3 THE ENROLLMENT PROCESS The Fabick family appreciates and values the ongoing commitment of its employees and is pleased to offer a comprehensive benefits program as part of an employee s total compensation package; one that is focused on the employee and is highly competitive in today s marketplace. IMPORTANT RESOURCES Various tools and resources are available to you now and throughout the year to help you understand your benefits and enroll. We encourage you to utilize them in your decision making process; turn to page 15 for further details. CONTENTS START DATE REVIEW A New Hire packet will be ed to you and the contents will be available on the Company s Intranet. Enrollment through ADP Workforce Now will become available on your start date You will also receive detailed instructions by on logging on to ADP Workforce Now for the first time from your dedicated HR specialist. Make sure that you fully fill out your contact information and retain your user ID, password and security questions for future reference. FIRST 30 DAYS AFTER START DATE ENROLL NOW Log on to ADP Workforce Now to make your new benefit elections. First 30 days after your start date When you enroll as a New hire you will be eligible for the following electable benefits of your choice on the following time-line. On your start date: Medical/Prescription Drug Coverage, Dental & Vision Insurance, 401k, FSA/DCFSA(s), and Wellness Programs. 60 days after your start date: Company Provided Life Insurance, Voluntary Life Insurance, and AD&D Insurance. 6 months after your start date: Both Company Provided Short & Long Term Disability Insurance(s). 31 DAYS FROM START DATE LAST CHANCE Until the next Open Enrollment this is the last day you can log on to ADP Workforce Now to make your new benefit elections. Deadline: 31 days after your start date After your enrollment period has ended, you will not be able to make changes to your benefit elections until the next yearly Open Enrollment unless you experience a qualifying life event. We encourage you to take personal responsibility for the benefits decisions you make. The Employee Benefit Enrollment Guide provides an overview of the benefits offered to eligible employees of all locations, election considerations and information on how and when to enroll. Remember, the decisions you make during the annual enrollment period will be decisions you will live with for the full calendar year unless you experience a family status change. 5 HEALTH BENEFITS PROGRAM One of the most important decisions you will make as a Fabick Cat employee is what health care coverage to elect through the Fabick Cat Health Benefits Program. Fabick Cat provides you and your family with valuable medical, dental, prescription drug and vision benefits and flexible spending accounts as well as disability and life insurance benefits. LEARN ENROLLMENT DEADLINE You may begin enrolling in your benefits for 2018 on your hire date, and you will have 31 days to complete. You must enroll in health coverage within those 31 days in order to have healthcare coverage in If you do not enroll by the deadline, you will not have healthcare coverage in 2018 unless you have a qualifying family status change (see page 21 for more information) during the year; otherwise you will have to wait for the next open enrollment period which usually occurs in the fall for coverage beginning January 1 of the following year. When you enroll for medical, dental and vision coverage you may choose from several options and elect different coverage categories for each type of plan in the Health Benefits Program. Over the next few pages we have highlighted the benefit coverage in each of the above areas. Please review carefully before making your elections. ELIGIBILITY Regular full-time employees working 30 hours or more per week are eligible for Medical, Dental and Vision coverage on date of hire. Spouses and children are eligible for health insurance coverage. However, if a spouse is eligible for benefits on an employer/group plan they are not eligible for coverage on the Fabick medical and/or dental plans. Dependents are covered through the end of the month in which they turn 26 years of age. To add your spouse as a dependent, you must complete and submit a Spousal Affidavit form, along with supporting documentation. All forms should be uploaded via ADP Workforce Now. BIOMETRIC HEALTH SCREENING For 2018, a $10 medical plan premium discount is given per pay period to employees who participate in the biometric health screening, complete the health survey and enroll in Fabick s medical plan. A $20 medical plan premium discount is given if both employee and spouse participate in the screening, complete the health survey and are on the medical plan. EMPLOYEE BENEFIT ENROLLMENT GUIDE

