Registration will take place Saturday morning at Greek Peak. SATURDAY EVENTS 7:30-9:00 a.m. 7:45-11:00 a.m. 9:00-11:30 a.m. Registration for Day s Events at Greek Peak Adventure Park and Tubing Center All teams must register. All team members must wear PPE to registration and, once you have a tubing ticket, head to the tubing hill. Tube Racing at Greek Peak Adventure Park and Tubing Center If you are not at registration by 8:30, your team will not participate in this event. Once your team is eliminated, proceed to Stokes Basket Carry (former Sled Pull). Stokes Basket Carry (Former Sled Pull) Location in the parking lot next to the Adventure Park and Tubing Center. Firefighter Geocache (Scavenger Hunt) Complete before the start of Tug-O-War. Teams will use GPS devices and cameras (bring your own) along with written clues (provided by event staff) to find and gather evidence to simulate locating a lost person. This event will challenge the mind as well as the body. 9:30 a.m.-1:00 p.m. 11:00 a.m.-1:30 p.m. LUNCH ON YOUR OWN 1:30-4:00 p.m. Hose Relay at Greek Peak Location in parking lot next to the Adventure Park and Tubing Center. As You Have a Break in Your Events, Please Make Sure You Eat! Tailgating is encouraged. Prize awarded to best setup. Tug-O-War at Greek Peak Location in the parking lot next to the Adventure Park and Tubing Center. PLEASE NOTE: Times for the events are for reference only and may change at the discretion of the Committee on the day of the event depending on snow conditions. Team captains should check in with event staff before leaving the Greek Peak premises. SUNDAY EVENTS Broomball Tournament 7:00 a.m.-1:00 p.m. Park Center Alumni Arena at SUNY Cortland 7:00-7:30 a.m. Registration Captains sign your team up at Alumni Arena SUNY Cortland 7:30 a.m. Games Start Off the ice by 1:00 p.m. Awards Ceremony Following the Broomball Tournament. PLEASE NOTE: No alcohol or tobacco products are allowed in the ice rink. Teams in violation of this policy will be disqualified and asked to leave.

3 Greek Peak Mountain Resort Adventure Center ASSUMPTION OF RISK AGREEMENT PLEASE READ CAREFULLY THIS MAY AFFECT YOUR LEGAL RIGHTS 1.Express Assumption of Risk. I understand that participation in the Adventure Center activities, including but not limited to use of the Zip Lines, High Peaks Aerial Challenge Course, Peak Plunge, Teambuilding Activities, Mountain Coaster, Snow Tubing, Bounce Houses, Climbing Walls, Laser Tag, and Mountain Bike Park involve significant risks of serious injury or death from, including but not limited to, falling, slipping, jolting, jarring or shaking, impacting man made or natural objects or being impacted by such objects, equipment malfunction, adverse weather conditions, natural elements, including, but not limited to, trees, rocks, stumps, and uneven terrain, dizziness, fatigue, encounters with plants, insects or animals, contact with other participants or their personal property, physical exertion, and falls. I acknowledge that I have sufficient skills and am in adequate physical condition to participate in these activities, and that I understand and accept the risks of serious injury or death. 2.Participation of Minor. I am the parent or guardian of the minor on whose behalf I have signed this agreement. I have explained the risks inherent in the Adventure Center activities to my child in an age appropriate manner and he/she has acknowledged that he/she understands and accepts those risks. 3. Right of Privacy Release. I agree to the photographing, filming or videotaping of my appearance while visiting Greek Peak and the reproduction of such materials by Greek Peak Holdings and its corporate affiliates ( Greek Peak ) for Greek Peak s marketing materials ( Materials ). I agree that Greek Peak is, and will be, the sole owner of all rights in and to the Materials and the reproductions thereof. Greek Peak shall have the right, among other things, to reproduce the Materials one or more times for any and all lawful purposes, including the promotion of Greek Peak s business. Greek Peak shall also have the right to use my name, portrait, picture and biographical material for such purposes. I agree to hold Greek Peak and any third parties harmless against any liability, loss or damage, including reasonable attorneys fees caused by, or arising from, the photographing and reproduction of my appearance for the Materials. 4.Effect on Legal Rights. I have read this agreement carefully and understand its contents. I have read this agreement to my child and he/she has acknowledged that he/she understands its contents. I am aware that the agreement includes an express assumption of risk. I understand that this agreement may affect legal claims for damages in the event of death or any injury to me or my child. I acknowledge that any questions I or my child have about this agreement or the risks inherent in participating in the Adventure Center activities have been answered to my and my child s satisfaction. I have signed this document of my own free will. 5.Controlling Law/Venue. I agree that the terms of this agreement are binding on me and my child and shall be governed by the Laws of the State of New York. I also agree that any legal action relating to my or my child s participation in the Adventure Center activities will be brought in the New York State Supreme Court, County of Cortland or the United States District Court for the Northern District of New York. 6. Inclement Weather I understand that hours and activities of Greek Peak Adventure Center are dependent upon weather. No guarantees or refunds will be distributed. Revised

