11 UC Health Savings Plan Health Savings Account (HSA): Individual Contribution Limit will increase from $3,350 to $3,400 Minimum threshold balance for investing HSA funds is changing from $2,000 to $1,000
12 UC Health Savings Plan & Core Travel benefit maximums for transplants, transgender and bariatric surgery will change from $5,000 one-time maximum to: $10,000 per transplant at a designated Center of Medical Excellence or Blue Distinction Center for Specialty Care $10,000 per transgender surgery or series of surgeries $5,000 per bariatric surgery at a designated Center of Medical Excellence or Blue Distinction Center for Specialty Care

22 Dental Plans Both plans continue to be fully paid by UC on behalf of employees Delta Dental continues as the carrier PPO New coverage of risk-based second oral exam per year Orthodontia $1,500 max for children is applied as long as treatment initiated prior to 26 th birthday and billed within one year DHMO Some slight benefit enhancements due to dental coding changes
23 Vision Plan VSP continues as the carrier Active Employees No benefit changes No change to premium rate Plan continues to be fully paid by UC on behalf of employees
24 AD&D Plan Prudential will be carrier in 2017 No change to rates for active employees Slight benefit changes to the plan Addition of benefits for Felonious Assault and Bereavement/Trauma Counseling See plan booklet for other minor changes Always open for enrollment

27 Health FSA Reminder The Health FSA maximum contribution amount is $2,550 You must re-enroll if you want to participate in 2017 You must contribute a minimum of $180 per year If you enroll in the UC Health Savings Plan, you cannot enroll in the Health FSA
28 Health FSA 2016 timeline You have until December 31, 2016 to incur eligible expenses for reimbursement from 2016 plan year funds The 2016 plan year has a run-out period which provides additional time to submit claims for eligible expenses incurred in The run-out period is January 1, 2017 through April 15, 2017 The deadline to file claims for expenses incurred in 2016 is April 15,
29 Health FSA - carryover reminder You may carry over up to $500 of unused Health FSA funds into the next plan year Carryover balance is determined after the April 15 run-out period (early May of the following plan year). Unused funds greater than $500 are forfeited
30 Health FSA: effective 2017 Plan Year If you do not re-enroll in the Health FSA for 2017: You must have a minimum of $25 in your account to be able to carry over remaining funds into the next plan year. Remaining funds under $25 are forfeited Remaining funds will only be carried over for one plan year.
31 Health FSA: effective 2017 Plan Year IRS Guidance Carryover and COBRA Prior to the IRS guidance, separated participants could not carryover funds unless they continued coverage through COBRA Starting in 2017, employees enrolled in the Health FSA who experience a COBRA qualifying event now have access to carryover funds similar to employees who do not re-enroll in the Health FSA Must have a $25 balance for funds to carryover Funds under $25 are forfeited Funds will only carryover for one plan year
32 Dependent Care FSA Unlike Health FSA, no change to grace period extension (no carry-over feature) Must re-enroll if you want to participate in 2017 Minimum contribution of $180 per year Maximum contribution of $5,000 per year ($2,500 if married filing a separate tax return) Last day to submit claims is April 15, 2017 for the 2016 plan year

34 OE Highlights New Disability Plan Options for 2017 Expands coverage to all benefits eligible (full, mid-level or core) One-time opportunity to enroll in Short-Term Disability (benefits up to six months), Long-Term Disability (benefits after six months, up to your social security age) or both Cannot be denied enrollment Current Basic still max of $800/month for 6 months STD and LTD both provide 60% of eligible pay, up to $15,000 One waiting period: 14-days 34