All four medical plans are designed to provide employees and their dependents with access to quality, affordable health care by covering a wide range of services and supplies. You can also choose to waive medical coverage. Rx Benefit Solutions strives to provide the lowest cost and offers a wealth of services designed to increase well-being. LEARN Under the Fabick plan, after 2 retail refills at your local pharmacy, mail order is required for all maintenance or long-term medications. The mail order program is a cost effective and convenient way for you to order up to a 90 day supply of prescriptions delivered right to your home offering several payment options. BENEFIT Short Term Disability Insurance (Company Provided) Long Term Disability Insurance (Company Provided) INFORMATION For hourly/non-exempt employees, this benefit is 60% of base salary up to a maximum weekly benefit of $500 in the event of a non-work related disabling illness or injury. Disability situations are reviewed, approved and managed by MetLife. Exempt employees are eligible for salary continuation for up to 13 weeks provided medical documentation is provided. This benefit replaces 60% of base salary up to a maximum per month. Benefit may be reduced by multiple sources of income and disability earnings. 9 COVERAGE OPTIONS CONTINUED... LIFE AND AD&D Full-Time Employees are eligible for the following benefits on the 1 st of the month following 60 days of employment. BENEFIT INFORMATION LEARN Life Insurance (Company Provided) AD&D Insurance (Company Provided) A $15,000 life benefit is offered to employees who participate in the medical plan. Participants also receive a $1,000 life insurance benefit for their spouse and child(ren), if also enrolled. A $15,000 AD&D benefit is offered to employees who participate in the medical plan. Employees Earning: 3 Tier Life Insurance & AD&D (Company Provided) <$50,000 receive a benefit of $100,000 =>$50,000 but < $100,000 receive a benefit of $150,000 => $100,000 receive a benefit of $250,000 Additional life insurance is offered to employees at reasonable rates, in $10,000 increments, up to the Guarantee Issue (GI) Amount of $150,000 without providing a Statement of Health (SOH) at initial election or 1 st open enrollment. If employees do not elect during either of those enrollment periods they will be required to provide a Statement of Health. Employee coverage has a medical maximum of $500,000. Coverage may not exceed 5 times annual salary. Voluntary Term Life Insurance Spouse benefits are available in $5,000 increments up to the Guarantee Issue Amount of $50,000 without a Statement of Health (SOH) at initial election or 1st open enrollment. Spouse coverage has a medical maximum of $100,000. Child(ren) benefits are available in $1,000 increments with a $10,000 maximum. A statement of health will be required for employee or spouse coverage that is above the Guarantee Issue (GI) Amount of $150,000 for employee and $50,000 for spouse). The insurance cost is based on employee age & tobacco usage. Refer to the SPD on the Company s Intranet for rate details. FLEXIBLE SPENDING ACCOUNTS (FSAs) ENROLL IN AN FSA AND SAVE MONEY You may enroll in the Healthcare and/or Dependent Care FSAs, which enable you to set aside money, before taxes, to use on eligible healthcare and dependent care expenses. The annual amount you elect to contribute will be taken in equal increments from each payroll. When you enroll in an FSA, which is administered by JW Terrill, your election stays in effect for the entire year. You cannot increase, decrease or cancel your contributions during the year, unless you experience a qualified family status change, such as a marriage, divorce, birth or adoption of a child. Each enrolled participant will receive a Mastercard fully loaded with their annual election amount and can be used on or after January 1 for any qualified healthcare expenses. Just present the card at time of payment. The other option is to complete a claims reimbursement form (located on the Company s Intranet site), attach receipts and submit directly to JW Terrill. FSA CONTRIBUTION LIMITS Healthcare FSA up to $2,650 per plan year. If you and your spouse each have a healthcare flexible spending account, you both can contribute up to $2,650 annually. Dependent Care FSA - up to $5,000 per plan year. If you and your spouse each have a dependent care FSA, you can contribute up to $5,000 between the two of you filing a joint return or $2,500 if married filing a single return. KEEP YOUR RECEIPTS Always keep copies of your receipts as you may be asked to submit them to substantiate your reimbursement because FSAs are regulated by the IRS. MAXIMUM HIGHLY COMPENSATED EMPLOYEE (HCE) LIMIT HCEs (employees with total compensation of $120,000 or more in 2017) may contribute up to $3,800 to the dependent care FSA in 2018 unless further limitations are required under the Internal Revenue Code or related regulations or other guidance. USE IT OR LOSE IT Under