Open enrollment is your chance to make sure you?re on the right track with your current benefits?and, if you want, to participate in the Health Care or Dependent Care Flexible Spending Account (FSA) for DO YOU NEED TO ENROLL? Enroll by Nov. 16, 2018, if you want to: Switch medical, dental, or vision plans?or the family members you?re covering. Participate in the Health Care or Day Care FSA. Start or change your Health Savings Account (HSA) contributions. Enroll in or make changes to other benefit options, including life, accidental death & dismemberment (more coverage in 2019), legal, and additional coverage for accidents or critical illness. Enroll from anywhere Inside NVIDIA Network: Log in with single sign-on using your NVIDIA credentials. Outside NVIDIA Network: Log in using your bswift user ID and password or through the bswift app. If you don?t want to make any changes or participate in the Flexible Spending Accounts, you don?t need to enroll. 2 Health Savings Account Contributions Cigna Medical Plan Enhancements and News Kaiser Plan Enhancements More Protection for Your Family and Finances Simplified Support for Your Emotional Health Your Cost for Coverage More Information Health Savings Account Contributions Get More From NVIDIA In 2019, NVIDIA HSA contributions (available if you enroll in a NVIDIA HSA, NVIDIA HSA Plus, or Kaiser HSA medical plan) will increase by up to $500 for those covering more than one child (if you are covering a spouse or domestic partner and children, NVIDIA already contributes the higher amount). And nobody will get less. See What's Changing You + 2 or More Children NVIDIA HSA NVIDIA HSA Plus Kaiser HSA $2,500 $3,000 $1,250 $1,500 You Can Save More In the HSA You can contribute to your HSA, too. Remember: Every dollar you save in your HSA is a dollar you don?t pay federal income tax on. The IRS is increasing the annual HSA limit for all contributions to: Individual: $3,500 (up from $3,450 in 2018) Family: $7,000 (up from $6,900 in 2018) Add $1,000 to the above limits if you are or will be age 55 or older in (Note: These HSA limits include NVIDIA's annual contribution.) Cigna Medical Plan Enhancements and News Cigna Cancer Support Program The Cigna Cancer Support Program can help provide support for every step in the process?from diagnosis to treatment to remission. You?ll get a personalized care and support plan and access to specially trained oncology nurses who are knowledgeable about your treatment plan and benefits coverage. Your care team will also

3 facilitate authorizations, help you manage claims and coordinate care across multiple doctors, and identify and connect you with other caregivers and support. Expanded Coverage for Physical, Occupational, and Speech Therapy With approval from Cigna, you?re covered for up to 100 visits for physical, occupational, and speech therapy (increase from 30-visit maximum). Chiropractic or acupuncture remains limited to 30 visits. Additional medical necessity review for physical therapy, occupational therapy, speech therapy, chiropractic, and acupuncture will be required after 12 visits instead of 8 visits. Combined ID Card for Medical, Mental Health, and Prescriptions By the end of the year, you and each family member enrolled in a Cigna medical plan will receive a new ID card to verify coverage for medical and mental health services (transitioning from ComPsych to Cigna in 2019), and prescriptions. Be sure to use your new card beginning in January Kaiser Plan Enhancements Hearing Aid Coverage for Kaiser HMO The plan will cover up to $2,500 toward the cost of hearing aids, once every 36 months. Increased IVF Benefit for Kaiser HMO Your copayment for GIFT (gamete intrafallopian transfer, a technique for assisting conception) services will reduce from 50% to $20. Coverage includes one-cycle maximum per lifetime, including ZIFT and IVF. More Protection for Your Family and Finances Increased Life and Accident Insurance The maximum benefit from your Life and Accidental Death & Dismemberment (AD&D) Insurance coverage (that you receive automatically and at no cost to you) will increase from 2.5 times your pay, up to $1 million, to 2.5 times your pay, up to $2.5 million. Please note that Prudential, our life insurance provider, requires an insurability review for coverage amounts above $1 million. Plus, you now have the option to buy additional voluntary AD&D coverage?up to $1 million?for yourself and your family. LegalEASE Portal Improvements

4 Starting Jan. 1, 2019, an online attorney search directory will make locating the right attorney on the LegalEASE portal easier than ever. Search by location and area of law, such as estate planning, traffic tickets, adoption, and more. Learn more about LegalEASE. Simplified Support for Your Emotional Health Cigna Will Replace ComPsych Beginning in January, administration of mental health benefits will transition from ComPsych to Cigna, which offers a broader network of providers. For Cigna plan members, this means you?ll be able to find mental health providers on mycigna.com, which also lets you view coverage and manage claims in one convenient place. If you or a family member is currently in treatment, chances are you can keep seeing your same provider. Most of ComPych providers also participate in the Cigna network. Confirm whether your provider is in the Cigna network at mycigna.com, and if so, show them your new Cigna ID card in January in order to re-route claims. If your current provider is not in the Cigna network, contact Cigna at Improved Employee Assistance Program (EAP) Portal A redesigned homepage for the GuidanceResources (EAP) portal will be available on Jan. 1, 2019 (register with Web ID: NVIDIA). This will provide easy access to work-life solutions, including parenting, nutrition, relationships, and stress management. Your Cost for Coverage After three years with no increases, your paycheck deductions [PDF] for medical, dental, and vision coverage will increase slightly, reflecting the overall trend in healthcare costs.
