2 GENERAL PRICE LIST The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. This price list does not include prices for certain items that you may ask us to buy for you, such as Cemetery Services, Death Certificates, Flowers, Newspaper Notices, Police Escorts, etc. These items are goods and services that we purchase from third-parties on your behalf. The prices for those items will be shown on the statement describing the funeral goods and services you selected as Cash Advance Items. METHOD OF PAYMENT Payment for goods and services are payable at the conclusion of the Arrangement Conference or at the least, 48 hours prior to the funeral service. For the convenience of the families we serve, Warford-Walker Mortuary will accept cash, check, all major credit cards (Visa, Master Card, Discover, and American Express), verified insurance and pre-need insurance policies. At, we understand that the passing of a loved one can and often times produce financial hardships for families. We do not charge our families added fees for using credit cards or insurance. Warford-Walker Mortuary will not deny funeral service to anyone and will work with families to provide goods and services within their budget. is not a financial institution and does not provide financing for the goods and services we offer. If financial arrangements other than those mentioned above must be made, please feel free to discuss them with the Funeral Director prior to the conclusion of making final arrangements. 2

3 PROFESSIONAL SERVICES Basic Services of Funeral Director and Staff $2, Our fee for the services of funeral director and staff includes, but is not limited to, staff to respond to initial request for service, arrangement conference with family or responsible parties, arrangement of funeral, preparation and filing of necessary authorizations and permits, recording vital statistics, preparation and placement of obituary notices, and coordination with those providing other portions of the funeral, e.g. cemetery, crematory, vault companies and others as required. Also included in the charge are overhead expenses relative to our facility such as insurance, maintenance and utility expenses, secretarial and administrative cost, and equipment and inventory expenses. This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee, on a pro rata basis is already included in our charges for direct cremations, immediate burials and forwarding or receiving remains). Embalming $ Embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements such as a funeral with viewing. If you do not want embalming you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial. Embalming is a chemical process which provides temporary preservation of the body and eliminates health hazards: therefore, it is a policy of this funeral home that remains be embalmed or refrigerated if held over 24 hours after death. This is for the protection of this facility, the general public and our employees. 5 ADDITIONAL SERVICES Forwarding Remains to Another Funeral Home $2, This fee includes Basic Services of Funeral Director and Staff, transfer of the remains into the care of the Funeral Establishment, embalming, transportation to or from local airport or other place of shipment within a 50 mile radius. This charge does not include visitation, ceremonies, merchandise or cost of shipment. Receiving Remains From Another Funeral Home $1, This fee includes Basic Services of the Funeral Directory and Staff, transportation to or from local airport or other places of shipment within 50 miles radius, Temporary shelter; transportation of remains to cemetery or crematory and obtaining necessary authorizations. This charge does not include visitation, ceremonies, or merchandise. Direct Cremation $1, The charges for a direct cremation without an attendant, rites or ceremonies include removal of remains, local transportation to crematory, crematory fee, necessary services of staff and authorizations. It does not include taxes or permit. State and local law do not require a casket for direct cremation. If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering). The containers we provide are Cadence Kraft, White Crepe Interior. 1. However, many cemeteries require that you have such a container so that the grave will not sink. Either a grave liner or a burial vault will satisfy these requirements.
