling services in our area and around the nation. Well trained and compassionate, the counseling groups we work with are there in your time of need. 2

However, any funeral arrangements you select will include a charge for our basic services. To simplify the General Price List we have established the following options that reflect traditional types of services most frequently requested. While you are free to arrange services on an itemized basis, the options listed below and on the following page, represent a discounted price from the same goods and services if selected from our itemized list. 4 BASIC SERVICES OF THE FUNERAL DIRECTOR & STAFF $875 This fee for our basic services will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials, and forwarding or receiving remains.) This fee includes the following: Personnel available 24 hours per day, 365 days per year to respond to initial calls. Arrangement Conference with a Funeral Director. Coordinating service plans with cemetery, crematory, and/or other parties involved with the final disposition of the deceased. Clerical assistance in the completion of all paperwork associated with the funeral. Our cost, including facility maintenance, equipment, inventory costs, insurance, our administration expenses and governmental compliance. Preparation of the body Except in special cases, embalming is not required by law. Embalming may be necessary, however, if you select funeral arrangements such as a funeral with a viewing. If you do not choose embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial. This charge includes: minimal services of funeral director and staff, procuring necessary authorizations and permits, shelter of remains and transportation from airport, and transportation of remains to cemetery or crematory. If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of fiberboard or composition materials (with or without an outside covering.) The containers we provide are fiberboard. 9 Children s Services: The loss of a child is the most devastating experience a parent can face-and missing the child never goes away. A piece of yourself is lost and your future is forever changed. Our way to help those in this situation is to offer our services at a discounted and /or waived professional fee. We also offer all children s items such as the casket and vault at cost. Still birth to one year...no Charge Cremation 12 years and younger....no Charge PAYMENT OPTIONS Hughes Mortuary has established a uniform payment policy to serve all families equally. This payment policy enables us to contain our costs & REQUIRES ALL FINANCIAL ARRANGEMENTS BE COMPLETED (1) FULL DAY BEFORE THE SERVICE TIME OR THE USE OF OUR MERCHANDISE AND PROPERTY. The following are acceptable payment options: 1. Cash, Personal Check, Cashiers Check or Money Order 2. Pre-arranged funeral plan through Hughes Mortuary 3. 12 Jimmie & Tawny Hughes About Us Jimmie and Tawny Hughes are deeply rooted in Southern Utah with Jimmie being a fifth generation native of this beautiful area. As the youngest of ten siblings, Jimmie was raised on the values of honesty, integrity, hard work, and providing service to others.
