3 To enter a new business policy: 1. Under New Business-Home select FULL QUOTE. Page 1 of 33
4 APPLICANT INFORMATION Complete the following Application Information. Fields with an asterisk* are mandatory and must be completed before new business can be quoted or submitted: *Agency/Branch-Select the agency code / location and or discounts for the policy to be issued under. *Policy Form Select the appropriate form: HO3, 6288, HO4 or HO6. *Effective Date Enter the policy effective as MM/DD/YYYY. The application will automatically populate the expiration date to one year from the effective date. Is there an auto insured with Mt. Washington or another Plymouth Rock Company? If yes, policy number or effect date required. Has Your Insurance been transferred within the agency? If yes, how many years loss free within the agency. Page 2 of 33

Yes/No. *Insured s prior address - *House#, Street, Apt#, *City, State, *Zip Code. *Property address - (address may not be a P.O. Box.) *House#, Street, Apt#, *City, State, *Zip Code. Applicant s mailing address is different Check box if different mailing address or P.O. Box. *House#, Street, Apt#, *City, State, *Zip Code. *Residence Type: Select-Primary/Secondary/Seasonal *Miles to Fire Station: *Feet from Hydrant: *Structure Type: Select Next to advance to the ELIGIBILITY REQUIREMENTS screen Page 3 of 33
6 Eligibility selection Select Next to advance to the Eligibility-Heating Information Screen. If an Eligibility Requirement above is selected, the risk is ineligible. Select Next to advance to the Eligibility-Heating Information Screen. Page 4 of 33
7 Eligibility-Heating Information *What is the primary heat type? Select from drop down. Select Next to advance to the Pet Information screen. Eligibility-Pet Information Does the applicant or any tenant have any animals or exotic pets under their care, custody or control? If yes, select type. Select Next to advance to the Pool Information screen. Eligibility-Pool Information *Is there a pool on premises? Select - Inground, Above Ground, Not Applicable. You will be required to answer questions bases on the type of pool selected. Select Next to Advance to the HOME VALUE screen. Page 5 of 33
8 Home Value Replacement Value: *Minimum: *Median: *High: These fields will be filled once the replacement value has been calculated. *Dwelling Coverage: Defaults to the Median once, this coverage can be changed it can not be below the Minimum or greater than $150k from the High. *Inspection year: Defaults to current year. Distance to Water: Value is returned from Homevalue Key Features: o *Construction Method: o *Construction Type: o *Total Living Area: o *Year Built o *Building Style: Page 6 of 33
9 o *No of stories: o *Garages/Carports: o *Bathrooms: o *Exterior Wall Type: RECALCULATE-Once Successful, Replacement value fields will be field in. Select Next to advance to the Loss History screen. Page 7 of 33
10 Loss History Users may add losses for the risk that may not appear on the clue report. Manually added claims can be deleted. *First Name: *Last Name: *Address: *Claim Date *Cause of Loss: *Claim Description-Free form field Select +Add a Loss at the bottom of the screen to add more losses. Select Next to advance to the Property Info Screen. Page 8 of 33
11 Property Info-Underwriting Questions Check any questions off that apply to the insured and or property. Select Next to advance to Roofing Information screen. Roofing Information *What is the age of the roof? - required on HO3, 6288 and HO6 forms *Type of Roof required on HO3, 6288 and HO6 forms Select Next to advance to the Plumbing Information screen. 12 Plumbing Information *What type of plumbing system? required on HO3, 6288 and HO6 forms. For additional plumbing system select +Add a Plumbing System at the bottom of the screen. Select Next to advance to the Electrical Information screen. Electrical Information *What type of electrical system? required on HO3, 6288 and HO6 forms. *What is the amps? For additional electrical systems select +add a Electrical System at the bottom of the screen. Select Next to advance to the Coverages screen Page 10 of 33
13 Coverage *Coverage A Dwelling: Will be the value entered as dwelling coverage on the Home Value screen. *Coverage B Other Structure: Defaults 10% of coverage A. *Coverage C Personal Property: Defaults to 70% of coverage A. *Coverage D Loss of Use: Defaults to 20% of Coverage A. *Coverage E Liability: Select from drop down box *Coverage F Medical Payments: Select from drop down box *Deductible: Select from drop down box Select Next to advance to the Optional Coverages screen. 14 Optional Coverages Coverage Categories: Select Section I-Property, Section II Liability, Company Specific or Scheduled Personal Property *Coverage Type: Depending on the category selected the appropriate list of endorsements will display. Select the endorsement you would like to add. For additional coverages select +Add Another Coverage at the bottom of the screen. Select Rate to obtain a quote. Page 12 of 33
15 Rating The Premium Summary page will give you a calculated premium. Select previous or any tab to change information on policy and re-rate. If you choose not to submit the policy you may select decline. Select Bind Quote to advance to Additional Interest screen. Page 13 of 33
16 Additional Interest To select Next to advance to the Billing screen. Billing *Bill Type: Direct Bill, EZ Paid, Mortgagee *Payment Plan: *Down Payment Method: WebPay, Insured Check (bill insured). To issue the policy select Submit. Page 14 of 33
17 Final Steps Webpay button will appear if Web payment was selected as Down Payment Method. Select WEBPAY to process down payment. New Hampshire Homeowner Acord Application is available to print for submitted policies. Page 15 of 33
18 *To Endorse a Policy: Under New Business-Home, select Endorsements. You will be brought to the Endorsement Search screen. Page 16 of 33
19 ENDORSEMENT SEARCH Enter the policy # or the insured s first and last name to retrieve the policy to be endorsed. Select Start Search to advance to the Search Results screen. SEARCH RESULTS Confirm the policy number and policy term in the search results. Select Endorse to begin entering the endorsement and advance to the Endorsement Details screen. Page 17 of 33
20 ENDORSEMENT DETAILS Enter the *Endorsement Effective Date and select Next to advance to the Applicant screen. Note: You will not be able to do out of sequence endorsements. Those will need to be sent in to Customer Service for processing. ENDORSEMENT TO CANCEL POLICY To cancel a policy, enter an *Endorsement Effective Date, Select Cancel Policy and select a *Cancellation Reason. Then select Cancel Policy Page 18 of 33

