2 DENTAL Buy-Up Option - (BLUE CROSS BLUE SHIELD OF NC) Dental Buy-Up coverage is also available for employees and their dependents to be paid by the employee. Employee: $5.18 per month/ $2.59 per pay period Employee/Child/Children: $39.98 per month/ $19.99 per pay period Employee/Spouse: $28.00 per month/$14.00 per pay period Family: $68.48 per month/ $34.24 per pay period Calendar year Deductible for Dental is $50.00 for each Individual and $ for Family. Benefit %: Preventive Services 100% Basic Services 80% after deductible Major Services 50% after deductible Orthodontia Services 50% after deductible Maximum Calendar Year Cost: $ for ALL Preventive, Basic and Major Services Lifetime Maximum for Orthodontia Services: $ VISION - (COMMUNITY EYE CARE) Employee: No Cost for the EE, coverage is paid by CBC/CCS at $3.79/month Employee/Child/Children: $2.74 per month deduction will be withheld from first paycheck Employee/Spouse: $5.20 per month deduction will be withheld from first paycheck Family: $8.95 per month deduction will be withheld from first paycheck Eye Exam once a year ($15 co-pay) Contact lens fitting, re-fit or evaluation once a year ($15 co-pay) Eyewear allowance of $ (per person) every 12 months ($25 co-pay) The allowance can be applied to frames, spectacle lenses, contact lenses, special lens options, or any combination. As long as you select eyewear having a retail price that s less than or equal to your allowance, your only out of pocket expense for the eyewear is the $25 co-pay. AFLAC Employees are offered a supplemental insurance coverage with AFLAC. Any and all coverage elected will be paid strictly at the expense of the employee through payroll deduction. 401(k) - Transamerica All employees are encouraged to participate in the 401(K) retirement program. The CBC/CCS matches up to 5% and the employee is 100% vested. Employees are eligible to contribute upon being hired with no waiting period. Maximum percentage allowable is 100% - Maximum dollar amount allowable for employees under the age of 50 years of age is $ and employees over the age of 50 years of age is $ for Catch-Ups are allowed for employees over age 50, please consult the HR Office for details. TRIBAL PENSION BENEFIT Is a Defined Benefit Plan, with 1.5% of final average compensation, and is 100% employer funded for a monthly retirement income. Employee has to work 5 years in a Tribal Entity to be 100% vested.

3 MetLife Life/Short-Term/Long-Term/AD&D Insurance - Life Insurance coverage on each employee is based on 3 times the employee s yearly salary rounded to the next $ Premiums for this coverage are paid by CBC/CCS. - Accidental Death coverage on each employee is 6 times the employee s yearly salary rounded up to the next $1000. Premiums for this coverage are paid by CBC/CCS. - Supplemental Life Buy-Up Option coverage is offered in increments of $10,000 to a maximum of $100,000. Premiums would be paid by the Employee. - Dependent Life- Buy-Up Option coverage is offered for Spouse in increments $5,000 to a maximum of $20,000 and for dependent children in increments of $2,000 to a maximum of $10,000. Premiums would be paid by the Employee. - Short-Term Disability coverage is 60% of weekly salary up to $300 (1 st 26 weeks of disability). Premiums for this coverage are paid by CBC/CCS. - Short-Term Disability Buy-Up Option coverage is up to $300 additional benefit based on annual salary. Premiums would be paid by the Employee. - Long Term Disability coverage is 60% of monthly salary up to $5,000 (this starts after 26 weeks of short term disability). Premiums for this coverage are paid by CBC/CCS.

4 FREQUENTLY ASKED QUESTIONS: - WHEN WILL MY BENEFITS BECOME EFFECTIVE? Employees hired Full Time will need to complete a 60 day probationary period before benefits become effective. (Example- An Employee hired on 3/18/16, 60 days = 5/17/16- benefits will not be effective until 6/1/16.) A New Hire Orientation on the benefits package will be scheduled before benefits become effective for employees. - WHO IS YOUR HEALTH & DENTAL CARE PROVIDER? BCBSofNC - WHO IS YOUR VISION PROVIDER? Community Eye Care - WHO IS YOUR LIFE INSURANCE PROVIDER? METLIFE Insurance - WHEN WILL MY 401K GO INTO EFFECT ONCE I BEGIN EMPLOYMENT? Employees are eligible to contribute upon being hired with no waiting period. - WHAT IF I NEED TO MAKE CHANGES TO MY INSURANCE COVERAGE THROUGHOUT THE YEAR? Changes can be made outside of the Open Enrollment period only if it is a qualifying event. In order to add or drop coverage the employee will need to provide documentation in regards to the changes. (birth certificate, adoption certificate, death certificate, marriage certificate) Employees have a 30 day waiting period to add or drop a dependent from their coverage with the documentation. o Birth, Adoption o Marriage, Divorce, Legal Separation What is Open Enrollment? Open Enrollment is when all Full Time employees with benefits are allowed to make any necessary changes to their insurance coverages. The changes will be effective January 1, The Open Enrollment event is held in the month of November each year. All Full Time employees will be issued an Open Enrollment packet that tells all information in regards to the benefits package and any changes to the benefits. The front page of the packet will state the actual dates, times and location of the Open Enrollment event. Any changes that are made during this time will become effective January 1, What is the Employee Wellness Incentive? Employees who are eligible or will be eligible for BCBSofNC health benefits, by 12/31/17, have the opportunity to earn an Employee Wellness Incentive. Each Full Time Permanent employee, who receives health benefits and submits a Wellness Incentive Certification Form to the EBCI Benefits Offices that verifies the wellness requirements have been satisfied, shall receive a Wellness Incentive, in the form of a $ gift card. Employees must complete the following requirements by December 31, 2017, to be eligible for the Employee Wellness Incentive. Wellness Incentive Forms may be picked up at the CBC HR Office or at the CCS nurses office for distribution.

5 DOES YOUR COMPANY RECEIVE ANY SPECIAL DISCOUNTS? Employees are able to receive a discount with Verizon Wireless- Employees will need to contact Verizon Wireless in regards to how to receive the discount. (a current check stub is usually needed to show that you are an employee of the Cherokee Boys Club/Cherokee Central Schools) Full Time Permanent employees may also purchase materials or services from the Cherokee Boys Club Garage Department. If an employee makes a purchase from the CBC which is at least $ and not over $600.00, the employee may elect to go on to payroll deduction for up to five pay periods to pay the amount owed. (an approval form from the Garage Department will need to be signed off from the employees department Manager or Supervisor before the transaction is made) If an employee makes a purchase through the CBC for an item which the CBC does not normally sell, this amount must be paid in full at the time of purchase and, therefore, cannot go on payroll deduction. DO YOU HAVE A SUPPLEMENTAL RETIREMENT PROGRAM? There is no Supplemental Retirement Program set up for employees. WILL I HAVE HEALTH INSURANCE COVERAGE AFTER I RETIRE? There is no Health Care coverage for employees that retire. Employees that retire have the option to pick up the COBRA coverage for 18 months, Employees will be responsible for paying for their own coverage. IF I TERMINATE, WHEN WILL MY BENEFITS STOP? Benefits will terminate the date of the actual termination, resignation or retirement.
