1 SKI CLUB SIGN-UP INSTRUCTION SHEET Sign-Up Process Sign-ups are first come first-served on Monday, November 9 th in room 232 after 6 a.m. The final day to sign-up is Monday, November 23 rd. - All paperwork and payment must be complete from ALL roommates at the time you sign up in order to be considered. No exceptions will be made, and any incomplete packets will go to a wait list, then the bottom of the sign-up list once complete. 1) List your preferred roommates for each trip on that trip s slip. 2) Anywhere there is an X, a parent must sign. This document must have one parent sign their name and notarize their signature (either by a notary or witnessed at school by any school employee. The other parent just needs to sign their name without a notary. 3) It is fine if one person brings in the forms and money for all their roommates. 4) Make checks or money orders payable to Conestoga High School 5) You reserve a spot once everyone in your group has turned in all the forms and made payment. If ANTHING is missing, you will be placed on a wait list until EVERYTHING is completed, then placed at the bottom of the sign-up order. 2 The prices below are based on participants staying in quad rooms. If you sign up with fewer than 4 people, roommates may be combined to create quad occupancy. In the case there are fewer than 45 sign-ups, the trip may not run and all money will be refunded. Gore, New York Ski Trip January 29-31, 2016 $300 Includes: Round trip motor coach transportation from Conestoga 2 nights lodging at the Clarion Inn, Lake George New York (518) continental breakfasts Saturday Night dinner 2 day ski pass to Gore Mountain All taxes and gratuities Not included: Lunches and two dinners on the road Equipment, which must be rented prior to the trip Okemo, Vermont Ski Trip February 19-21, 2016 $335 Includes: Round trip motor coach transportation from Conestoga 2 nights lodging at the Comfort Inn, Rutland Vermont 2 continental breakfasts Saturday Night dinner 2 day ski pass to Okemo Mountain All taxes and gratuities Not included: Lunches and dinners Equipment, which must be rented prior to the trip Sample Overnight Itinerary Friday Before school- leave all bags and equipment in the gym hallway (in-between the two gyms) before 7:20 am. After that the room will be secured. Anything needed on the bus keep in your locker. Baggage will be searched. 3 Saturday Wake-up call- 6:00 am Breakfast- Breakfast at 6:30 am in the hotel lobby Departure- load bus at 7:15, depart 7:30, skiing on your own Return- load bus at 4:00 p.m. Social Time until dinner- swimming, etc. Dinner 7 p.m. at the hotel lobby meeting room Sunday Wake-up call- 5:45 am Breakfast- 6:30 am in the hotel lobby Departure- load bus at 7:15 with all bags and belongings we will not return to the hotel after skiing Return home- Depart at 3:00 p.m. Dinner- On the road Arrive at CHS- between 9:30-10:30 p.m. -Emergency Contacts: CHS field trip phone (610) Jonathan Goodman cell (610) There will be 3-4 teacher chaperones on each bus Cancellation -Once payment is made, money is partially refunded (approximately $75-$100) unless another student is able to join the same room in place of the cancelled student, in which case there is a full refund. -No money is refunded for cancellation within 15 days of the trip. -In the event that CHS or T-E School District cancels the trip after money has been paid, all attempt will be made to reschedule the trip if possible, or if not a partial refund will be made (approximately $75-$100). - PLEASE consider purchasing travel insurance at The site compares 20 travel insurance companies. Also look at Rules -Students may be denied participation on the trip for discipline reasons at the discretion of school officials, in which case no refund will be given. -The CHS Code of Conduct will be in effect at all times. In the case of a serious infraction, the head chaperone will confer (by telephone) with an administrator to determine whether the student(s) will be sent home at the cost of parents or reported, in writing, to the assistant principal upon return to school. The CHS administration have the responsibility to deny any student access to the trip based upon the student s demonstrated inability to conform to the rules of the schools as indicated by the student s discipline record. -Students will be supervised at all times except during the ski day and when sleeping. The hotel has 24 hour security. - A parent/student meeting will be held in January to review details of the trip and answer questions that may exist. Cloud 9 Skiing LLC