current tax rules, once you elect to set aside money in a Dependent Care FSA, you must use it by the end of the plan year. You may not use contributions to the Dependent Care FSA to reimburse health care expenses, and vice versa. So, be sure to estimate dependent care expenses when considering how much to contribute to the FSA in $500 ROLLOVER HEALTHCARE FSA If you have funds remaining in your 2017 Healthcare FSA on December 31, 2017 you may continue to use those funds, up to $500, to pay for eligible expenses incurred through the end of the current year. 11 HEALTH SAVINGS ACCOUNT (HSAs) Employees enrolled in the QHDP option are eligible to set aside up to $3,450 for single coverage and $6,900 for family coverage annually of pretax income for qualified medical plan expenses for you, your spouse and eligible dependents. Fabick provides an enrollment incentive of $600 for single coverage or $850 for family coverage and pays administrative fees on the account. There is a $1,000 catch up provision for employees 55 or older. LEARN A Health Savings Account (HSA) is a special account used to pay for current and future qualified medical expenses. They must be used in conjunction with a Qualified High Deductible Health Plan (QHDP) - HSA Plan. May over fund as a savings vehicle future healthcare costs Potential premium savings from standard medical plan Administrative fees paid for by the Company FABICK WELLNESS PROGRAM COMMIT TO FIT We are excited to enhance the company s Wellness Program in It s still about helping employees on their wellness journey to a better and healthier lifestyle! Fabick Cat is pleased to offer a comprehensive wellness program that rewards commitment to ongoing wellness. The Commit to Fit Wellness Program is voluntary and not required of any employee. All employees are eligible to participate in the Activity Based Recognition Program. The Activity Based program recognizes employees who participate in various health related activities and rewards them with bonuses. The program is administered in accordance with all federal rules permitting employer-sponsored wellness programs that seek to improve employee health or prevent disease, including the Americans with Disabilities Act of 1990, the Genetic Information Nondiscrimination Act of 2008, and the Health Insurance Portability and Accountability Act, as applicable, among others. If you are unable to participate in any of the wellnessrelated activities or achieve any of the health outcomes required to earn an incentive, you may be entitled to a reasonable accommodation or an alternative standard. 12 WELLNESS PROGRAM CONTINUED... LEARN PREMIUM DISCOUNT Employees, and employees spouses, that are currently covered on the Company s medical plan are eligible for a premium discount if they participate in the annual biometric screening and complete the health survey. The on-site and clinic screenings are only available during the Annual Screening Period. Employees on the company s medical plan are eligible for a premium discount of $260 per year if they participated in the screening and completed the health survey, and $520 per year if they and their spouse (covered under the medical plan) both participated in the screening and completed the health survey. Visit for more information. *New employees are allowed to participate in the Activity Based program at any time but will not qualify for the insurance incentive until they ve participated in an Annual Biometric Screening. ACTIVITY BASED RECOGNITION PROGRAM The Activity Based Program is comprised of several different wellness activities. The wellness activities are designed to both promote and improve personal health. Employees may participate in as many of the activity based options as they wish throughout the calendar year The activities included in the program are as follows: Biometric Screening results discussed with physician $100 Reward Discussing your biometric screening results with your physician is the best way to stay in control of your health. By sharing your results, you and your physician can stay a step ahead of any potential health problems. Employee must participate in the annual biometric screening and upload a completed Physician Visit Verification form signed and dated by you and the physician via the H&H Wellness portal. Annual Physical $50 Reward A physical exam by your primary care physician is required to be eligible for this benefit. Employee must complete and submit the Annual Physical form available on Company s Intranet, via the H&H Wellness portal. Dental (2x teeth cleaning) $50 Reward A bi-annual preventive teeth cleaning is required to be eligible for this benefit. Employee must complete and submit the Annual Dental form available on Company s Intranet, via the H&H Wellness portal. Payment will only be processed when both preventive teeth cleanings have been performed. Participation in 2 