You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. Prior to drafting any contract for goods or services, the responsible party or the decedent s survivor who is handling the funeral arrangements is entitled to receive a copy of any preneed agreement in the possession of the funeral establishment that has been signed and paid for, in full or in part, by or on behalf of the decedent. Payment in full is respectfully requested for services and merchandise at the time of arrangements, or prior to services being rendered. We accept cash, check, money order, Visa, Master Card, American Express or Discover cards. 2 Professional Services 1. BASIC SERVICES OF THE FUNERAL DIRECTOR AND STAFF..$ 1, This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. This fee is already included in our charges for direct cremations, immediate burials and forwarding or receiving of remains. This fee includes the following: o Personnel available 24 hours a day, 365 days a year to respond to initial call o Arrangement conference o Coordinating disposition plans with cemetery and/or crematory o Clerical assistance in the completion of legally required forms and permits, including the death certificate. o Overhead expense of our facility relative to insurance, maintenance, taxes and utility expenses. o Notification of Social Security 2. EMBALMING.$ Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have a right to choose and arrangement that does not require you to pay for it, such as direct cremation or immediate burial. 3. 3 12. PREPARATION OF PAPERWORK FOR INTERNATIONAL SHIPPING (not including the fees for the individual consulate)... $ Transportation/Automotive Equipment 1. TRANSFER OF REMAINS TO FUNERAL HOME..$ This fee is within a radius of fifty miles. An additional charge will be made for distances greater than fifty miles at the rate of $3.00 per mile one way. 2. FUNERAL COACH (Hearse). $ This fee is within a radius of fifty miles. An additional fee of $4.50 a mile will be charged per mile after one way. 3. UTILITY CAR.$ This fee is within a radius of fifty miles, and is used for transportation of service related items. An additional fee of $3.00 a mile will be charged per mile after one way. 4. TRANSPORTATION FROM SAN JOSE INTERNATIONAL $ This fee includes the transfer to funeral home and 153 miles extra. Facilities and Staff 1. USE OF FACILITIES AND STAFF FOR VISITATION..$ Includes the set-up of the visitation area, display of floral arrangements and supervision of the visitation. 2. USE OF EQUIPMENT & STAFF FOR OFF SITE VISITATION..$ Includes the set-up of the visitation area, display of floral arrangements and supervision of the visitation at a church or facility of the families choosing. 3. USE OF FACILITIES & STAFF FOR FUNERAL/MEMORIAL IN OUR CHAPEL...$ USE OF EQUIPMENT & STAFF FOR FUNERAL/MEMORIAL AT OUTSIDE LOCATION...$ USE OF EQUIPMENT & STAFF FOR GRAVESIDE SERVICE..$ WEEKEND AND HOLIDAY FEE..$ Services that occur on a Saturday, Sunday or a recognized Holiday are subject to this fee. 7. 5 Minimum Care Charges 1. FORWARDING REMAINS TO ANOTHER MORTUARY....$1, Includes the basic services of funeral director and staff, transfer of remains to the funeral home (within 50 mile radius), embalming, other preparation of the deceased, and utility vehicle (within 50 mile radius) for transportation to requested airport. Additional transportation to airport will be charged via utility vehicle charges. Does not include shipping charges, services at the mortuary, casket or shipping container, or charges from the receiving funeral home. See merchandise. 2. RECEIVING REMAINS FROM ANOTHER FUNERAL HOME....$1, Included in this price when there are no services to be held is basic services of the funeral director and staff, transfer of remains to the funeral home (within 50 mile radius) and transportation to local cemetery via the utility vehicle (within a 50 mile radius.) Does not include the cost of a casket, shipping charges, or charges for services at the funeral home. 3. IMMEDIATE BURIAL Prices ranging from $2, to $13, Includes the basic services of funeral director and staff, transfer of deceased to mortuary, refrigeration, other preparation of the body, utility vehicle and transfer of deceased to place of final disposition within 50-mile radius. This service is non-witnessed and completed at the convenience of the funeral home and the cemetery. DIRECT CREMATION.$ 1, to $ 3, Includes basic services of the funeral director and staff, transfer of remains to the funeral home (within 50 mile radius), refrigeration, basic cardboard alternative container and cremation (crematory fee). If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering). The containers we provide range from cardboard to wood.
