2 2018 CONTENTS EMPLOYEE BENEFIT GUIDE This guide provides an overview of the benefits and insurance plan options available to employees and their qualified dependents. It also contains important information you may wish to refer to throughout the year. ABOUT THIS PUBLICATION The information in this booklet is presented for illustrative purposes and was taken from various summary plan descriptions and benefit information. While every effort was taken to accurately report your benefits, discrepancies and errors are always possible. In case of discrepancies between this booklet and the actual plan documents, the actual plan documents will prevail. If you have any questions about your benefits, contact a member of the Human Resources Department. This guide discusses certain plans of Fabick Cat and participating subsidiaries as they apply to plan participants. Employees of such employers covered by other plans, or employees covered by collective bargaining agreements, will have benefits as prescribed by those plans and agreements. 3 MEDICAL PLAN OPTIONS ELIGIBILITY Regular full-time employees working 30 hours or more per week are eligible for Medical, Dental and Vision coverage on date of hire. Spouses and children are eligible for health insurance coverage. However, if a spouse is eligible for benefits on an employer/group plan they are not eligible for coverage on the Fabick medical and/or dental plans. Dependents are covered through the end of the month in which they turn 26 years of age. To add your spouse as a dependent, you must complete and submit a Spousal Affidavit form, along with supporting documentation. All forms should be uploaded via ADP Workforce Now. BIOMETRIC HEALTH SCREENING For 2018, a $10 medical plan premium discount is given per pay period to employees who participate in the biometric health screening and enroll in Fabick s medical plan. A $20 medical plan premium discount is given if both employee and spouse participate in both the screening and the medical plan. MEDICAL BENEFITS Coverage, choice, cost and convenience are factors each of us consider important when selecting a medical plan. All four medical plans are designed to provide employees and their dependents with access to quality, affordable health care by covering a wide range of services and supplies. You can also choose to waive medical coverage. 4 DENTAL/VISION PLAN OPTIONS DENTAL BENEFITS Fabick Cat offers employees the option to choose from two dental plans through MetLife Dental (PDP Plus Plan). This plan pays the cost of one vision exam every twelve months by a network provider and gives an allowance for most eyeglasses and frames. A discount on LASIK or FPK is offered through participating surgery providers. 5 LIFE/DISABILITY INSURANCE & LEAVE LIFE AND AD&D Full-Time Employees are eligible for the following benefits on the 1 st of the month following 60 days of employment. BENEFIT INFORMATION Life Insurance (Company Provided) AD&D Insurance (Company Provided) A $15,000 life benefit is offered to employees who participate in the medical plan. Participants also receive a $1,000 life insurance benefit for their spouse and child(ren). A $15,000 AD&D benefit is offered to employees who participate in the medical plan. Employees Earning: 3 Tier Life Insurance & AD&D (Company Provided) Voluntary Term Life Insurance <$50,000 receive a benefit of $100,000 =>$50,000 but < $100,000 receive a benefit of $150,000 => $100,000 receive a benefit of $250,000 Additional life insurance is offered to employees at reasonable rates, in $10,000 increments, up to a maximum of $500,000. Coverage may not exceed 5 times annual salary. Spouse and child(ren) benefits are available with a $100,000 max for spouse (limited to 50% of employee s elected coverage) and a $10,000 max for child(ren). The insurance cost is based on employee s age and tobacco usage. Refer to ADP Workforce Now for rate details. DISABILITY BENEFITS Full-Time Employees are eligible for the following benefits on the 1 st of the month following 6 months of employment. BENEFIT Short Term Disability Insurance (Company Provided) Long Term Disability Insurance (Company Provided) INFORMATION For hourly/non-exempt employees, this benefit is 60% of base salary up to a maximum weekly benefit of $500 in the event of a non-work related disabling illness or injury. Disability situations are reviewed, approved and managed by MetLife. Exempt employees are eligible for salary continuation for up to 13 weeks provided medical documentation is provided. This benefit replaces 60% of base salary up to a maximum per month. Benefit may be reduced by multiple sources of income and disability earnings. This is a taxable benefit to employees since it is Company Paid. PARENTAL LEAVE Birth and adoptive parents may be eligible to take Family Medical Leave. BEREAVEMENT LEAVE Employees are provided paid time off in the event of a death of an immediate family member EMPLOYEE BENEFIT GUIDE

