2 General Price List Prices are effective as of August 24, 2018 but are subject to change without notice. The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. You may choose only the items you desire. If you are charged for any items you did not specifically request, we will explain the reason for the charges on the written memorandum. Please note that there may be charges for items such as cemetery fees, flowers, newspaper notices, certified death certificates, police escorts, etc. BASIC SERVICES OF FUNERAL DIRECTOR AND STAFF... $ This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials, and forwarding or receiving of remains.) This charge is also included in the other packaged funeral offerings as allowed by law. This fee includes but is not limited to the following: Personnel available 24-hour availability of funeral director and/or staff to respond to initial request for services, arrangement conference with family or responsible party, arrangement of service, tribute or memorialization. Staff to advise, assist and recommend appropriate personalization and individualization of the services to commemorate the unique life of your loved one. Obtaining and recording vital statistics and biographical information. Death certificate preparation, filing and obtaining necessary permits, authorizations, consents and assignments. Coordination with other parties such as hospital, doctor s office and staff, cemetery, crematory, clergy, and others involved in the services and disposition. Sheltering and care of decedent Also included in this charge are the overhead expenses relative to our facilities such as professional licensing, administrative services, insurance, maintenance, building, and utility expenses, taxes, legal and accounting fees, record keeping, equipment and inventory expenses. Cremation Society of Idaho Reception and Life Event Center We are proud to offer area families a comfortable place where they may enjoy a time of fellowship and gathering with friends and family. Our facility allows you the opportunity to come together while relieving you of the burden of preparing, hosting and cleaning up after a gathering somewhere else. Our reception facility is also available to commemorate other life events, such as anniversaries, reunions, etc. Rental Fee $250* Mon-Fri: 10:00 AM 2:00 PM Use of Facility Completed by 4:00 PM After 4:00 PM - each additional hour at $100 per hour. Saturday or Sunday Services/Gatherings are an additional $250 *Includes: Use of facility for up to 2 hours after funeral services, coordinator (setup and hosting), table covers, and centerpieces. For your convenience, we offer a full selection of catering options. 2

3 PREPARATION OF REMAINS: Except in certain special cases, embalming is not required by law. Embalming may be necessary however, if you select certain funeral arrangements, such as a funeral with a viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial. If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering). The containers we provide are the minimum cardboard containers without outside coverings or interior linings. Direct cremation includes reduced minimum services of funeral director and staff, transport from place of death to funeral home (within 20 mile radius), refrigeration, sanitary care of unembalmed remains, crematory fee, service vehicle, and permit.
