8 OE General Information OE dates: Monday, October 29 through Friday, November 9 Active enrollment (Tobacco Certification) Enroll in OneUSG Connect Benefits system or contact the OneUSG Connect Benefits call center University System of Georgia 7

9 Tobacco Certification All employees on a USG healthcare plan, as well as each of their covered dependents (age 18 and older) must certify/recertify their tobacco use status in order to avoid being charged the $75 (per person) surcharge fee. Those who do not certify/recertify their tobacco use status during open enrollment will be defaulted as a tobacco user and a $75 surcharge (per eligible dependent) will be added to their premium.
