1 Enrolling as a New Employee or Newly Eligible for Benefits Employee If enrolling dependents, you will need to provide the following information and documentation: Marriage Certificate or Oregon Certificate of Registered Domestic Partnership for a spouse or domestic partner Birth Certificate for Children Legal Guardianship - Guardianship/Custody documents issued by a court Social Security Number for dependent(s) (exception for children under one year of age) Date of Birth for dependent(s) 1) Go to the CIS Employee Benefits portal: Scroll down and click on New Hires-Getting Started 2) Welcome New Hires-Getting Started page Guides available to assist with enrollment o Instructions for New Hires Step by step enrollment instructions o New Hires Helpful Hints Before You Get Started Helpful information o Flexible Spending Account Enrollment Form When ready to start your enrollment o Click on the click here to start Page 1 of 13
2 3) Register for your account by clicking on the Register button If you were at a previous employer with CIS benefits, you will still need to register for a new account for your benefits with your new employer 4) On Info Page The Company Key is CIS if does not automatically populate Enter Social Security Number Enter Date of Birth Click Continue Page 2 of 13
3 5) Create Account Create your own User Name, Password, and Security Questions (there are three questions) o User Name, Password, and Security Questions Answers are all case sensitive Click Continue 6) Confirmation page Click Continue o Will take you back to the Log in Screen Page 3 of 13
4 7) Log in with your User Name and Password If you have forgotten, click on the Forgot your user name or password? and follow the prompts. o If you need assistance, you can contact your employer or CIS Benefits Helpline at ) First Time Accessing Account - Agreement and Contact Preference Pages Will only see these pages the first time accessing your account Agreement for Electronic Enrollment o You must click on Yes and then click Continue How do you want to be contacted by CIS or the Insurance Carriers for benefits administration only? o Enter the or phone number o Select Primary Preference o Click Continue Page 4 of 13
5 9) From your Home Page Click Start Here from the banner or from the To Do box 10) Let s Get Started Review the Benefits Enrollment & Eligibility Guide prior to starting your enrollment (below) Start your enrollment by clicking the Start Enrollment button Page 5 of 13
6 11) Review and make corrections to your address, , or phone number. For other corrections, contact CIS. Fields outlined in red with a red asterisk (*) are required fields If you don t have your own work number, ask your employer for a generic one To continue, click Next ) Dependents Click the + Add a New Dependent button to add eligible dependents you want to enroll If you won t be enrolling a dependent in coverage, DO NOT add them to the system and skip to step 12 13) Add a Dependent Under the dropdown, select the applicable relationship o Complete the information fields Fields outlined in red with a red asterisk (*) are required fields Page 6 of 13
7 Enter Dependent Social Security Number o Must provide Social Security Number to add the dependent to enrollment Exception Newborn children under 1 year of age SSNs will be requested at a later date Only add address if the dependent address is different than yours To complete, click on Save Changes Repeat this process to enroll multiple dependents If finished adding dependents, click Next Page 7 of 13
8 14) Electing your enrollments Click the Select button next to the plan option you want Check Yes or No for each dependent listed Click Next to continue to Dental and then Vision, if offered by your employer Page 8 of 13
9 15) Life and Disability enrollment If your employer offers Basic Life, Accidental Death & Dismemberment (AD&D) and Statutory Life with CIS, you will automatically be enrolled. You cannot Waive any of these coverages Voluntary (Employee Paid Plans) if offered by your employer o Supplemental Employee/Spouse Life - Select the plan option and the coverage amount o Voluntary Dependent Life - Select the plan option. Supplemental Employee Life As a new hire or newly eligible employee, you may elect up to $100,000 on a guarantee issue (GI) basis. Any amount elected over the GI is subject to approval by The Hartford Supplemental Spouse/Domestic Partner Life If newly eligible, and if the employee elects to enroll in Supplemental Employee Life, the spouse may elect up to the same amount as the employee elected Guarantee issue (GI) amount is $20,000. Any amount elected over the GI is subject to approval by The Hartford Voluntary Dependent Life Coverage is $10,000 for each eligible dependent Flat fee is charge regardless of number of dependents covered Click Next to continue through the enrollment pages 16) If you requested an amount for Supplemental Employee or Supplemental Spouse Life that is over the GI amount, you will see this notification of EOI page If you have added new dependents that will require dependent verification you won t be able to click on the EOI link and complete until dependents have been verified Page 9 of 13
10 17) Long Term and Short Term Disability If your employer offers Long Term Disability you will automatically be enrolled. You cannot Waive this coverage If Short Term Disability is offered by your employer o This is a Voluntary, Employee Paid plan o Select level of coverage. Please note: You are not eligible to select a plan that exceeds 60% of your weekly earnings 18) Flexible Spending Account (FSA) for Healthcare or Dependent Care, If offered by your employer, the enrollment form is available on the website o Click on New Hires-Getting Started o Download FSA enrollment form Complete and give to your employer who will forward the form to CIS to add the enrollment 19) Add Beneficiaries if you have Life and Disability enrollments If no Beneficiary listed or you need to add additional Beneficiaries, click + Add Beneficiary Click Next when finished Page 10 of 13
11 20) Review your enrollment selections If you have any corrections, click on the Edit button and make any necessary changes Enrollments will show as pending until you click the Approve button 21) Confirmation and Attestation Click I Agree to complete enrollment Page 11 of 13
12 22) If documentation is required, Action Required screen(s) will show. These will show on your homepage once you have completed the enrollment in the To Do box o Reminder you won t be able to do the Evidence of Insurability (EOI) until required marriage certificate has been submitted and approved by CIS Click Next to complete enrollment 23) You will see a confirmation number after completing your enrollment You can print a copy of your benefit elections by clicking Print Benefit Summary Click Home to go back to your homepage or Click Logout to exit Page 12 of 13
13 24) How to Upload Required Documentation From your homepage o Click on either the View Details button in the To Do box or Message Center at the top of the page o View the Action Required Submit Documentation to Verify Eligibility message o Upload documentation in requested format Page 13 of 13
