1 WINSPORT HOCKEY CAMP PROGRAM INFORMATION PACKAGE The following is important information to ensure your child has a great week with us. Before your Arrival What to Bring Camp ADD-ONS To ensure a smooth check-in process on the first day of camp, please print and complete the attached forms: Participant Information Form (2 Pages) Summer Camp Waiver Please Note: Waiver Forms are mandatory for each child that has been registered in a camp at WinSport. If your child would like to be in the same group as another camp participant, please skate@winsport.ca no later than five (5) days prior to the start of your child s camp. First Day Check-In On the First Day of Camp, please arrive between 7:45am and 8:30am at the Markin MacPhail Centre. **Discover Hockey Participants should arrive between 8:30am- 9:00am. Please refer to the attached map for directions. There is ample free parking available in the area. Lunch (if lunch program was not purchased) Snacks for the morning and afternoon breaks Water Bottle Hat Sunscreen Sunglasses Bug Repellent Comfortable Athletic Shoes (no sandals please) Change of Clothes Sweater/Pants/Raingear* Ball Hockey Stick Full Hockey Gear Bikes and Blades Camp Mountain Bike (If Mountain Bike rental was not purchased) Helmet Armour (knee, shins, elbow, gloves) Recommended Bike Lock * Our Camps are designed to run in all weather conditions. Please provide appropriate clothing to ensure your child will be protected from the elements during outside camp activities. Please label all belongings with your Child s First and Last Name. Lunch Program is optional and may only be purchased for the entire week $60 /5 day week $48 /4 day week Bikes and Blades Camp Only Mountain Bike Rentals $75 /5 day week REFUND & CANCELLATION POLICY Before/After Care is available if needed at no additional charge and located on the 2nd Floor of the Marking MacPhail Centre above visitcalgary.com arena (Arena D). Before Child Care is between 7:30am - 8:30am After Child Care is between 4:30pm - 5:30pm *Camp ADD-ONS must be pre-purchased by calling Guest Services Cancellations greater than 14 days prior to the start date will be refunded less a $20.00 processing fee. No additional documentation is required. Cancellations between 14 and 5 days prior to the start date where a doctor s certificate is provided will be refunded less a $20.00 processing fee. Cancellations between 14 and 5 days prior to the start date where a doctor s certificate is not provided will be credited to the purchaser s account less a $20.00 processing fee. Cancellations received less than 5 days prior to the start date will not be refunded. In the event that WinSport is required to cancel, a credit will be provided. Credits will be kept on the purchaser s account for one calendar year. You may choose for your child to be signed out by a parent or guardian, or you may choose to have your child sign him/herself out. Please indicate your choice by checking the appropriate box. Please note on the first day of camp you will be required to give your child s instructor a memorable pin number or code word that must be supplied to staff before being able to pick up your child. For security reasons only those listed below will be authorized to sign out your child. I agree that WinSport may use all or any part of his/her likeness, and may alter or modify it, regardless of whether or not he/she is recognizable, and may use my child s name in connection therewith if WinSport so chooses. I hereby agree that all taped footage, images, photographs and interviews will remain the sole property of WinSport, for future use at WinSport s discretion. I hereby release and discharge WinSport from any and all claims and demands arising out of or in connection with the use of such material, including any and all claims related to rights of publicity or privacy or defamation. Further, I consent to the use of such material or its reproduction in any manner and by any medium by which WinSport and its representatives may deem appropriate. This authorization and release shall also ensure to the benefit of the legal representatives, licensees and assigns of WinSport and any other parties or person(s) for whom WinSport took the photographs, images or video. 