wellness seminars including Financial $50 Reward Wellness seminars and presentations are a great way to promote health and topics vary widely. The purpose is for the employee to get health and wellness information to improve and/or enhance your overall well-being. Some seminars may be offered through Fabick while others are available via the H&H Associates Wellness Portal or you may seek out on your own. Employee must submit documentation of seminar attendance, via the H&H Wellness portal, to be eligible for this benefit. Participation in a 5k walk/run $50 Reward Participating in a 5K Walk/Run promotes health and wellness and may be an opportunity for employees and their families to join together to raise funds for a charity and its program. Employee must submit registration and payment documentation, via the H&H Wellness portal, to be eligible for this benefit. Intramural Sport Team or participation in 25 exercise classes $50 Reward Participating in an intramural sport team (non-beer leagues) or 25 exercise classes (i.e. yoga, pilates, spinning, Zumba, Cross Fit, etc.) provides opportunities for employees to get active and promotes overall good physical fitness. Employees must submit registration and payment documentation, via the H&H Wellness portal, to be eligible for this benefit. Activity Based Recognition Program Rewards will be paid the first available pay period following the 10th of each calendar month via your payroll check less applicable taxes and withholdings. Activities must be entered by the last day of the month to qualify for payment the month following. 13 OTHER BENEFITS GYM MEMBERSHIP DISCOUNT In addition to the Wellness Program, the Company provides gym membership discount of 50% up to a maximum of $200 per year, provided the employee provides proof of attendance (average of 3 times per week for each quarter) payable via payroll each quarter, less applicable taxes and withholdings. LEARN 24 HOUR NURSE HOT-LINE The Meritain Health Nurse Line is available 24 hours a day, 7 days a week to Premiere or Value Plan members for your health related questions. You and your family can get more information about your health-related questions and concerns. You can also talk directly with a Registered Nurse (RN) or listen to recorded health topics. The RN can put you in contact with community resources in your area as well. Just call TELEDOC 24 hour access to board-certified, licensed doctors is available through this resource for those who are enrolled in either the Premiere or Value Medical Plan options. It is on-demand healthcare solution that provides you with the medical care you need, when you need it. The Retirement Savings plan provides participants the opportunity to save for retirement through convenient payroll deductions. A company match is offered each pay period. 15 STEP 2 PREPARE TO ENROLL REVIEW ELIGIBILITY DEFINITIONS AND RULES CAREFULLY REVIEW THIS SECTION PRIOR TO ENROLLING IN YOUR BENEFITS: You are eligible to participate in the health insurance programs of Fabick Cat if you are a regular, full time employee scheduled to work 30 hours or more per week at the time of hire. PREPARE In general, eligible dependents for whom you may elect coverage under the health benefits include: Your spouse to whom you are legally married in accordance with the plan document provided they are not eligible for another employer/group plan. Covering a spouse requires a signed affidavit. Your eligible dependents per the plan documents up to the age of 26. Under the medical, dental and vision plans coverage ends for dependents who turn 26 at the end of the month. Spouse means any person who is lawfully married to you under any state law, including persons of the same sex who were legally married in a state that recognizes such marriages. The Plan Administrator may require documentation proving a legal marital relationship. DEPENDENT ELIGIBILITY AUDIT The Human Resources department conducts dependent eligibility audits of participants periodically who have dependents enrolled in the Company s health insurance plans. It is important to ensure that your dependents meet the eligibility requirements specified in this section and in the actual plan documents prior to enrolling. Furthermore, after enrolling in your health benefits coverage, you should review the list of dependents in ADP Workforce Now to ensure they still meet the dependent eligibility requirements. If they do not meet the requirements, you should drop coverage for any ineligible dependents timely. You can drop dependents by calling the Human Resources department. Also, please ensure you have proper documentation for your enrolled dependents in your files, including but not limited to birth certificate and a valid marriage license. 