Legal Parent or Guardian must initial 9 places & sign in 1 place. Pass Holder information must match your attached proof of age document. 3. Prove the identity of the pass holder s LEGAL Parent or Guardian and return entire packet. This can be done 2 ways: a. IN PERSON with LEGAL Parent/Guardian present OR the notarized acknowledgment (page 4): Mountain High Resort State Highway 2 Wrightwood, CA Pass will be issued immediately. b. MAILED TO RESORT Complete notarized acknowledgment (page 4), mail entire packet to: Mountain High Resort ATTN: Season Pass Liability P.O. Adult Season Pass: Adults (ages 13-69). Child Season Pass: Child (ages 7-12). Limit two (2) Child Season Passes per Adult Season Pass purchased. Children turning 13 before January 1, 2020 are required to purchase an Adult Season Pass (birth certificate required). Free Child Season Pass: Child (ages 6 & under). Limit two (2) Free Child Season Passes per Adult Season Pass purchased. Children turning 7 before Jan. 1, 2020 are required to purchase a Child Season Pass (Birth Certificate required). Senior Season Pass: Adult (ages 70 & over). THERE IS A $10.00 PROCESSING FEE PER PASS AND ALL ORDERS ARE TAKEN ON A FIRST-COME, FIRST-SERVE BASIS. Your Season Pass must be available for all Lift Attendants to view: As holder of this pass and as a condition for use of these facilities, holder agrees that the management will not be responsible if the pass is lost or stolen. The permission to use the Season Pass is granted solely to the individual to whom it is issued. Season Pass may not be re-sold or transferred to anyone. Misuse of the Season Pass, including misconduct, providing false information on this application, transfer or resale of the pass, or tampering in any way, will result in forfeiture of the pass without refund. Additionally, you must have your Season Pass in order to use the facilities. If you forget your pass you are entitled to a one-time only replacement ticket. If your pass is lost or stolen notify Season Pass Office immediately. To replace a lost pass you will be charged $15.00 the first time, and $40.00 the second time. (Initial) Mountain High Reunited, LLC will no longer offer carry over credit due to injury or illness. Mountain High Reunited, LLC at its discretion will determine the length of season and the number of lifts in operation at any one time based on snow conditions and other variables. THERE ARE NO REFUNDS FOR LACK OF SNOW, LENGTH OF SEASON, OR ANY OTHER REASON. (Initial) I consent to the use by Mountain High Reunited, LLC of any pictures (video and print) for commercial purposes or otherwise, of myself (or my child) in connection with my activities at Mountain High Reunited, LLC. I also understand these photos or other materials are the sole property of the resort and may be used at their discretion. Additionally, I agree and understand I will receive no compensation for their use either now or in the future. Take-Off T = Take-Off Zone is for making moves that start your trick. Approach Maneuver Landing M = Maneuver-Zone is for controlling your body in the air and setting up for landing. L = Landing-Zone is the prepared slope between the knuckle and the run out STUFF TO KNOW: READ AND OBEY ALL POSTED SIGNS, INSTRUCTIONS AND WARNINGS BEFORE USING FREESTYLE TERRAIN. 3 The National Ski Area Association along with Burton Snowboards has put together a cooperative effort to educate guests on the hazards associated with Terrain Park use. Mountain High Resort has added this program to its brochures, and pass agreements to educate its guests and employees and make them aware of those hazards associated with the use of terrain features and parks that are a part of this resort. The Orange Oval symbol designates Freestyle Terrain such as parks and pipes. Park Smart is all about Freestyle Terrain safety - Know it, Respect it, Use it! TERRAIN PARK EDUCATION PLEASE READ: ASSUMPTION OF RISK - Winter Activities Participant is aware that participation in winter activities generally and particularly the