2 2018 Open Enrollment Open enrollment is the time of year to change your benefit selections without penalty. Open enrollment at Labouré College is November 28 th December 8 th, All paperwork must be into HR no later than December 8 th During the open enrollment period Labouré College employees may: Enroll themselves and eligible dependents in the medial, dental programs as well as enroll in voluntary benefits. Drop medical and/or dental Change election tier from single to single + or family Employee To Do List Complete the Harvard Pilgrim medical enrollment form If new to dental, fill out the Delta Dental enrollment form If you would like to participate in the Mutual of Omaha Voluntary Life program fill out an enrollment form If you would like to participate in the PetAssure/PETplus program fill out the enrollment form. Complete a 2018 FSA election form November 20,

3 Renewal Summary Life and Long Term Disability For 2018 Mutual of Omaha will be our Basic Life and Long Term Disability partner. Effective January 1, 2018 the basic life benefit is now 3x Annual Earnings to a maximum of $500,000. There will be no changes to the Long Term Disability Benefit Labouré College will continue to pay 100% of the premium for Basic Life and Long Term Disability. Voluntary Life and AD&D For 2018 Mutual of Omaha will be our Voluntary Life/AD&D partner. This coverage is 100% employee paid. Voluntary Short Term Disability, Accident and Critical Illness Insurance No change for 2018 These coverages are 100% employee paid. Voluntary Vision No Change, renewing with NVA Dental Insurance Renewing with Delta Dental of Massachusetts No plan design changes Medical Effective January 1, 2018 Labouré College will move to Harvard Pilgrim Health Care Pet Assure We will be introducing Pet Assure Coverage is 100% employee paid November 20,

9 Vision Benefits National Vision Administrators No change in schedule of benefits Eligible members and dependents are entitled to receive one (1) pair of lenses and a frame or contact lenses and contact lens evaluation/fitting once every calendar year. If you visit a participating provider you will receive a richer benefit Lenses: NVA provides coverage in full for standard glass or plastic eyeglass lenses. Frames: Select any frame from the participating provider s inventory. Any amount in excess of your plan allowance is the member s responsibility. Contact Lenses: The contact lens benefit includes all types of contact lenses such as hard, soft, gas permeable and disposable lenses. Medically necessary contact lenses may be covered with prior authorization when prescribed for: post cataract surgery, correction of extreme visual acuity problems that cannot be corrected to 20/70 with spectacle lenses, Anisometropia or Keratoconus.
