The components of these packages may be selected individually from the General Price List itemized section-see the last few pages. Compare our Chapel and Reception Hall and what we include and offer in our services. We have a full service facility, and include more in our services without upselling. If you would prefer a economy sized venue, those options are available in the area. They may be able to offer what your family needs if you decide to have a viewing or memorial service after the passing of your loved one at an offsite location. If you need a place for more than 150 people, there are firms in town that may be a better option. Our prices for traditional funerals and services, may appear to be more, but maybe not when comparing our casket and outer burial container prices and more importantly what we include in our Memorial Services and Receptions and what our Chapel and Reception Hall have to offer compared to other economy structured venues. We would rather adjust our prices down than jack them up. If you wonder why we offer such a range of service options, it s no different than countless other service providers. The price is determined by when, how fast, and all the extras and options that are available. New cars and airplane tickets for example. We feel each family is entitled to a price that matches the service they request instead of making everybody pay the same amount. We do not have sales goals or offer vacation getaways for upselling products or services. There are very few locally owned funeral homes left in Boise-which may, or may not matter to you. We are not associated with the people or facility that have our name on their website down in Garden City. We have character and don t need to steal. 1

3 3-C CREMATION WITH PRIVATE FAMILY VIEWING $ 2300 Saturday $ 1700 Monday-Friday $ 3400 Saturday(embalming/ autopsy care) $ 2800 Monday-Friday (embalming/ autopsy care) This package offers family members a viewing without embalming, dressing, or a casket. This is for family members, not a public viewing, not a service. We prefer the viewing time be held (Monday Friday or Saturday from 10:00 a.m. to Noon or 1:00 p.m. to 3:00 p.m.) Often times, families confuse this package with others listed. This package includes Transportation from the place of passing, arrangements to schedule and discuss the viewing time and details, professional care, including setting facial features, combing hair, the use of single use sheets and linens for appearance purposes, on our viewing table. We place bottled water and soda out in the reception hall. This include the cremation process our basic urn and death the certificate process. If an autopsy has been performed and the family viewing is requested, we require embalming and autopsy care. If the viewing is public or announced, we require embalming, dressing and a casket. IDENTIFICATION VIEWING 1 PERSON ONLY No charge for this service. It is not a traditional viewing, this is to verify we have the correct person. LIMITED TO NEXT OF KIN for a brief exposure, to IDENTIFY THE BODY. This does not include any preparations and requires a signed release form prior to viewing. We will accompany you to view, and ask that you be prepared for an environment only. This is not held in our chapel, we do nothing to enhance the appearance of the body. This is not a chance to say goodbye, it s a verification process that is very brief. 3

This package includes the cremation service and death certificate process. We have almost any music you request. If chosen, A Video Tribute created by a professional video family history company in Boise. If you are having someone that s good at that making the video, we need it one day prior to the service so we are familiar with it and able to play it without embarrassment of technical issues during the service. Any after hours, weekends or holiday charges are included. "Service Notice" upon request to the Statesman (WE PAY FOR THE SERVICE NOTICE) Services of our staff to communicate with outside parties including the Cemetery, Newspaper, Phone calls from the public and florists about the service details, Veterans Benefits and Arrangements. Delivery of the urn to the Idaho State Veterans Cemetery at a set date and time, including flower arrangements that have been delivered to our funeral home prior to the service, filling out the interment request, faxing it, speaking with the cemetery staff to set up service time verifying service times with pastor/minister/family etc. Service arrangements with a licensed funeral professional at the time of, or prior to death to obtain information and consultation regarding services and details. Completing and Filing the death certificate and ordering copies (one copy included) Basic Urn and Nice Colored Velvet Bag at no charge for veterans or spouses. Consultation and assistance for numerous "service related details" at the time of need and after the initial time of service. INVOLVEMENT WITH MEMORIAL SERVICES WE ARE NOT PROVIDING WEEKEND ARRANGMENTS OR MEETING US FOR DETAILS OBITUARY ASSISTANCE IN ANY PAPER URNS BROUGHT IN ANYTHING ELSE 7-C Standard Cremation with Premium Services Option (Traditional Cremation) $1280 VIEWING AT THE TIME OF THE ARRANGEMENT-- IF YOU ASK TO SEE OR VIEW If we can accommodate the request at that time-- for one person, or anyone else at the arrangement meeting RUSH SERVICE- You ask us to try and perform the cremation service or get death certificates ASAP. This means you need the urn ready or death certificates sooner than we would normally be able to have them. We call the Doctors office, we call again, and we prioritize the ing, filing and submitting of all paperwork, and expedited local issuance copies of the death certificate so we can obtain certified copies ASAP. We RUSH and PUSH to get our work done if you ask for it, this means coming at night to get the cremation done for the next morning, or coming in on a Saturday. In this service we will wait to be paid from some insurance companies, we will help with the obituary submission and process payment on your behalf. Other Goods or Services that are requested beyond the Standard Cremation Goods and Services above can often times be accommodated with the Premium Service Option. If you ask for services, in fairness, we have additional charges, and usually the premium upgrade covers the wide range of requests from families. 5

