The Missouri Botanical Garden (Garden) offers a comprehensive, cost-effective, and competitive benefits package to help promote your health and well-being. It is your responsibility to make educated choices and select the plans that are right for you and your family. This Open Enrollment Guide will serve as a reference to your benefit choices and assist you as you complete your online enrollment for your 2017 benefit selections. 3 WHO IS ELIGIBLE? Full time employees working 37.5 or more hours weekly are eligible to participate in all benefits. Part time employees regularly scheduled to work on average 30 or more hours per week or who on average work at least 30 or more hours per week in a 12 month measurement period may become eligible for medical coverage and FSA participation. If eligible, you will have a PPACA Enrollment wizard to complete. With certain benefit options, you may cover your eligible dependents, which include your legal spouse, your children to the age limit of 26 (in most cases or as set by the policy), and your domestic partner (eligible for medical only). HOW TO ENROLL If you click on the Benefits icon in Dayforce and have an Open Enrollment link or a PPACA Enrollment link, you MUST enroll by November 16, WHEN TO ENROLL The open enrollment period runs from November 2 nd through November 16 th. Educational sessions will be held at the Monsanto Center, 1 st floor Lecture Hall: Wednesday, November 2 2:00 to 3:30 pm Thursday, November 10 9:30 to 11 am Dayforce Enrollment Assistance: HR Conference Room HOW TO MAKE CHANGES Unless you have a qualified life event, you cannot make changes to the benefits you elect until the next open enrollment period. Qualified life events include: marriage, divorce, legal separation, birth or adoption of a child, change in child s dependent status, death, change in employment status or change in coverage under another employer-sponsored plan. Changes must be submitted via the Dayforce Life Event Declaration Form and then the Life Event Wizard within 30 days of the qualifying event. 3
4 WHAT S NEW FOR 2017? MOVING TO ANTHEM Medical insurance will be offered through Anthem beginning January 1, We are excited that we were able to keep the majority of the benefits the same and many even better for our employees. Please take the time to review the information provided to you regarding the benefits offered through Anthem. OPEN ENROLLMENT USING DAYFORCE The 2017 open enrollment will be completed through Dayforce. Additional information is included in this guide on how to navigate through Dayforce. Additional enrollment assistance will be available in the HR Conference Room. Before visiting the HR Conference Room, call ext. 260 or hrbenefits@mobot.org. 5 MEDICAL AND PRESCRIPTION DRUGS Effective January 1, 2017, the Garden medical insurance will be provided by Anthem. During this transition you will want to proactively work with Anthem on continuation of care (this applies if you are currently in the middle of a procedure or ongoing medical care) and transition of prescriptions that may be subject to step therapy. Go to to search for a provider in the Blue Access Choice network, review the formulary, and track your claims and healthcare costs. 6 MEDICAL AND PRESCRIPTION DRUGS QHDHP HRA Buy Up HRA Garden Funding First-time enrollees receive $500 from the Garden to their HSA Account The Garden will fund $500 toward the last part of the innetwork deductible. The Garden will fund $750 (individual) or $1500 (family) toward the last part of the innetwork deductible as well as the additional coinsurance to the out of pocket maximum. If you select coverage for a spouse or DP you must submit a completed Spouse/Domestic Partner Coverage Affidavit by 11/16/16. The Affidavit is attached to each medical plan in Dayforce and is posted on the HR link of the MBG Intranet Benefits section. 6

9 LIFE, AD&D & LONG TERM DISABILITY The Garden pays the full cost of the following benefits for you: life insurance, AD&D, and long-term disability insurance. In the event of your death or if you become disabled from a non work-related injury or sickness, you or your beneficiaries would receive the following benefit. Life and AD&D Insurance 2 times Annual Salary Maximum of $500,000 Long Term Disability Monthly Benefit 60% of Monthly Pay (to a maximum monthly benefit) Elimination Period: 90 Days Benefit Duration: Age 65 *As long as the definition of disability is met OPTIONAL LIFE INSURANCE Additional life insurance is available for you to elect on yourself, your spouse and your dependent children. You pay the cost for this coverage, but it is made available to you at discounted group rates. For yourself, coverage is elected in $10,000 increments to a maximum of the lesser of $500,000 or 5 times your salary. The guaranteed issue amount is $150,000. AGE REDUCTION Coverage amounts for Life Insurance will reduce to 65% of the original amount at age 70, and to 50% of the original amount at age 75. This applies to both the employer paid and the optional life benefit. You may also elect optional life coverage for your spouse and dependent children. For your spouse, you may elect coverage in $5,000 increments to a maximum of $500,000 but not to exceed 100% of your employee optional life election. The guaranteed issue amount is $25,000. You may elect coverage for your dependent children in $2,000 increments not to exceed $10,000. If electing dependent coverage, all of your eligible dependent children would be covered. To enroll in Optional Life for your spouse/child, you must elect coverage for yourself. If you would like to enroll in this coverage for you or your dependents and it is not your initial enrollment period, you will be subject to medical underwriting or evidence of insurability (EOI). If you are currently enrolled but would like to increase your coverage for you or your spouse, you may increase your benefit amount to the guarantee issue amount without being subject to medical underwriting. Any amount over the guarantee issue amount is subject to medical underwriting. You may increase or enroll your children up to a maximum benefit of $10,000 without being subject to medical underwriting. The EOI form is attached to the Optional Life Plans in Dayforce or on the HR link of the MBG Intranet Benefits section. 9