6 RETIREMENT PLAN & PTO RETIREMENT BENEFITS Fabick Cat provides highly competitive retirement benefits. The Retirement Savings plan provides participants the opportunity to save for retirement through convenient payroll deductions. A company match is offered each pay period. Fabick will match 50% of your deferral up to the first 6%. Fabick may provide a discretionary match on an annual basis at the end of the year based on company profitability. Company contributions will vary and are not guaranteed. A $6,500 catch up is allowed for employees over the age of 50. Participants may elect to contribute up to a maximum of $18,500 per year or as defined by the IRS. PAID TIME OFF BENEFITS PTO may be used in one hour increments. Proper notification and approval is required. PTO may not be carried over and is accrued based on years of service. Employees terminated with less than one year of service will not be paid for pro-rata PTO. Employees with one or more year of service leaving the Company will be paid earned pro-rata PTO. WORK/LIFE BENEFITS BONUS PLAN Fabick provides a discretionary annual bonus plan for all employees and is based on Fabick s overall performance. EMPLOYEE REFERRAL BONUS Receive $1,500 for any person you refer who stays with Fabick Cat for at least six months. 24 HOUR NURSE HOT-LINE The Meritain Health Nurse Line is available 24 hours a day, 7 days a week to Premiere or Value Plan members for your health related questions. You can also talk directly with a Registered Nurse (RN) or listen to recorded health topics. MERCY NURSE ON CALL Mercy Nurse On Call is available 24 hours a day, 7 days a week to Mercy Plan members for your health related questions. You and your family can get more information about your health-related questions and concerns. The RN can put you in contact with community resources in your area as well EMPLOYEE BENEFIT GUIDE

7 WORK/LIFE BENEFITS CONTINUED... TELEDOC 24 hour access to board-certified, licensed doctors is available through this resource for those who are enrolled in either the Premiere or Value Medical Plan options. It is on-demand healthcare solution that provides you with the medical care you need, when you need it. WELLNESS The Commit to Fit Wellness Program is voluntary and not required of any employee. All employees are eligible to participate in the Activity Based Recognition Program. The Activity Based program recognizes employees who participate in various health related activities and rewards them with bonuses. GYM DISCOUNT Fabick will pay 50%, up to a maximum of $200 per year, provided the employee provides proof of attendance (average of 3 times per week for each quarter) payable via payroll each quarter, less applicable taxes. HEALTHCARE SAVINGS ACCOUNT Employees enrolled in the HSA Medical Plan Option are eligible to set aside up to $3,450 for single coverage and $6,900 for family coverage annually of pretax income for qualified medical plan expenses for you, your spouse and eligible dependents. Fabick provides an enrollment incentive of $600 for single coverage or $850 for family coverage and pays administrative fees on the account. There is a $1,000 catch up provision for employees 55 or older. FLEXIBLE SPENDING ACCOUNTS Dependent care and health care flexible spending accounts are available. Employees can set aside a maximum of $2,650 annually of pretax income for reimbursement of eligible health care expenses not otherwise covered and $5,000 annually for eligible day care expenses. BENEPLACE EMPLOYEE DISCOUNT PROGRAM All employees can access an online website with discounted movie tickets, theme park tickets, products, electronics, etc. EDUCATION REIMBURSEMENT Tuition, books and other required fees are paid for approved courses up to a maximum of $5,000 per calendar year. Employees are eligible for this benefit after one year of service. EMPLOYEE ASSISTANCE PROGRAM Fabick uses MetLife Employee Assistance Plan services. The EAP provides free, easy access to many tools and professional counselors trained to help with a variety of personal or family issues. This is a confidential service for employees and their family members. Entitled to up to 3 consultations with a licensed clinician per incident, per individual, per calendar year at no charge. Employees can call WILL PREPARATION SERVICES Will preparation services are available to employees that participate in the Voluntary Life Insurance program. This benefit provides one free visit with an attorney to put together a Will. DIRECT DEPOSIT AND ONLINE STATEMENTS ADP Workforce Now provides real advantages by giving you early access to view your paycheck. You can also access W-2 forms, 1095 form and use the paycheck calculator or W-4 assistant tool via ADP Workforce Now EMPLOYEE BENEFIT GUIDE