16 PREPARE QUESTIONS TO ASK YOURSELF IN THE DECISION MAKING PROCESS TAKE CARE OF WHAT MATTERS HEALTH, FAMILY, FINANCES AND FUTURE Choosing the health plan option that is best for you and your family can leave you feeling a little lost. The type of benefit plan you choose will depend on a number of factors, such as whether you are single or married, whether you have any preexisting medical conditions, whether you have small children, and whether you require frequent sick days or time off to name a few. Before deciding which benefit plan to enroll in, it s important to get details about your benefits by reading through the summary of benefit plans provided. In addition, below are some questions to ask yourself to help guide you to the best decision for you and your family. QUESTION 1 Which best describes your (and your family s) general healthcare usage? Low user I am (we are) generally healthy, rarely get sick and have mostly only preventive care needs. Moderate user: I/we receive regular preventive care but sometimes get sick or have a medical issue that requires additional attention. High user: I/we have an ongoing medical condition that requires regular care and frequent trips to the doctor or hospital. QUESTION 2 What do you think your medical costs will be next year? Think about your costs during the past 12 months and any upcoming expected costs. Few costs: I/we only see the doctor for preventive care, or an occasional office visit, so very low out-of-pocket expenses if any, are anticipated. Moderate costs: I/we have some out of pocket costs from unexpected illnesses or injuries. High, one-time costs: I/we generally have a low to medium amount of healthcare expenses, but expected events such as pregnancy or planned surgery means I/we anticipate high costs this year. High, regular costs: I/we have a chronic medical condition meaning regular diagnostic and ongoing high out of pocket costs. QUESTION 3 Describe your typical prescription drug costs. Few costs: I/we have one or two low cost prescriptions or no prescriptions at all. Moderate costs: I/we take one maintenance drug and occasionally have additional low-cost prescriptions. High costs: I/we have multiple expensive medications of ongoing medical conditions. QUESTION 4 Describe how you are most comfortable paying your provider. Consistency: I/we prefer to have a relatively consistent budget from month to month even if it costs more in the end and know for example that an office visit will cost $10 or $20 and that no additional payment will be required. I/we prefer to pay higher contributions now and have lower out of pocket costs later. Once you have answered these questions you should look at each of the plans to determine which plan based on your answers would provide you with the level of coverage you and your family need at the cost you want to spend. 19 STEP 3 ENROLL WITH ADP WORKFORCE NOW After you have carefully reviewed your benefits and the enrollment tools and resources available to you, you are ready to enroll online via ADP Workforce Now. Keep in mind these important points: ENROLL Remember you must enroll in your health benefits within 31 days of your hire date. Be prepared before you start the process to enroll have ready your and your dependents SSNs, date of births and addresses available. Locate your user name and password to ADP Workforce Now. If you are not able to remember or find it, please contact the Payroll department to have it reset. All benefits will become effective on your hire date. New deductions will be on your first paycheck following enrollment. 25 STEP 4 MANAGE YOUR BENEFITS AFTER ENROLLMENT Even after enrollment, know what to expect, what steps to take and where to turn so that you can help ensure your health care needs are met. MANAGE WHAT YOU WILL RECEIVE Once you enroll on line you will receive an confirmation of your enrollment once approved. If there are questions or discrepancies related to your elections a Human Resource Specialist will contact you. You may log in to ADP Workforce Now and print out a copy of your 2018 elections as well under the Benefits Tab. CONFIRMING YOUR COVERAGE Be sure to review your first few payroll checks to confirm that your per-paycheck contribution amounts are correct. If you find inconsistencies with your elections or if you have questions about your coverage or pay deductions, please call your dedicated HR Specialist or Payroll. NEW IDENTIFICATION CARDS If you elect coverage you can expect to receive ID cards in the mail for your medical insurance plan. Dental and Vision cards are not needed; however, you can request a generic card through the HR Department. Note: if you need to see a doctor (or obtain a prescription) prior to receiving your ID cards on or after your eligibility date the provider may contact the carrier to verify eligibility. If you have any issues you may contact your dedicated HR Specialist. We ask that you refrain from making any appointments, if at all possible, until later in the month of eligibility.
