4 1 Overview This BRD describes the new rates for the University s Health and Welfare (H&W) Rates to be included with the advance transactions in the year-end processing schedule for the Payroll and Personnel System (PPS). The non-bargained rates correspond to the rates provided for the AYSO Open Enrollment (OE) 2017 application. Supplemental life insurance rate changes have also been incorporated into the BRD, so they can be included with the other advance rate transactions. 7 Table After final 2017 H&W plan rates have been determined, HR&B will upload a comprehensive file and request that production control run PIP Job PIPRATEU. This job will use the file to generate BRT update transactions and deliver them to a file in MVS. The transactions should be processed into the BRT of a test instance of PPS for verification. For the following plans and the unrepresented bargaining unit, the transactions should include the following (no MCB tiers) 2017 Legal Rates Plan J2 Bargaining Unit Dental Rates Plans D1, D3, P3, P4 Bargaining Unit Vision Rates Plan VI and P5 Bargaining Unit 00 R0003 System-wide Bargaining Unit Exceptions Distributed to all campuses: For the following plans and bargaining units, the transactions include the following, tiered by 2017 MCB values of 53, 104, 156, and 999 (no restrictions): 2017 Medical Rates, Plans AC, HB, KN, KS, AU, AP, WH; Bargaining Units BX, DX and LX. These standard default rates apply to any combination of bargaining unit, health plan, or pay band that is not described as an exception. Bargaining unit IX has agreed to the 2017 bargaining unit 00 rates and Pay Bands. They do not have or require rows in the rates table; they will currently default to the bargaining unit 00 rates and Pay Bands. Bargaining unit PA does not currently have rows in the rates table; they will default to bargaining unit 00 rates and Pay Bands (no unique rows needed). Bargaining unit PX plans are the default rates for the post-doc plans. For the following plans and bargaining units, employee nets are limited to no more than a $25/month increase for each covered category, using prior year's employee monthly premium as benchmark for determining the $25 threshold. This applies to 2015, 2016, and 2017 plan years. For 2017 the rates did not exceed threshold, so 00 unit rates apply Medical Plans HB, KN, KS; Bargaining Units HX, NX, RX, and TX For the following plans and bargaining unit, Pay Bands 1 and 2 nets are frozen at 2014 nets for plan year 2015, 2016, and Pay Bands 3 and 4 nets were held to no more than a $25/month cap over prior year: Medical Rates Plans, HB, KN, KS; Bargaining Units EX For the following plans and bargaining unit, Pay Bands 1 and 2 nets are frozen at 2014 nets for plan year 2015, 2016, and 2017: Medical Rates Plans, HB, KN, KS; Bargaining Unit SX SR Open Enrollment 2017: Payroll Rate Advance Transactions 7

8 For the following plans and bargaining unit, employee nets are allowed to increase by no more than 20% over the dollar amounts established in the previous year. This applies to Plan years 2013 through If an employee net for either plan would exceed 20%, UC contributes the amount in excess of 20%. Benchmark for determining the cap is the full amount of the prior year's employee contributions, (i.e., the combination of the actual employee contribution plus UC subsidy); for 2017 the rates did not exceed threshold so 00 unit rates apply Medical Rates Plans HB, KN, KS; Bargaining Unit CX R0004 Local Bargaining Unit Exceptions Distributed to specific campuses: For the following local bargaining units 00 -unit rates and Pay Bands apply. 9 6 Attachment Informational Reference: Medical Plan Rate Rule Set and Contract Language by Bargaining Unit Code Ruleset/Contract Language BX CX In each subsequent calendar year of this Agreement (2013, 2014, 2015, 2016 and 2017), when gross rates increase, the University may increase the monthly employee contributions in the Health Net Blue & Gold Plan and in the Kaiser plan by twenty percent (20%) over the dollar amounts established by the University for the preceding year without meeting and conferring. EX In the event the University establishes employee contribution rates for either of these two plans that exceed this twenty percent (20%) threshold, the University will contribute the amount of the increase that exceeds (20%). The subsequent year s cap will be determined using the full amount of the prior year s employee contributions, i.e., the combination of the actual employee contribution plus the UC subsidy. The University may not increase monthly employee contributions for the Health Net Blue and Gold Plan and for the Kaiser plan for pay band 1 and pay band 2 for plan year 2015, plan year 2016, and plan year F3 F7 GS HX For pay bands not listed above, beginning in calendar year 2015, increases in employee contribution rates for the Kaiser and Health Net Blue and Gold plans shall not exceed $25 per month (up to aggregate increase of $300/year) over the prior year for each year of the agreement.
