1 CANTON UNION SCHOOL DISTRICT 2018 Open Enrollment for All Insurance Plans and Flex Benefit Plans The Open Enrollment period for the Canton Union School District #66 Health, Vision, and Dental Insurance plan will be November 15 th -30 th, for coverage during Employees who are eligible for insurance coverage will be able to make changes to their current insurance plan(s) or add/drop coverage, with an effective date of January 1, 2018 during that time period. This is the only time during the year that changes can be made to your plan elections without the occurrence of a life changing event. Open enrollment will be completed online this year, through the Bernie Portal. The portal will include important information about new benefit options and programs available to you for All employees who are eligible for insurance will need to complete the open enrollment process in Bernie Portal, even if there are no changes to your current benefit elections. Instructions for accessing and utilizing Bernie Portal will be provided to you prior to November 15 th. An All-District Meeting will be held at 3:45pm on Tuesday, November 14 th in the High School Auditorium, at which time Stephanie Jackson from J.L. Hubbard Insurance and Bonds, Inc. will present the 2018 benefits and instructions for the online enrollment process. BCBS HEALTH PLANS No change to benefits for 2018 There will continue to be two health insurance plan options through Blue Cross Blue Shield in One will be a $1,000 deductible plan (PPO). The other is a $2600 Qualified High Deductible plan. Employees that enroll in the high deductible plan may be eligible to contribute to a Health Saving Account in order to pay for medical, dental, and vision expenses with money payroll deducted to the HSA account on a pre-tax basis. The summary of benefits for these two plans is attached, so that you can make comparisons and choose the best plan for you/your family. The 2018 premiums for the health insurance plans and dental and vision plans are included at the end of this memo. The CUSD self-funded health insurance plan was extremely fortunate to receive only small adjustments to insurance premiums for 2018, including reductions for those with Employee Only coverage on both plans. This is due in part to members using in network providers and carefully reviewing claims to verify accurate billing for services provided. There were two plan changes implemented in 2017 (affecting both plans) that will continue to apply for 2018: CVS is not a part of the pharmacy network. Specialty drugs must be ordered through Prime Specialty Pharmacy Program. NEW for 2018: All district BCBS health plan participants, including spouses and children, will be eligible to sign up for the ProtectMyID Experian identity theft protection service, provided through BCBS at no cost. Information will be available in Bernie Portal and also in your BCBS member portal on Information regarding other BCBS programs and resources that are available to you will be posted in Bernie Portal and presented at the meeting on November 14 th.

2 DENTAL, VISION, and LIFE INSURANCE - Changes for 2018 As a result of increases on the Guardian renewal offer for 2018, the Insurance Committee reviewed competitive proposals from several other insurance carriers and will be transitioning the dental, vision, and life insurance to the following carriers effective : DENTAL INSURANCE Delta Dental PPO Plus Premier Plan - new features include: Enhanced Benefits Program To Go Carryover Benefit Dental Implant coverage VISION INSURANCE Dearborn National Voluntary Vision Plan- new features include: EyeMed Select Vision Provider Network Lower Exam and Materials Copays Higher Frame and Contacts Allowances Members may use their frame and lens benefits (either contacts OR spectacle lens) in the same benefit period. If you use your contact allowance and want to purchase glasses, you will get your frame allowance and then receive a 20% discount off the spectacle lens. LIFE INSURANCE As of , Dearborn National will be the insurance company for the Life and Accidental Death and Dismemberment coverage that the district provides for you at no cost. VOLUNTARY LIFE INSURANCE New Benefit for 2018 For 2018, you have the opportunity to purchase additional amounts of term life insurance through Dearborn National for yourself, spouse, and/or children. Coverage includes a matching Accidental Death and Dismemberment (AD&D) benefit. If you enroll for coverage, you may elect up to the Guarantee Issue amount without answering any medical questions. Semi-monthly premium costs will be available to you in Bernie Portal based on the amount of coverage selected; you must elect coverage for yourself in order to elect any coverage for your dependents. Voluntary Life/AD&D Guarantee Issue Amounts (Not subject to medical questions): Employee: $150,000 (may elect coverage in $10,000 increments) Spouse: $20,000 (may elect coverage in $5,000 increments but cannot exceed 50% of Employee coverage election) Child(ren): $5,000 per child (reduced for children age 6 months or less) If you do not elect voluntary life coverage during Open Enrollment, any future elections will be subject to medical underwriting and are not guaranteed to be approved. Additional amounts (above the Guarantee Issue) are available for purchase, but you must complete the Evidence of Insurability form and be approved by Dearborn National.

3 FLEXIBLE SPENDING ACCOUNT (FSA) - HEALTH CARE AND DEPENDENT CARE As part of the online enrollment process, employees will be required to enroll or decline to participate in the district s 2018 Flex (FSA) plan. Benefit Planning Consultants (BPC, Inc.) will continue to administer the district s FSA plan. For 2018, employees may payroll deduct up to $2650 for Health Care FSA. This is money you set aside pre-tax to pay for unreimbursed medical expenses such as PPO deductible/coinsurance, copays, Rx copays, and dental and vision expenses. (If you enroll in the $2,600 high deductible insurance plan and open a Health Savings Account, you are not eligible to enroll in the Health Care FSA plan.) You may also payroll deduct up to $5000 pre-tax for Dependent Care expenses. FSA and Dependent Care FSA are use it or lose it benefits. Documents for the Flex Benefit plan will be available to you in Bernie Portal. AFLAC Any changes to AFLAC must also be completed during Open Enrollment November 15 th -30 th ; however, AFLAC will not be part of the online enrollment process.