4 RELEASE OF LIABILITY, WAIVER OF CLAIMS, ASSUMPTION OF RISKS AND INDEMNITY AGREEMENT BY SIGNING THIS DOCUMENT YOU WILL WAIVE CERTAIN LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE PLEASE READ CAREFULLY! INITIAL The Participant Name Address Last First Initial Street City Province Postal Code IN CONSIDERATION of permission granted now or in the future by the Calgary Olympic Development Association to the above named person to participate in a summer camp program operated by the Canada Olympic Development Association, the Participant agrees and acknowledges: In this Agreement: (a) The Activities include but are not limited to the following sporting and recreational activities, either collectively or individually; Archery, Biathlon, Swimming, Zipline, Ski Jumping, Skyline Luge, Activity Centre, Amazing Race, Ball Hockey, Biathlon, Biking, Closing Ceremony, Canada s Sports Hall of Fame, Flag Rugby, Fitness Testing, Fundamental Movement Skills ( FMS ), Geocaching, Gymnastics, Handball, High Ropes, Hiking/ Hike to Big Rock, Ice Hockey, Ice Skating, Lacrosse, Low Ropes, Mini Golf, Opening Ceremony, Outdoor Living Skills, Parachute Games, Park Explorer, Parkour, Ringette, Trampoline, and Wilderness Games. (b) (c) (d) The Facilities means the premises and facilities known as Canada Olympic Park as well as such other off-site third party facilities as may be used from time to time in conjunction with Activities conducted as part of the summer camp program including, but not necessarily limited to, the Calgary North East Sports Facility, the Bowview Outdoor Pool, the Peter Lougheed Provincial Park (Kananaskis Country) and Canmore Nordic Centre; The Releasees means Canada Olympic Development Association and its directors, officers, employees, instructors, agents, representatives, independent contractors, subcontractors, volunteers, successors and assigns, as well as any third party facility providers used in conjunction with the Activities and Facilities; Participating in means going to, coming from, before, after or during. ASSUMPTION OF RISKS The Participant is aware that participating in the Activities involves many risks, dangers and hazards including, but not limited to personal injury, death or property loss or loss resulting therefrom, and NEGLIGENCE ON THE PART OF THE RELEASEES. THE PARTICIPANT UNDERSTANDS THAT NEGLIGENCE INCLUDES FAILURE ON THE PART OF THE RELEASEES TO TAKE REASONABLE STEPS TO SAFEGUARD OR PROTECT THE PARTICIPANT FROM THE RISKS, DANGERS AND HAZARDS REFERRED TO ABOVE. The Participant is also aware that the risks, dangers and hazards referred to above exist throughout the Facilities and that many hazards are unmarked. THE PARTICIPANT FREELY ACCEPTS AND FULLY ASSUMES ALL SUCH RISKS, DANGERS AND HAZARDS AND THE POSSIBILITY OF PERSONAL INJURY, DEATH, OR PROPERTY DAMAGE OR LOSS RESULTING THEREFROM. RELEASE OF LIABILITY, WAIVER OF CLAIMS AND INDEMNITY AGREEMENT In consideration of the Releasees accepting my application for participation in the Activities at Canada Olympic Park in Calgary, Alberta and permitting my use of Facilities, the Participant hereby agrees as follows: 1.TO WAIVE ANY AND ALL CLAIMS that the Participant has or may in the future have against THE RELEASEES, and TO RELEASE THE RELEASEES from any and all liability for any loss, damage, expense or injury including death that the Participant may suffer, or that the Participant s next of kin may suffer, resulting from either the Participant s use of or the Participant s presence on the facilities or participation in the Activities, DUE TO ANY CAUSE WHATSOEVER, INCLUDING NEGLIGENCE, BREACH OF CONTRACT, OR BREACH OF ANY STATUTORY OR OTHER DUTY OF CARE, INCLUDING ANY DUTY OF CARE OWED UNDER ANY APPLICABLE OCCUPIERS LIABILITY LEGISLATION ON THE PART OF THE RELEASEES. THE PARTICIPANT UNDERSTANDS THAT NEGLIGENCE INCLUDES FAILURE ON THE PART OF THE RELEASEES TO TAKE REASONABLE STEPS TO SAFEGUARD OR PROTECT THE PARTICIPANT FROM THE RISKS, DANGERS AND HAZARDS REFERRED TO ABOVE; 2. TO HOLD HARMLESS AND INDEMNIFY THE RELEASEES from any and all liability for any damage to property of or personal injury to any third party, resulting from the Participant s use of or presence on the facilities or participation in the Activities; 3. This Release Agreement shall be effective and binding upon the Participant s heirs, next of kin, executors, administrators, assigns; and representatives, in the event of the Participant s death or incapacity; 4. This Release Agreement and any rights, duties and obligations as between the parties to this Release Agreement shall be governed by and interpreted solely in accordance with the laws of the Province of Alberta and no other jurisdiction; 5. Any litigation involving the parties to this Release Agreement shall be brought solely within the Province of Alberta, and shall be within the exclusive jurisdiction of the Courts of the Province of Alberta; and 6. In entering into this Release Agreement the Participant is not relying upon any oral or written representations or statements made by the Releasees with respect to the safety of participating in the Activities other than what is set forth in this Agreement. THE PARTICIPANT OR HIS/HER LEGAL GUARDIAN HAVE READ AND UNDERSTAND THIS RELEASE AGREEMENT, AND ARE AWARE THAT BY SIGNING THIS AGREEMENT, THE PARTICIPANT IS WAIVING CERTAIN LEGAL RIGHTS WHICH THE PARTICIPANT OR THE PARTICIPANT S HEIRS, NEXT OF KIN, EXECUTORS, ADMINISTRATORS, ASSIGNS AND REPRESENTATIVES MAY HAVE AGAINST THE RELEASEES.
