This Annual Enrollment Guide explains the 2016 plans available to Exclusive Agents R3001 (EAs) and Exclusive Financial Specialists L2000s (EFSs) which are collectively referred to as Exclusive Independent Contractors (EICs) throughout this guide. Annual Enrollment is the one time of the year in which you can enroll or make changes to your elections, unless you experience a life event in accordance with plan provisions. If you do not make any changes during Annual Enrollment, your current elections will continue in New Enrollment in Long Term Care Insurance Plan Ends 12/31/15 The Long Term Care Insurance Plan through CNA will accept new applicants through December 31, Beginning January 1, 2016, coverage will only continue to be available for those already enrolled in the Plan. Evidence of insurability may be required. How to Enroll Access the Your Benefits Resources (YBR) website at from Monday, November 30 through Friday, December 11, After making your Annual Enrollment selections, please wait until you see the Completed Successfully page or your elections will not be saved. Print the page by clicking on the print icon located in the upper right hand corner of the page. This will be your enrollment confirmation statement. After You Enroll What s Next The elections you make during Annual Enrollment will become effective January 1, Enrollment Tips 1. Make sure you know your YBR user ID and password before Annual Enrollment for 2016 begins. If you ve forgotten yours or need to create new ones: Go to YBR at and follow the instructions on the Log On page. Call the Allstate Benefits Center at (888) and follow the prompts to reset your password. 2. You may change your elections anytime during the enrollment period. The last completed election you make on or before Friday, December 11, 2015, will be your final election for Annual Enrollment for 2016 Monday, November 30 through Friday, December 11, 2015 If you do not make any changes during this Annual Enrollment period, your current elections will continue for 2016.

4 4 A n n u a l E n r o l l m e n t G u i d e < R E T U R N T O I N D E X Your Cost for Coverage Your premium which is the cost you pay to be covered under any plan in which you enroll for 2016 is made up of two components: Negotiated Rates are the costs that the insurance carrier charges to be covered under a plan. These rates are typically negotiated each year. Administrative Costs are those associated with administering the EIC plans. Administrative costs are spread pro-rata to all participants across all EIC plans. As a result, although not typical, midyear changes (increases or decreases) are possible based on the outcome of 2016 plan enrollment. Making Premium Payments Premiums for each month s coverage are due at the Allstate Benefits Center on the first business day of the month. Save the Hassle Go Electronic With Direct Debit You may elect to have your premium payments automatically deducted from your designated bank account. Elections made by the tenth of the month will go into effect on the first business day of the following month's billing cycle. For example, if your direct debit election is effective on December 10, 2015, your premium for your January coverage will be debited from your account on January 4, 2016 (the first business day of the month). You can register for direct debit on the Your Benefits Resources (YBR) website. On the home page, go to At a Glance and click Sign Up. The Allstate Benefits Center will send you a Direct Debit Enrollment Confirmation Notice once you ve successfully enrolled in direct debit. This notice will include the effective date of your direct debit cycle. Direct Billing If you are not enrolled in direct debit, the Allstate Benefits Center will send you a monthly bill for coverage which will be due in full on the first business day of the folllowing month. Your payment must be received by the due date to ensure continuous coverage. Failure to remit payment in full by the due date may result in your coverage being terminated without the ability to reinstate coverage.

5 5 A n n u a l E n r o l l m e n t G u i d e < R E T U R N T O I N D E X Dental Plan The Dental Plan offers two plan options: a Dental Maintenance Organization (DMO) and a Dental Preferred Provider Organization (PPO). Availability of these options is based on your home ZIP code. Dental Maintenance Organization (DMO) option n All dental services must be provided by a Primary Care Dentist (PCD) selected from the network of participating DMO providers in order for services to be covered. Dental Preferred Provider Organization (PPO) option n You have the freedom to choose at the time of service either a PPO participating provider or any nonparticipating provider. PPO participating providers have agreed to provide care at a negotiated fee schedule, while services provided by a nonparticipating provider are subject to reasonable and customary charge limits. Refer to the Aetna Benefit Summary Sheets available on Your Benefits Resources (YBR) for detailed plan provisions. Eligibility All Exclusive Independent Contractors (EICs) and their eligible dependents can enroll in the dental plan. However, eligibility for each option is based on your home ZIP code of record. Please be sure that your home address is correct on the YBR website to ensure your dental plan option eligibility is accurately reflected during Annual Enrollment for Plan Design Changes There are no plan design changes for Helpful Hints To Select a Primary Care Dentist (PCD) Visit Aetna s website, or; Contact Aetna Member Services, toll-free at (877) To select a PCD, enter the PCD ID number on the YBR website during enrollment. You may change your PCD any time. PCD changes made between the 1st and the 15th of the month are effective the 1st of the following month. PCD changes made after the 15th of the month are effective the 1st of the second following month.