22 HOME VALUE Fields with an asterisk* are required and must contain existing, revised or new data before leaving this screen. Some data may not be available for update. In these cases, the fields will not be editable. *Dwelling Coverage can be changed, but can not be greater than $150,000 of the replacement value high. These changes will need to be forwarded to Underwriting for approval. Select Next to advance to the Coverages screen. Page 20 of 33

In these cases, the fields will not be editable. *Cover E Liability: to change coverage e select from drop down list. *Coverage F Medical Payments: to change coverage f select from drop down list. *Deductible: to change the policy deductible select from the dropdown list. Select Next to advance to the Optional Coverages screen. Page 21 of 33
24 OPTIONAL COVERAGES Fields with an asterisk* are required and must contain existing, revised or new data before leaving this screen. Some data may not be available for update. In these cases, the fields will not be editable. Mandatory policy coverages can not be deleted, the only action available is to View/Edit. Add Coverage - Select Add Another Coverage o Chose the coverage category, Section I property, Section II Liability, Company Specific, or Personal Property. o *Coverage Type: Select the appropriate optional coverage. Select Next to advance to the Additional Interest screen. Select Rate to obtain a quote. Page 22 of 33
25 ADDITIONAL INTEREST Fields with an asterisk* are required and must contain existing, revised or new data before leaving this screen. Some data may not be available for update. In these cases, the fields will not be editable. Add additional interest Select ADD ANOTHER ADDITIONAL INTEREST: o Type: Select type of additional interest, First Mortgagee, Second Mortgagee, Third Mortgagee, Additional Insured, or Additional Interest. o * Name, *House#, Street, Apt#, *City, State, Zip Code: Edit additional interest Select View/Edit for the appropriate interest. Delete additional interest Select Delete for the appropriate interest. If the policy is Mortgagee Billed, you will need to forward this request to Customer Service for processing. Select Rate to obtain a quote. Page 23 of 33
26 RATING The premium summary displays the old and new annualized premiums. Select Next to advance to the Billing screen. Page 24 of 33
27 SUBMIT/BILLING *Bill Type, and *Payment Plan- Pay plan changes are not available for processing at this time. Please forward requests for mid-term pay plan changes to Mt. Washington Customer Service for processing. Select Exit if the endorsement should not be submitted. Select Submit to process the endorsement and advance to the Confirmation screen. CONFIRMATION-Endorsements o If the endorsement was successfully processed, a message indicating such will be displayed. Page 25 of 33
28 To retrieve an existing homeowner quote select Search Quotes under New Business-Home. Search Quotes You can search for a quote by Last Name, First Name, or quote number. Page 26 of 33
29 Select Submit to advance to the Search Results screen. Search Results Edit Application-allows you to modify the current quote. Copy and Edit Quote-allows you to have different variations of a quote. Manage Quotes Manage Quotes displays all quote activity within the last 45 days. Status definitions Application quote Issued quote has been submitted for issuance. Page 27 of 33
30 The Issue tab displays all policies that have been submitted. Page 28 of 33
31 Web Pay Search. To make a payment you can search by last name, first name, or Policy Number. Select Start Search to advance to the search results. Verify the policy Name and Address, policy number, and effective date of the policy. Select Webpay to make the payment. Page 29 of 33
32 The Web Pay screen will automatically populate with the following fields: Policy #: First Name Last Name Address 1 City, State and Zip Code Select a *Payment Method: Select Submit for Payment Confirmation Page 30 of 33
33 Credit Card *Credit Card #: *Expiration Date: Expiration date of the credit card. Format MM/YYYY *Credit Card CVV#: Typically the 4 numbers on the back of the credit card. *Amount: Dollar amount of the web payment. Check off box to certify that you are authorized to make payment. Select Submit for Payment Confirmation Electronic Withdrawal Select from the dropdown menu *Account Type Checking, Savings, or Money Market. *Bank Routing Number - This is always 9 digits. Typically this is the first set of numbers on a personal check. *Bank Account Number - Typically the second set of numbers on a personal check. *Re-verify Bank Account #. *Amount - Dollar amount of Web payment. Check off box to certify that you are authorized to make payment. Select Submit for Payment Confirmation Page 31 of 33
34 All payments made by 5:00 p.m. on a non-weekend day or holiday will be posted the same day. Most withdrawals from the designated bank account will occur on the following business day. A copy of this screen may be printed out for the insured s records. You can search for payments made on a policy by the Customer Information, Policy Number or Confirmation Number. Select Review Payment Results Page 32 of 33
35 You can cancel a payment on the same day it was submitted. You are unable to edit an existing payment, if the payment is incorrect please cancel and process another web payment. Page 33 of 33