4 Release of Liability, Waiver of Claims, Express Assumption of Risk and Indemnity Agreement: Please read and be certain you understand the implications of signing. Express Assumption of Risk Associated with Sport, Venue Use, and Related Activities. Participant hereby affirms and acknowledges to have been fully informed of the inherent hazards and risks associated with skiing and snowboarding and with Cloud 9 Skiing Programs, including but not limited to the transportation of equipment related to the activities, and traveling to and from activity sites in which participant is about to engage. Inherent hazards and risks include but are not limited to: 1. Risk of injury from the activity and equipment utilized is significant including the potential for broken bones, severe injuries to the head, neck, and back or other bodily injuries that may result in permanent disability and death. 2. Possible equipment failure and /or malfunction or misuse of my own or others equipment. 3. Participant agrees to wear protective gear as decreed by the governing body of the sport participant is participating in. However, protective gear cannot guarantee the participant s safety. Participant further agrees that no helmet can protect the wearer against all potential head injuries or prevent injury to the wearer s face, neck or spinal cord. 4. Variation and/or steepness of terrain, variation or changes in surfaces including but not limited to snow surfaces, ice, bare spots, rocks, stumps, debris, cliffs, trees, light poles, signs, buildings, roads, walkways, ramps, rails, stairs, pyramids, bowls, halfpipes, jumps, padded and non-padded barriers, other persons, and other natural and man-made hazards. 5. My own negligence and/or the negligence of others, including but not limited to operator error and guide decision making including misjudging terrain, weather, riding surfaces, or other obstacles. 6. Exposure to the elements and temperature extremes may result in frost nip, frost bite, heat exhaustion, heat stroke, sunburn, hypothermia and dehydration. 7. Dangers associated with exposure to natural elements include but are not limited to avalanche, rock fall, inclement weather, thunder and lightning, severe and or varied wind, temperature and other weather conditions. 8. Accidents or illness occurring in remote places where there are no available medical facilities. 9. Fatigue, exhaustion, chill, and/or dizziness, which may diminish my/our reaction time and increase the risk of accident. 10. Impact or collision with other athletes, spectators, facility employees, pedestrians, motor vehicles, and cyclists. 11. All the danger of transportation and driving to and from the destination of the Cloud 9 Skiing trip. This includes but is not limited to the negligence of the bus driver, mechanical failure of the bus, hazardous road conditions, the negligence of other drivers on the roads. Participant understands the description of these risks is not complete and unknown or unanticipated risks may result in injury, illness, or death. Release of Liability, Waiver of Claims and Indemnity Agreement 1. Participant hereby releases and holds harmless Cloud 9 Skiing, LLC; it s owner and employees (the Releasees ), with respect to any and all injury, disability, death, or loss or damage to person or property, whether caused by negligence or otherwise. 2. Participant hereby releases the Releasees, their officers, directors, employees, representatives, agents, and volunteers from liability and responsibility whatsoever and for any claims or causes of action that participant, their estate, heirs, survivors, executors, or assigns may have for personal injury, pain and suffering, property damage, or wrongful death or survival claims arising from the above activities where caused by active or passive negligence of the Releasees or otherwise. By executing this document, participant agrees to hold the Releasees harmless and indemnify them in conjunction with any injury, disability, death, or loss or damage to person or property that may occur because of my engaging in the above activities. 3. By entering into this Agreement, participant is not relying on any oral or written representation or statements made by the Releasees, other than what is set forth in this Agreement. 4. This agreement shall apply to all injury, disability, death, or loss or damage to person or property occurring at any time after the execution of this agreement. 5. Participant understands that at the discretion of the program supervisor and staff any participant in the Cloud 9 Skiing Program may be dismissed from the camps without refund for inappropriate behavior. 6. Participant gives permission to use, reprint and produce any photographs or videos taken of during the skiing program. 7. Participant hereby authorizes the Releasees to act on behalf of participant in case an emergency and waive and release Cloud 9 Skiing staff from any and all liability for any and all injuries and illness occurred while attending the program. 8. Participant agrees to abide by all rules of the ski resort. I have read and understand this Release of Liability and Assumption of Risk Agreement, I fully understand its terms, I understand that I have given up legal rights by signing it, and I sign it freely and voluntarily without any inducement, and iutending to be legally bound.
