2 Explanation of Services Professional & Staff Services Co-ordinating activities, rites, ceremonies Professional consultation with licensed personnel, including, but not limited to, expert advice on options to meet your needs. Co-ordination of all activities, rites and ceremonies, before, during and after they have been provided, including services and supplies provided by us, as well as those provided by third party suppliers. Documentation permits, forms, etc. Completion and filing of all documents necessary to carry out the services and supplies requested, including, but not limited to, death registration, burial permit, coroner s certificate for cremation. Provision of 6 Proofs of Death issued by Affordable Burials and Cremations (our proofs of death are usually accepted for most estate settlement purposes, but sometimes a provincial Death Certificate will be necessary and is available from the Office of the Registrar General for an additional charge) Transport remains initial or additional (within 30 km) All transportation of the body/remains, including from place of death to our facility, from our facility to place of disposition and (in the case of cremations) from the crematorium back to our facility. A charge applies for each additional transportation request. 4 Other Available Services and Equipment Peacehold Estate Documentation $ 450 Additional Proof of Death After One year (This fee is for families requesting a proof of death after a year has passed since the death and is in addition to the standard fee of $10 each) Funeral day staff Includes licensed and non licensed staff for the day of the service. Transportation, set up and take down of all related items (ie, flowers, stands, isles, tables, pall etc.) $ 100 $ 600 Additional Transportation of Urn within 30km $ 150 Shipping cremated remains within Canada (Includes packaging, paperwork and transportation to post office and postage fees) $ 250 Transferring cremated remains between vessels $ 75 Body storage after 3 days. ($50 per day) $ 50 Staff services for coordinating off site services $ 375 Staff services for facilitating a memorial service where human remains are not present. A licensed funeral director will help locate and plan a memorial service and help setup according to your wishes. (Please note that none of our staff may take part in, or direct a funeral service.) $ 625 Identification This includes the opening of the container to visually identify the deceased, this will occur at the crematorium and will be for up to 10 people and no longer than 10 minutes. $ 375 Licensed funeral director (per hour charge) $ 150 Refundable Deposit for Cremated Remains When human remains are to be cremated, a refundable deposit might be required to be paid. If the cremated remains are not claimed within one year of the cremation, the deposit may be used to inter the cremated remains in common ground. If the cremated remains are claimed before they are interred, the deposit will be refunded. $ 350 Use of Funeral Coach (3 hours) $ 550 Unless otherwise identified, all prices do not include applicable taxes

5 Disbursements Disbursements are items arranged for and possibly paid for on your behalf by us. You are expected to pay for these items at the time the contract is entered into. The following are some items that are often identified as disbursements. Coroner s fee this is a fee charged by the coroner for issuing a certificate that allows cremation to take place. The fee is set by the Chief Coroner for Ontario. Clergy honorarium this is an amount paid to the clergy for their services. In some cases the amount is set by the clergy, in others it is an amount to be determined by you. Municipal death registration fee this is a fee charged by the local municipality to register the death. The fee is set by the municipality and may vary. Cremation Fee - This fee is charged every time a cremation is requested and will depend on the crematorium selected and their current prices. Payment, financing and funding Payment Payment is due immediately upon signing the contract. Payment may be made by cash, cheque, debit, and most major credit cards. If you are prepaying for services or supplies, pre-authorized chequing is also available. Funding We offer the option of prepaying for services or supplies. When prepaying, you pay for the services and supplies today and the money will be used to pay for the services and supplies when they are delivered at the time of death. Insurance: You may purchase an insurance contract where the proceeds of the insurance contract will be used to pay for the services or supplies at the time they are delivered or the contract is cancelled. Additional information about our payment, financing and funding options is available upon request. Payment is due in full during the first arrangement conference and before the services are carried out. We accept Cash, Cheque, Debit, Visa or Master Card.