4 Height, Weight and Age Requirements Mountain Coaster *The rider must be at least 54 inches tall to ride alone. *The rider must be at least 38 in. tall and 3 years old to ride with a companion who is 16 years of age or older. *To ride double, at least one rider must be 54 in. tall and/or 16 years of age at our discretion, with the taller rider in back. *There is a maximum combined weight of 350 lbs. per car (330 lbs. in wet weather.) Zip Lines *Participants must be at least 7 years of age and 48 in. tall to ride. *Weight restrictions are 60 lbs. minimum, 270 lbs. maximum. Eurobungy *Participants must be 32 in. tall to use the eurobungy jump. *Weight restrictions are 40 lbs. minimum, 200 lbs maximum. High Peaks Aerial Challenge Course & Peak Plunge Jump Bag *Green: Participants must be at least 7 years of age and 48 tall; between the weights of 60 & 270 lbs. *Blue: Participants must be at least 7 years of age and 60 tall; between the weights of 60 & 270 lbs *Black: Participants must be at least 15 years of age and 60 tall; between the weights of 60 & 270 lbs *Peak Plunge Jump Bag: Participants must be at least 9 years of age and 48 tall; between weights of 60 & 270 lbs. Bounce House/ Kiddie Climbing Wall *Participants must be at least 3 years of age and between 36 in. and 60 in. tall. Laser Tag *Participants must be at least 6 years of age. 5 ASSUMPTION OF RISK, WAIVER, RELEASE and HOLD HARMLESS IN CONSIDERATION of your acceptance of the application for entry and participation of Participant Name, in the Firefighter Winter Games, held February 9th - 10th, 2019, at Resort and SUNY Alumni Arena at Park Center., and Greek Peak Ski Resort, in Cortland New York, sponsored by the Firemen's Association of the State of New York, and other entities, without fee or compentsation to the Firemen's Association of the State of New York, I, as Participant [and I, as parent or guardian of * Participant Name ], hereby agree, acknowledge and represent as follows: 1. The undersigned understand(s) that the participation in some or all of the Firefighter Winter Games,consisting of events generally known as the "Tubing Races", "Hose Relay Competition", "Stokes Basket Carry "Tug-O-War", "Scavenger Hunt" and "Broomball Tournament" may result in injury or damage, in the form of personal injuries, death and/or property damage. 2. The undersigned is/are familiar with the goals and purposes, dangers, risks, requirements,rules, practices, systems and objects of the events in which the Participant is participating and have received information concerning these events and have had an opportunity to investigate and observe them. 3. The undersigned represent(s) that the Participant is in good physical condition and physically fit and able to participate in these events and that the Participant will not participate while under the influence of drugs or alcohol; that it is understood that at any time the Participant may withdraw from any activity, for any reason. 4. The undersigned assume(s) full responsibility for any injury or damage, including injury,death or property damage, which may occur to the Participant or others while participating in, or being involved in the Firefighter Winter Games, including, but not necessarily limited to, any claims for personal injury, death or property damage resulting from or arising out of the Participant's acts or the acts or negligence of the Firemen's Association of the State of New York, or the officers, agents, representatives, servants or employees of the Firemen's Association of the State of New York, Cortland County Firemen's Association and the State of New York. 5. Participant, and if a minor, parent or guardian, hereby waive(s) any and all claims that Participant, his or her heirs, successors, representatives and/or assigns may have by reason of injury, death or property damage sustained by Participant in Participant's participation and do hereby release and discharge the Firemen's Association of the State of New York and the officers, agents, representative, servants or employees thereof from any claims for injury or damage against them, or any of them, which Participant may have by reason of Participant's entry and participation in said Firefighter Winter Games and for any improper or negligent acts in the conduct of the events. 6. The undersigned also do hereby indemnify and hold harmless the Firemen's Association of the State of New York, Cortland County Firemen's Association and the State of New York against all loss, liability, cost, damage, act or injury to persons or property occurring as a result of Participant's acts in the Firefighter Winter Games herein. 7. Participant, and if a minor, parent or guardian, have read this Assumption of Risk, Waiver, Release and Hold Harmless and fully understand its terms and sign it after having read and reviewed it, with our signatures being freely and voluntarily given.