6 8-C DIRECT CREMATION $595 If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering) This is one of the cheapest cremation package offerings. Yes, we do offer a complete cremation service for this cheap price, please understand the level of service is equal to the price paid. It s unusual that families pay for a better service and request less, but oddly, we often have families ask for this package, and then ask for services listed in others. We have no issue providing a cheap cremation services, but to be fair to families that pay for other packages, we don t add services that are not included below. Local transfer of remains (within 15 mile radius of our facility), from the place of death by an employee to our Licensed Funeral Home in a service vehicle. An arrangement meeting at an agreed time and day, with a Licensed Funeral Professional, at our office, Monday through Friday from 930 a.m. to 4:00 p.m. to complete 3 necessary items: 1. Gather all vital statistics to complete the death certificate. 2. Having the legal next of kin or legally designated person at that meeting, to sign our cremation authorization form if possible. 3. Payment in a non-credit form of $ Add $120. for each additional staff if required on removal. If payment is not available at the time of the arrangement, it can be made later that same day. Vital Statistics can be faxed or ed to our office if all of the information is not known at that time. We send a death certificate to the Dr. of we are apprised of. We do not call them or the coroner. Refrigeration for 3 days Sending it to the coroner for cremation authorization as schedule allows..this is not a rush service. Complete the Death Certificate and File it with Vital Statistics after the Dr. and Coroner have signed it. Cremation Process by a Licensed Funeral Professional Here at Idaho s First Crematory. We will make a phone call to the person designated on our form that the urn is to be released to in approximately 10 business days. The urn must be taken Monday through Friday from 9:30 a.m. to 4:00 p.m. within 30 days of when we notify the agent. You can request the following at an added cost. Direct Cremation by definition does not include a cremation container or an urn. You may provide both of these items. If the body is over 300 lbs add $300 unless the person was disabled. If we are informed of a disability this fee may not apply. Direct Cremation Temporary Urn. No Charge for this urn... we will give it to you. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you select. This list does not include prices for certain items you may ask us to buy or order for you. These cash advance items include; purchasing death certificates, honorarium, cemetery expenses, flowers, newspaper notices, etc.. The prices for those items will be shown on your bill or statement describing the funeral services and merchandise you selected. We have the right to charge you for our services in buying these items. Our Funeral Home makes no representations of warranties regarding caskets, outer burial containers, urns, or other funeral goods sold by the Funeral Home. The only warranties granted, expressed or implied, in connection with the goods sold with this funeral service are the express written warranties, if any, and extended by the manufacturers thereof. No other warranties, including the implied warranties of merchantability and fitness for a particular purpose are extended by our Funeral Home. Our Funeral Home has established a uniform payment policy to serve all families fairly and to prevent any misunderstandings. If the deceased does not have a pre-paid insurance plan on file at our Funeral Home, we require payment for cremation or funeral expenses to be paid in full before we will provide services or merchandise. We will accept insurance assignments only after we verify the beneficiary and whether the policy allows the assignment. Insurance assignments do not include cash advances and we do charge an administrative fee for processing assigned insurance policies. The normal processing time for a policy is 4 to 6 weeks. 9

10 FUNERAL GOODS AND SERVICES PROFESSIONAL SERVICES Basic Services of Funeral Director and Staff and Overhead $1500 Our basic services of funeral director and staff and overhead includes conducting the arrangements conference, assisting in Planning the funeral, consulting with family and clergy, shelter of remains, preparing and filing necessary notices, obtaining necessary authorizations and permits, coordinating with the cemetery or other third parties. In addition, this fee includes a proportionate share of our basic overhead costs which includes, but is not limited to, overhead expenses relative to our facility such as insurance, maintenance and utility expenses, secretarial and administrative costs, and equipment and inventory expenses. This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. This fee is already included in our charges for direct cremations, immediate burials and forwarding or receiving remains. Limited Services of Funeral Director and Staff: $500 This fee is for limited services and overhead that is added to the total cost of the funeral arrangements that are selected. This fee is in our charges for direct cremation, immediate burial, and forwarding and receiving remains. Our limited professional services include the following: On Call Services for the removal of deceased, shelter of remains, a limited time arrangement conference. CARE OF DECEASED Embalming $700 (Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial.)