10 OPTIONAL SHORT TERM DISABILITY You have the opportunity to purchase Short-Term Disability (STD) insurance through Aetna. You are responsible for the entire cost of this benefit. Your STD plan provides benefits when a non work related illness or injury has rendered you unable to work. Days before benefits begin: Injury/illness 21 days Benefit duration period: Up to 10 weeks Income replacement level: 60% of weekly earnings Maximum weekly benefit: $1,000 More details and the EOI form are available in Dayforce enrollment wizard or on the HR link of the MBG Intranet EOI will be required for the first time late enrollment (more than 31 days from your hire/benefit eligibility date). FLEXIBLE SPENDING ACCOUNTS (FSA) FSAs provide you with an important tax advantage that can help you pay health care and dependent care expenses on a pretax basis. By anticipating your family s health care and dependent care costs for the next year, you can actually lower your taxable income. HEALTH CARE FSA allows employees to pay for certain IRS-approved health care expenses with pretax dollars. The maximum contribution of $2,600 can be used for eligible health care-related expenses, including medical, dental and vision expenses. DEPENDENT CARE FSA allows employees to use pretax dollars toward the cost of care for eligible dependents, commonly daycare for children under the age of 13 or care of elderly dependents or members of your family with disability so you and/or your spouse can work. The annual maximum amount you may contribute to the Dependent Care FSA is $5,000 (or $2,500 if married and filing separately) per calendar year. IRS RESTRICTIONS both plans are subject to a use it or lose it rule meaning any money remaining in your account at plan year end is forfeited. Additionally, contributions cannot be changed throughout the year without a qualifying Life Event. The plan provides a convenient debit card, which automates the process of paying for eligible expenses. You may also submit claim forms to J.W. Terrill Benefit Administrators. 11 HEALTH SAVINGS ACCOUNTS (HSA) A Health Savings Account (HSA) is a tax-advantaged account that you can put money into to save for future health care expenses. You can contribute to an HSA if you: Have coverage under an HSAqualified qualified high deductible health plan (QHDHP) Have no other first-dollar medical coverage Are not enrolled in Medicare Cannot be claimed as a dependent on someone else s tax return HSA funds can be used to pay for eligible medical expenses, as well as eligible dental and vision expenses for you or your dependents. Your HSA account is owned by you, which means that you can take it with you if you change jobs or retire. Any unused money at the end of the year rolls over/remains in your account. For a complete list of eligible expenses please visit The IRS imposes a maximum contribution limit to the HSA on a calendar year basis. The 2017 maximum is $3,400 for an individual and $6,750 for a family. If you are age 55 or older, you are eligible for an additional catch up contribution of $1,000. The maximum contributions include both employee and employer money. Any amount used for purposes other than to pay for qualified medical expenses are taxable as income and subject to an additional 20% penalty. You are free to choose any bank for administration of your HSA. The Garden s preferred bank is BMO Harris Bank. Submit the MBG HSA Authorization form (attached to the QHDHP plan in Dayforce and available on the HR link of the MBG Intranet) to Liz.Barker@mobot.org by 11/16/16 to elect your 2017 HSA contribution or to change your contribution throughout the 2017 calendar year. Note: You must have an HSA account and authorize a minimum contribution of $1.00 to be eligible to receive the one-time Garden contribution of $500 available for first-time HSA enrollees. 11
12 403(b) RETIREMENT SAVINGS PLAN New full-time employees and part-time employees who work at least 1,000 hours per year will be automatically enrolled in the 403(b) Retirement Savings Plan at a 4% deferral with a 2% per year automatic increase election (not to exceed 10%). Participants may choose investments offered through the Plan, or if they do not choose where to invest contributions the default investment will be in the Principal Lifetime funds. Participants can: Opt out and not contribute to the plan; Opt out of the automatic increase election; and/or Change their contribution percent at any time utilizing the Principal Financial Group website or the Principal Tele touch phone system at The Plan number: Participants may make pre-tax or ROTH salary reduction contributions in multiples of 1% up to the statutory maximum. The Garden makes matching contributions on behalf of each eligible employee who has reached age 21, has met the one year service requirement and contributes to the Plan. Wages deferred into the plan by eligible participants will be matched dollar for dollar up to a maximum of 6% of your annual eligible pay. Eligible pay is limited under IRS rules; as a result, matching contributions are limited as well. Vesting is immediate. 15 DAYFORCE ENROLLMENT TIPS Click on the Benefits icon on your home screen. Click Start Enrollment on your Open Enrollment Update your Current Beneficiary Information Update your Current Dependent Information Complete the Tobacco Use Declaration Click Next to begin your Benefits Open Enrollment A green check mark appears next to your current election (The new medical plans will not have this since they did not exist for 2016 & if you waived enrollment prior to Dayforce, the waive option will not show a green check). You must place a check mark to the left of to the option you wish to enroll in (including waiving a plan if you don t want to enroll). Click the link on any plan to view plan summary documents or other important documents relating to that plan. Click the button on any plan to update covered dependents, update beneficiaries assigned to that plan or to change the coverage amount on life plans. You may click the button at the bottom or top of the enrollment pages at any time to save your progress and finish it later. You will be provided with an enrollment summary to print once you ve submitted your enrollment. The enrollment wizard will remain open until midnight Central Time on the last day of open enrollment. Should you need to change your enrollment, log back in, click the start enrollment button again and you can resubmit your enrollment. Please note the green checkmark will appear next to your original open enrollment selections so you know what you selected prior to your change but you will need to reselect each item again. 15
