The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain in writing on the statement we provide describing the funeral goods and services you selected. Payment must be made in full within 24 hours of the funeral arrangement conference. For your convenience, we accept all major credit cards. Payment may also be made by verified and assignable insurance. If payment is made by an insurance policy, an additional fee of $ OR 10% (whichever is least) will be added to the cost. Services Basic Services of Funeral Director and Staff: $1150. The fee for our basic services and overhead will be added to the total cost of the Funeral Arrangements you select. (This fee is already included in our charges for direct cremations, immediate burial and forwarding or receiving of remains.)

2 This fee includes the following: Personnel available 24 hours a day, 365 days a year to respond to the initial call; Arrangement Conference; Coordinating service plans with cemetery, crematory and/or other third parties involved with the final disposition of the deceased; Clerical assistance in the completion of various forms associated with a funeral; Securing and recording the death certificate and disposition permit; also covers overhead, such as facility maintenance, insurance and administration expenses and government compliance. Embalming $495. Except in certain special cases, embalming is not required by law. However, embalming may be required if you select certain funeral arrangements, such as a funeral with a viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as a direct cremation or immediate burial. 5 Immediate Burial (without service or ceremony) Range $1,695 to $15,795 This service includes: Minimum service of funeral director and staff and a portion of the overhead cost; refrigeration (3 days) and disinfection of the remains; removal of the deceased from the place of death to the funeral home within 25 miles; utility vehicle and transportation to a local cemetery within 25 miles. (This package does not include merchandise or other optional service.) Immediate burial with casket or container provided by purchaser...$1,695. Traditional Service $3,275. This service includes: The professional service of the funeral director and staff; embalming and/or disinfection of the remains; dressing, casketing, cosmetizing; the removal of the remains from the place of death to the funeral home within 25 miles; use of facility and staff for visitation up to two hours, funeral ceremony at the funeral home, or set up at other facility, graveside service, and the transportation such as hearse, utility vehicle and flower car. (This package does not include merchandise or other optional service.) B. Graveside $2,595. This service includes: The professional service of the funeral director and staff; disinfection and refrigeration of the remains; dressing and casketing; removal of deceased from the place of death to the funeral home within 25 miles; transportation to the cemetery within 25 miles; utility vehicle and flower car; professional staff for directing the graveside service. 6 D. Memorial Service (Cremation with Memorial Service) $1,295. This service includes: The professional service of the funeral director and staff; disinfection and refrigeration of remains; removal of the deceased from the place of death to the funeral home within 25 miles; transportation to the crematory; utility vehicle; crematory fee; use of the facilities and staff for memorial service/ceremony in the chapel or other facility. E. Traditional Cremation Service This service includes: The professional service of the funeral director and staff; embalming and preparation of the remains, dressing, casketing and cosmetizing; removal of remains from the place of death to the funeral home within 25 miles; use of facility and staff for visitation up to two hours; funeral ceremony at the funeral home or other facility; use of transportation equipment such as hearse, flower car and utility vehicle; transportation to the crematory and crematory fee, cremation casket (PBF Saguaro) and a plastic urn. (This package does not include merchandise or other optional service.) Traditional Cremation Service with a Cremation Casket...Range $3, $10, F. Direct Cremation (without service or ceremony) $ to $5, This service includes: Minimum service of the funeral director and staff and a portion of the overhead cost; refrigeration (1 day) and disinfection of remains; removal of deceased from the place of death to the funeral home within 25 miles; transportation to the crematory, utility vehicle and crematory fee. If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or compositions materials (with or without and outside covering) The containers we provide are: Alternative rigid cardboard container and basic alternative container. Direct Cremation with container provided by purchaser......$ Direct Cremation with standard cardboard container (up to 150 lbs)....$ Direct Cremation with composition material/wood ( lbs)..$ Direct Cremation with enforced wood bottom ( lbs)..$ Any persons over 350 pounds will require a special cremation container of enforced wood. The charge for the cremation will be the $ base charge plus $2.75 per pound starting at pound 351. Any persons over 600 pounds may require a surgical reduction procedure. If such procedure is needed all required consent forms must be signed by the responsible next of kin. If a surgical reduction procedure is required; the cremation will be at the charges listed above plus the cost of the reduction $500.00
