You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reasons in writing on the statement we provide describing the funeral goods and services you selected. This list does not include prices for certain items that you may ask us to buy for you, such as cemetery or crematory services, flowers, and newspaper notices. The prices for these items will be shown on your bill or the statement describing the funeral goods and services you selected. METHOD OF PAYMENT Payment for goods and services is expected at the time arrangements are made, unless other arrangements have been made prior to need. For your convenience we accept cash, check, credit card (Visa, Master Card, Discover or American Express), or a verifiable irrevocable insurance assignment. There will be a 3% surcharge on all cash advance items purchased with a credit card. Prior to drafting any contract for goods or services, the responsible party or the decedent s survivor who is handling the funeral arrangements is entitled to receive a copy of any preneed agreement in the possession of the funeral establishment that has been signed and paid for, in full or in part, by or on behalf of the decedent. 2 (Page 2 of 6) ADMINISTRATIVE AND PROFESSIONAL SERVICES OF FUNERAL DIRECTOR AND STAFF This is a basic fee that we charge every family for our professional expertise in assisting them with their preferences. It includes a proportionate share of the taxes, licenses, utilities, and business expenses required to serve the public. Our charge includes, but is not limited to, availability 24 hours per day, 365 days per year, consultation with the family, administrative and clerical services, preparation of the death certificate for certification of the attending physician or coroner, necessary permits, filing the death certificate with the appropriate agencies, consultation with the clergy, cemetery, crematory or common carrier, assistance with insurance papers, and placements of newspaper notices. There will be a charge for each service requested or required. This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials, and forwarding or receiving remains.) Embalming is the disinfection and temporary chemical preservation of the remains by a professional licensee of the State of California.
