HUB Internatinal HKMB is the insurance brker respnsible fr arranging the insurance prgram fr the Alberta Basketball Assciatin and its members. Fr mre infrmatin abut HUB Internatinal HKMB, yu can visit their website at What type f insurance is prvided by the ABA t its members? Liability Insurance and Accident Insurance This insurance is arranged by the ABA fr its registered members and the cst is included in the registratin fee paid by each member. What is the purpse f the General Liability and Accident Insurance Cverage? The primary purpse f the General Liability plicy is t prtect against lawsuits frm individuals alleging that the sprts rganizatin and/r its members have negligently caused "bdily injury" t a player r spectatr. The plicy als cvers ther types f lawsuits such as "prperty damage", "persnal injury" (libel & slander) and Nn Owned Autmbile liability. The plicy des nt cver expenses related t criminal charges brught against a member f the assciatin regardless f the nature f the charge(s). The Accident Plicy is designed t reimburse a member in gd standing wh suffers a cvered injury/ death while participating in an ABA Sanctined Activity. The Accident Plicy is designed t prvide cverage t thse members wh d nt have access t anther health insurance plan r t supplement existing health insurances. Wh is cnsidered an insured under the General Liability plicy? Cverage is prvided fr the Assciatin itself and Members registered in gd standing with the ABA, but nly while participating in activities sanctined by the ABA. Members are required t sign a Participant Waiver as part f the registratin prcess fr cverage t be affrded. Als cnsidered Insured's under the liability prgram are caches, managers, fficials, administratrs and vlunteer wrkers, but nly fr acts within the scpe f duties perfrmed n behalf f the ABA. What activities are cvered? Cverage is prvided fr all ABA sanctined basketball activities. Please see page three (3) fr a list f sanctined basketball activities. Are Club/League fundraisers cvered under the General Liability Plicy? N. The ABA Prgram des nt prvide Liability cverage fr fundraisers.

Frm the ABA Insurance Page 2. Upn cmpletin please frward t jbailey@basketballalberta.ca fr apprval and issuance. 3. Alberta Basketball Assciatin will the Certificate f Insurance back t the Club Cntact Persn listed n the frm. If pssible, please prvide the cntact name and fr the facility r schl bard that will be n the certificate. A cpy f the insurance certificate can be sent directly t the cntact as well as the club. 4. Please ensure that the Certificate Prgram CertExchange has been white listed s that yu will receive this Certificate. PLEASE NOTE INSURANCE CERTIFICATES WILL NOT BE ISSUED TO CLUBS WHO HAVE BEEN SUSPENDED OR HAVE OUTSTANDING FEES. What is defined as sanctined? Sanctining definitin/guidelines are as fllws: Sanctined Game: A game between tw registered Alberta Basketball teams that is fficiated by tw referees assigned by the lcal fficials bard. Sanctined Turnament: An event apprved by Alberta Basketball that cnsists f fur r mre registered Alberta Basketball teams participating in a cmpetitive structure that results in a team playing three r mre games n the dates submitted by the hst club. Exhibitin Game/Cntrlled Scrimmage: A game r scrimmage between tw registered Ontari Basketball teams but is nt fficiated by Member Officials. Is a parent cvered if they are driving his r her wn children t an event? N. The prgram des nt prvide cverage fr parents driving their children t the game/practice because they are, in that circumstance, exercising their parental respnsibility nt a team duty. Hwever, cverage is in place fr vlunteers f the assciatin while perfrming vlunteer duties. Therefre, parents assciated with a team r perfrming a specific duty assigned by the team (cach, assistant cach, r ther authrity) d have insurance cverage under this prgram.

4 Des the plicy prvide cverage fr lawsuits alleging sexual abuse/mlestatin? N. Abuse/mlestatin is excluded frm cverage. Des the plicy prvide cverage against liqur liability claims? Yes. There is cverage under the Liability prgram fr defense f liqur related claims but nly if: They arise in cnnectin with ABA sanctined banquets r award ceremnies Nte: T btain separate cverage fr the fllwing scenaris, please cntact alan.hllingswrth@hubinternatinal.cm. Events utside f ABA sanctined banquets r award ceremnies Are players insured when trying ut fr a team if they are nt registered with that club? Yes, players are cvered when participating in ABA club apprved tryuts, training camps and practices whether r nt they are registered with that team as lng as they have cmpleted and submitted the Alberta Basketball membership frm (that includes a waiver). They are nt, hwever, cvered if they play in a sanctined game with a team with which they are nt prperly registered.

5 Accident (AD&D) Plicy Wh is cvered? All players, managers, caches, trainers and members f fficiating crew f the gverning bdy/sprts assciatin listed in the plicy declaratins/ When are they cvered? The abve participants are cvered during sanctined activities f the ABA, such as practice sessin, game r activity apprved by and under the supervisin f prper authrity f the team, club, rganizatin r the Named Insured and fr which cverage has been declared and purchased. What is nt cvered? This Plicy des nt cver lsses directly r indirectly caused r cntributed t by: Injury sustained while in any f the armed frces (land, sea r air) f any cuntry r internatinal authrity; Intentinal self-inflicted Injury r attempted suicide; Being under the influence f alchl r having taken drugs r narctics unless prescribed by a legally qualified Physician r surgen. Injury ccasined r ccurring while the Participant is cmmitting r attempting t cmmit a criminal act r t which a cntributing cause was the Participant being engaged in an illegal ccupatin r activity; Pregnancy, childbirth, miscarriage r abrtin; Sickness r disease f any kind; Injury as defined in this Plicy, arising ut f a pre-existing medical r mental cnditin. Hwever, an Injury, fr which the treatment has nt been rendered r treatment medically recmmended fr the past thirty cnsecutive mnths shall nt be cnsidered a pre-existing cnditin unless therwise specifically excluded; Neurses, psychneurses, psychtherapies, psychses r mental r emtinal disrders f any type; Rest cures, sanatrium r custdial care r perids f quarantine r islatin; Csmetic r plastic surgery nt necessitated as a result f accidental bdily Injury; Dental examinatins, X-rays, extractins, fillings and general dental care except as a result f accidental bdily Injury; Rutine medical examinatins; and Csts incurred mre than ne hundred and eighty (180) days after the date f the event giving rise t them. resulting frm, r in cnnectin with any f the fllwing regardless f any ther cause r event cntributing cncurrently r in any ther sequence t the lss r expense: War, hstilities r warlike peratins (whether war be declared r nt),

Is the cverage under this plicy cnsidered excess? Yes. The Medical Expense cverage is "excess" r the secnd payer and requires ther cllectable insurance (grup medical plan, grup blanket plan, gvernmental plan r prgram, r cverage prvided r required by any law r statute, including autmbile "fault" and "n-fault" cverage and Wrkplace Safety and Insurance Bard) t respnd first befre the "excess" Accident plicy will pay any benefits.
