1 S Dubuque/Dyersville, Iowa January 1, 2014 HEALTH & WELFARE S PROGRAM Adoption Assistance Program Dental Insurance: Delta Dental of Michigan Reimbursement up to $3,000 per child for eligible expenses that are incurred during the adoption process. 100% Usual, Customary & Reasonable (UCR) coverage for preventive, 80% diagnostic and other covered services, orthodontics ($1,000 max a lifetime). Plan pays $1,000 max per year per person. and Employee Assistance Program (EAP) Flexible Spending Accounts Health Care Dependent Care Premium Account (health, vision, dental are pre-tax) All associates and eligible immediate family members. EAP is a short-term counseling benefit provided to help you deal with family or work/life issues that might be causing your life to feel out of balance. EAP offers up to five free counseling sessions. Pre-tax salary reductions to reimburse for eligible expenses. may elect to contribute pre-tax dollars each pay period up to an annual maximum : Dependent Care $5,000 Health Care $2,500

2 HEALTH & WELFARE S PROGRAM Legal Plan Hyatt Legal Plans Coverage for legal advice and assistance for most personal legal matters (employment and businessrelated matters are excluded from coverage). The Plan does not impose waiting periods or co-payment requirements. Life Insurance Basic Coverage Basic life coverage equal to 2x for FT and 1x for PT of the associate s base annual salary for both basic and accidental death and dismemberment insurance (AD&D). Life Insurance Supplemental Coverage Option to purchase 1, 2, 3, 4, or 5 times annual salary subject to plan maximums and medical Evidence of Insurability requirements. Life Insurance Spouse/Dependents Option to purchase coverage for an eligible spouse in increments of $10,000, $20,000, $50,000 or $100,000. Evidence of Insurability required for greater than $20,000. Option to purchase coverage for your eligible children in increments of $2,000, $5,000, $10,000 or $20,000.

3 HEALTH & WELFARE S PROGRAM Short Term Disability Long Term Disability Medical Insurance: Health Choices High Plan and Standard Plan Short term disability coverage equal 60 percent of base pay. Benefits begin after you are unable to work for at least 14 calendar days due to illness or hospitalization, and continue for up to 180 days (this includes the waiting period) Long term disability replaces monthly income of up to 60% beginning at 6 months (180 days) from date of disability. Benefits are payable to age 65 or longer, depending upon your age at time of disability. You have the option to buy up LTD coverage to 66 2/3 percent of base pay. PPO plans providing in-network and out-of network benefits for medical needs and prescription drugs. Buy-Up - and Medical Plan Discount Program 403(b) Retirement Savings Plan Employer Match SUSPENDED EFFECTIVE JANUARY 1, 2013 One year of continuous service s meeting eligibility requirements. s meeting eligibility requirements and participating in and contributing to the 403(b) Retirement Savings Plan. Depending upon income and family size, you may be eligible for a 25% or 50% discount on medical plan contributions. Voluntary tax deferred payroll deduction may be made to the Retirement Savings Plan. The service provider utilized is Transamerica Investment Advisors. Effective January 1, 2013, Matching contributions have been suspended. The program will be reviewed annually. and

4 HEALTH & WELFARE S PROGRAM Retirement Plan 2 Plans Defined Benefit Cash Balance Pension Plan Grandfathered Defined Benefit Plan s meeting eligibility requirements. Effective July 1, ) Defined Benefit Cash Balance Pension Plan: Same vesting requirements as current plan. The Cash Balance Plan benefit is earned in terms of a stated account balance, which once associate is vested can be paid as a lump sum upon termination of employment or can be converted to an annuity at retirement. Benefits earned after June 30, 2010 will include Pay Credits and interest at a set rate. Pay Credits will be based upon a points system that provides between 3% to 7% of pensionable pay annually. 2) Grandfathered Plan: Defined Benefit Plan that provides a monthly annuity at normal retirement age (65) once a member is vested at termination. Vesting occurs after 5 calendar years in which you work 1000 or more hours. The benefit is based upon final average earnings, years of service and age when retirement benefits begin. Vision Insurance: United Healthcare Vision High Plan and Standard Plan PPO plans providing in-network and out-of network benefits. Status changes that affect benefit eligibility. Effective date is first day of the pay period following

5 TIME OFF S Paid Time Off (PTO) Holiday Benefit OF ELIGIBILITY begins accruing PTO from date of hire and is eligible immediately to use PTO. 6 Fixed Holidays New Year's Day, Memorial Day, Independence Day, Labor Day, Thanksgiving and Christmas. 1 Floating Holiday (offers associate flexibility) PTO combines vacation, personal, sick and accrues each pay period to use for time off with pay. PTO accrual rate is determined by years of service. No waiting period to receive benefit. Holiday benefit is paid based on FTE status (i.e. 72 hrs per pp = 7.2 hrs) of benefit. Paid automatically on the legal/observed holiday PTO Cash Out Determined per policy PTO Eligible s Cash-out of up to a maximum of 40 PTO hours. Election is made during Open Enrollment. Payout occurs in the following calendar year in December. Bereavement/Funeral Leave Jury Duty Up to 3 days of paid time off to cope with the loss of an immediate family member meeting policy criteria; 1 day for other family member. Payment is provided only for the scheduled hours you are absent from work due to jury duty.

Credit Union All s Payroll deduction to Health Services Credit Union. Direct Deposit All s Convenient deposit of your N/A payroll earnings to your personal account. Pharmacy and Supplies ( Family Pharmacy) Tuition Assistance Wellness/Health Clubs All s Eligible for over-the-counter (OTC) discounted medications. 3 months of benefit-eligible Reimbursement of tuition employment expenses with calendar year max.
