ABOUT THIS BENEFIT SUMMARY The information in this booklet is presented for illustrative purposes and was taken from various summary plan descriptions and benefit information. While every effort was taken to accurately report your benefits, discrepancies and errors are always possible. In case of discrepancies between this booklet and the actual plan documents, the actual plan documents will prevail. If you have any questions about your benefits, contact a member of the Human Resources Department. This guide discusses certain plans of Fabick Cat and participating subsidiaries as they apply to plan participants. Employees of such employers covered by other plans, or employees covered by collective bargaining agreements, will have benefits as prescribed by those plans and agreements.

2 Medical Plan Options Eligibility Regular full-time employees working 30 hours or more per week are eligible for benefits. New employees are eligible for Medical, Dental and Vision coverage on the 1st of the month following 60 days of employment. Spouses and children are eligible for Medical, Dental and Vision coverage. Eligibility for spouses and age rules for children coverage differ per plan. Please refer to benefit plan document. IHS Screening For 2016, a $10 medical plan premium discount is given per pay period to employees who participate in the IHS screening and enroll in Fabick s medical plan. A $20 medical plan premium discount is given if both employee and spouse participate in both the screening and the medical plan. Medical Benefits Coverage, choice, cost and convenience are factors each of us consider important when selecting a medical plan. Fabick is pleased to offer three separate medical plans through Healthlink, and one medical plan through Mercy (Missouri Employees Only). All four medical plans are designed to provide participants with access to quality, affordable health care by covering a wide range of services and supplies. You can also choose to waive medical coverage. Employee contributions are reviewed each year and have typically been lower when compared to other large companies in the heavy equipment industry. 3 Dental/Vision Plan Options Dental Benefits Fabick Cat offers employees the option to choose from two dental plans through MetLife Dental (PDP Plus Plan). This plan pays the cost of one vision exam every twelve months by a network provider and gives an allowance for most eyeglasses and frames. A discount on LASIK or FPK is offered through participating surgery providers. 4 Life/Disability Insurance & Leave Life Insurance Eligibility Employees are eligible for the following benefits on the 1st of the month following 60 days of employment. Benefit Life Insurance (Company Provided) AD&D Insurance (Company Provided) Voluntary Term Life Insurance Information A $15,000 life benefit is offered to employees who participate in the medical plan. Participants also receive $1,000 life insurance benefit for their spouse and child(ren). A $15,000 AD&D benefit is offered to employees who participate in the medical plan. Additional life insurance is offered to employees at reasonable rates in $10,000 increments up to a maximum of $500,000. Spouse and child(ren) benefits are available with a $250,000 max for spouse and a $10,000 max for child(ren).the insurance cost is based on employee s age. Refer to Employee Online for rate details. Disability Insurance Eligibility & Leave Employees are eligible for the following benefits on the 1st of the month following 6 months of employment. Benefit Short Term Disability Insurance (Company Provided) Parental Leave Bereavement Leave Information This benefit is 60% of base salary up to a maximum weekly benefit of $500 in the event of a non-work related disabling illness or injury. Disability situations are reviewed, approved and managed by a third party administrator. Birth and adoptive parents are able to take Family Medical Leave. Employees are provided paid time off in the event of a death of an immediate family member. 3

5 Work/Life Benefits Holidays Fabick Cat provides nine paid holidays per year. Bonus Plan Fabick provides a discretionary bonus plan on an annual basis for all employees and is based on Fabick s overall performance. Healthcare Savings Account Employees enrolled in the HSA Medical Plan Option are eligible to set aside up to $3,350 for single coverage or $6,750 for family coverage annually of pre-tax income for qualified medical plan expenses for you, your spouse and eligible dependents. Fabick provides an enrollment incentive of $500 for single coverage or $750 for family coverage and pays administrative fees on the account. There is a $1,000 catch up provision for employees 55 or older. Flexible Spending Accounts Dependent care and health care flexible spending accounts are available. Employees can set aside a maximum of $2,550 annually of pre-tax income for reimbursement of eligible health care expenses not otherwise covered and $5,000 annually for eligible day care expenses. Beneplace - Employee Discount Program All employees can access an online website with discounted movie tickets, theme park tickets, products, electronics, etc. Nurse On-Call/TeleDoc All employees can access a registered nurse or board certified physician 24-7 to get immediate answers to your health related questions. Wellness We have a variety of wellness programs including but not limited to nutrition counseling, interactive health screenings, lunch n learns, gym discounts and step programs. Gym Discount: Fabick will pay 50% of your gym membership (not to exceed $200/year). You will be paid quarterly and must show proof that you utilized the gym three times per week for each quarter. Employee Assistance Program All employees have access to free, professional counselors that are trained to help with a variety of family issues. Employee Referral Bonus Receive $1,500 for any person you refer who stays with Fabick Cat for at least six months. Will Preparation Services Will preparation services are available to employees that participate in the Voluntary Life Insurance program. This benefit provides one free visit with an attorney to put together a Will. Direct Deposit and ipay Statements Go paperless today by signing up for ipay online. It provides real advantages by giving you early access to view your paycheck. You can also access W-2 forms and use the paycheck calculator or W-4 assistant tool. 4
