The goods and services shown on the following pages are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean that you must buy items that you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. 2 HMC General Price List (FD 362) As of June 2016 California State Regulations Requirement: Prior to drafting any contract for goods and services, the responsible party or the decedent s survivor who is handling the funeral arrangements is entitled to receive a copy of any pre-need agreement in the possession of the funeral establishment that has been signed and paid for, in full or in part, by or on behalf of the decedent. Discounted Service Program Packages Although services may be selected from the itemized portion of the general price list, the following service packages may help you select the most beneficial arrangements for your circumstances. However, it is important to understand that these service packages include only the items listed in the package description. Additional merchandise (such as casket and urn choice) and cash advance items (such as Certified Death Certificates) will be added to the total cost of the arrangement package you choose. A. COMPLETE TRADITIONAL SERVICE $ This package includes: the services of the director and staff, administrative staff and professional staff for coordinating and directing the service; obtaining all legal documents and other paperwork necessary to complete the arrangements; embalming and or sanitary care of the remains; other preparations of the remains, dressing, casketing, cosmetizing; use of the facilities for funeral ceremony or set up at the church or other facility, visitation room and preparation room; automotive equipment (funeral coach, pall-bearer's car to local cemetery, basic service vehicle); memorial folders, register book and acknowledgment cards. Monday through Friday. *Evening Visitation Extra B. GRAVESIDE SERVICE WITH VISITATION $ This package includes: the services of the director and staff, administrative staff and professional staff for coordinating and directing the service; obtaining all legal documents and other paperwork necessary to complete the arrangements; embalming and or sanitary care of the remains; other preparations of the remains, dressing, casketing, cosmetizing; supervision of visitation period, visitation room and preparation room; automotive equipment (funeral coach to local cemetery, basic service vehicle); memorial folders, register book and acknowledgment cards. Monday through Friday. *Evening Visitation Extra C. TRADITIONAL CREMATION SERVICE $ This package includes: the services of the director and staff, administrative staff and professional staff for coordinating and directing the service; obtaining all legal documents and other paperwork necessary to complete the arrangements; embalming and or sanitary care of the remains; other preparations of the remains, dressing, casketing, cosmetizing; use of the facilities for funeral ceremony or set up at the church or other facility, visitation room from 9:00 AM 5:00 PM; basic service vehicle, memorial folders, register book and acknowledgment cards. Monday through Friday. * Evening Visitation, Casket or Urn of your choice, Cremation Fee and Transportation To Crematory Fee all extra. 2

MEMORIAL SERVICE (No Casket at Service) $ This package includes: the coordination and direction of a memorial service in our facilities or another facility or church; memorial folders, register book and acknowledgment cards. Monday through Friday. *Any merchandise is additional. E. IMMEDIATE BURIAL Ranges from $ to $9, tax This package includes: the basic services of the funeral director and administrative staff for obtaining and filing legal documents; dressing and casketing only; transportation of the deceased to the local cemetery for burial; first 24 hours of refrigeration. Monday through Friday. *This package does not include: embalming or other preparations of the remains; viewing or use of the facilities for viewing; funeral service; selected merchandise such as casket, memorial folders, etc. 1. DIRECT CREMATION Ranges from $ 1, to $ 6, Tax This package includes: the basic services of the funeral director and administrative staff for obtaining and filing legal documents; sanitary care of the remains; transportation of the deceased to the crematory, utility vehicle and crematory fees. *This package does not include: embalming or other preparation of the remains; viewing or use of the facilities for viewing or funeral service; merchandise selected such as cremation urn, memorial register, memorial folders and acknowledgment cards. 1. FORWARDING REMAINS TO ANOTHER FUNERAL HOME $ This fee includes: removal from place of death within a 5 mile radius from 9 AM to 5 PM; minimum services of funeral director and staff; embalming. *Merchandise, transport to airport, air tray and common carrier are additional. H. RECEIVING OF REMAINS FROM ANOTHER FUNERAL HOME $ This fee includes minimum services of funeral director and staff and delivery to local cemetery. *This does not include visitation or funeral service. WARRANTIES: The only warranty, expressed or implied, granted in connection with the goods sold with the funeral services described above, are written warranties, if any, extended by the manufacturer thereof. No other warranties of merchantability or fitness for a particular purpose are extended by the funeral director. 3

4 SERVICES Basic Professional Services of Funeral Director and Staff $ Our professional services charge includes, but is not limited to, our 24-hour a day availability of staff 365 days a year to respond to the initial request for our services, consultation with family and clergy and the arrangement of services you select; preparation of necessary certificates, permits and authorizations; drafting and delivery of the death notice to the selected newspaper(s), administrative assistance in filing for Social Security, Veteran's Benefits and any existing funeral insurance; provision of other services prior to, during and following the funeral, including coordinating with those providing other portions of the funeral such as the cemetery, crematory and others. This is the basic fee every family is charged for our professional expertise in assisting them with their preferences. It includes a proportionate share of the taxes, licenses, utilities and business expenses required to serve the public. This fee for our services will be added to the total cost of the funeral arrangements you select. *(This fee is already included in our service packages such as direct cremation or immediate burial). EMBALMING Embalming $ This chemical process provides temporary preservation of the body and eliminates certain health hazards. Only a person licensed by the state, is allowed to complete this procedure. Except in certain special cases, embalming is not required by law. However, embalming may be necessary if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as a direct cremation or immediate burial. They may be constructed of varying gauges of steel, copper or bronze. These caskets are not designed by the manufacturer to resist the entry of air, water or other outside elements.