SPORT OF SNOW SLIDING IN ITS VARIOUS FORMS (including, but not limited to, the use of Alpine, Snowboard, Telemarking, Specialized Adaptive (Disabled) or other snow sliding equipment, All Mountain Freestyle Parks, terrain features, features that may exist on any open trails, boxes, rails, any intentional or unintentional jumping at any location, and the use of chairlifts or surface tows) INVOLVE INHERENT RISKS OF PROPERTY DAMAGE, INJURY OR DEATH that no amount of care, caution, instruction or expertise can eliminate. Such dangers include, but are not limited to, variations in terrain and surface conditions, falls, loss of control and collisions with other snow sliders, or with natural and/or manmade objects, continually changing weather conditions, moguls, bare spots, rocks, ruts, ropes, poles, grooming vehicles, snowmobiles and the failure to ski/board within one s own ability. Such risks and dangers are recognized and accepted whether they are marked or unmarked. THE PARTICIPANT FREELY AND VOLUNTARILY ASSUMES ANY AND ALL RISKS, INCLUDING THE RISK OF ANY ACTUAL OR ALLEGED NEGLIGENCE OF Mountain High Reunited, LLC and its parent and affiliated companies, owners, employees, agents, officers, directors, successors and landowners (collectively hereinafter MOUNTAIN HIGH ). (Initial) ADDITIONALS-Mountain Conditions In addition to the above risks, each participant understands and voluntarily accepts that SURFACE CONDITIONS on roads, parking lots, walkways, stairs and any other areas that may be exposed to the elements of nature are subject to the deposit, melting and refreezing of snow, rain, hail and ice, such that WALKING OR OTHER DAILY ACTIVITIES may become DANGEROUS. Snow, ice, debris or other materials may be tracked indoors and may present DANGER. PARTICIPANT FREELY AND VOLUNTARILY ASSUMES ANY AND ALL SUCH RISKS, INCLUDING THE RISK OF ANY ACTUAL OR ALLEGED NEGLIGENCE OF MOUNTAIN HIGH. (Initial) RELEASE OF LIABILITY AND COVENANT NOT TO SUE As a condition and in consideration of being allowed to voluntarily participate in winter activities at Mountain High Resort, PARTICIPANT AGREES TO NOT SUE AND TO FOREVER DISCHARGE AND RELEASE MOUNTAIN HIGH FROM ANY AND ALL LEGAL LIABILITY for any and all property damage, injury or death caused by or arising from any access to the Mountain High Resort premises or participation in any winter related activities in their various forms, INCLUDING, BUT NOT LIMITED TO, INCIDENTS ARISING FROM ANY ACTUAL OR ALLEGED NEGLIGENCE OF MOUNTAIN HIGH. (Initial) AGREEMENT TO INDEMNIFY Participant agrees for himself/herself and his/her heirs, assigns and representatives TO INDEMNIFY AND TO HOLD MOUNTAIN HIGH HARMLESS from any and all losses, claims, actions, causes of action, or proceedings of any kind which may be initiated by participant or by any other person or organization on participant s behalf, including demands for damages, judgements, costs, loss of services or expenses, TO THE FULLEST EXTENT OF THE LAW. I UNDERSTAND THAT THIS IS A LEGALLY BINDING CONTRACT and is intended to be a general release of liability that shall be construed as broadly as the law allows in favor of MOUNTAIN HIGH. I agree that any claim that I/we may bring against MOUNTAIN HIGH must be filed in the North District of the Los Angeles Superior Court, California. I/we understand that if any portion of this contact is held invalid, the balance shall remain in full legal force and effect. (Initial) BOUNDARIES Be advised: Areas beyond the ski area boundary are not patrolled or maintained, and avalanches and other hazards may exist. Read and obey all posted signs and warnings. Going beyond a boundary or ignoring posted signs may result in revoked privileges. Additionally, any search and rescue attempts beyond resort boundaries may not be possible, and you, your assigns and heirs may be responsible for search and rescue costs. (Initial) THIS IS A RELEASE OF LIABILITY, YOU HAVE VOLUNTARILY ENTERED INTO THE ABOVE AGREEMENT, DO NOT SIGN IF YOU DO NOT AGREE TO ALL TERMS AND CONDITIONS.