6 6 A n n u a l E n r o l l m e n t G u i d e < R E T U R N T O I N D E X Vision Plan Administered by Vision Service Plan (VSP), the plan offers you and your eligible dependents coverage for eye exams, eyeglass lenses, and frames. You have the choice to receive vision care and products through in-network providers at a set coverage level or from out-of-network providers according to a fixed schedule of benefits. Refer to the VSP Brochure available on the Your Benefits Resources (YBR) website for detailed plan provisions. You can buy eyewear online through the VSP store. Go to to purchase eyewear, including frames and contact lenses. You receive 20% off glasses and sunglasses plus free shipping. Save money on hearing aids. EICs and their covered dependents enrolled in the vision plan also have access to the TruHearing MemberPlus Program. Parents, siblings, grandparents, aunts and uncles can also enroll as guest members at no additional cost. The program offers discounts on hearing aids as well as yearly comprehensive hearing exams from participating providers for $75. 7 7 A n n u a l E n r o l l m e n t G u i d e < R E T U R N T O I N D E X Life, AD&D and LTD Plans The life, accidental death & dismemberment (AD&D), and long term disability (LTD) insurance plans are underwritten by Unum. Eligibility All Exclusive Independent Contractors (EICs) and their eligible dependents can enroll in these plans. Plan Design Changes There are no plan design changes for Visit Unum s website for plan provisions, rates, calculators, and Evidence of Insurability (EOI) forms. You may also refer to the Unum Highlights Sheets available on the Your Benefits Resources (YBR) website. Premiums Your monthly premium effective January 1, 2016 is based on coverage amounts elected, age, and for some plans, eligible Annual Earnings which are reclassified each year and may cause a rate adjustment. Need to Know Annual Earnings (AE) Your AE are reclassified each year based on an average of your eligible earnings for the 24-month period prior to August 1, If you haven t been an EIC for the entire 24-month period prior to August 1, 2015, your AE are the average of your eligible earnings prior to August 26, 2015 or $30,000, whichever is greater. Life Insurance EICs may enroll in life insurance coverage in $10,000 increments up to five times their AE or $500,000, whichever is less. You may also purchase coverage for your eligible: Spouse/domestic partner in $10,000 increments up to the amount of your coverage or $150,000, whichever is less. Children in $2,000 increments up to $10,000. EOI may be required. Accidental Death and Dismemberment (AD&D) Insurance AD&D Insurance provides coverage if you die in a covered accident. It also pays benefits if you suffer a covered dismemberment. EICs may enroll in AD&D coverage in $10,000 increments up to five times their AE or $500,000, whichever is less. You may also purchase coverage for your eligible: Spouse/domestic partner in $10,000 increments up to the amount of your coverage or $150,000, whichever is less. Children in $2,000 increments up to $10,000. EOI is not required. Note: All eligible commissions earned under a single agent number, even commissions earned by a sub-producer using that agent number, are included as eligible earnings for that agent. If you re a multi-location agent, AE is the sum of your individual agent numbers. Beneficiary Designations All EICs with life insurance and/or AD&D coverage must enter beneficiary information on the YBR website. If you elect spouse/domestic partner or child life insurance and/or AD&D coverage, you must be the beneficiary.

8 8 A n n u a l E n r o l l m e n t G u i d e < R E T U R N T O I N D E X Long Term Disability (LTD) Insurance LTD insurance pays a monthly benefit if a qualifying disability prevents you from working or limits your ability to work. You can select coverage based on various elimination periods (the length of continuous disability which must be satisfied before you re eligible to receive LTD benefits). EOI may be required. After satisfying your elimination period, the LTD plan provides the following monthly benefits: For the first 12 months: 60% of your monthly AE, up to a $7,500 maximum. After 12 months: 50% of your monthly AE, up to a $7,500 maximum. Legal Plan The legal plan is administered by Hyatt Legal Plans, Inc. You and your eligible dependents have access to a nationwide network of attorneys who can help you with various legal issues. Plus, the plan provides benefits that may be used to offset or partially offset the legal fees for certain eligible services, including those for: Child custody or support Wills and trusts Matrimonial matters Traffic ticket defense (excluding DUI) Small claims assistance Watch a video to learn how this plan works. Eligibility All EICs and their eligible dependents are eligible to enroll in the legal plan. Plan Design Changes There are no plan design changes for 2016.

9 9 A n n u a l E n r o l l m e n t G u i d e < R E T U R N T O I N D E X Long Term Care Insurance You may purchase a long term care insurance policy through CNA to provide coverage if you should need care in a nursing home, home care, or an assisted living facility. Long term care offers comprehensive, flexible benefits and is available to EICs, and their eligible spouses, domestic partners, adult children, parents, and grandparents. Eligibility All EICs and their eligible dependents can enroll in the long term care insurance plan. New enrollment is available only through December 31, Evidence of insurability is required. Plan Design Changes There are no plan design changes for Public Health Care Exchanges All Americans can purchase health care insurance through the public health care exchanges.
