1 2018 Bowdoin Summer Art Camp Registration Hours and Location Bowdoin Summer Art Camp will run for four weeks from June 25 th through July 27 th, with no classes being held 4 th of July week. The times and length of each session vary. Please refer to the individual listings on the brochure to confirm times and dates. All sessions will take place in the Edwards Art Center. Tuition and Materials Fee All art supply and reception costs are included in the tuition fee charged for any given week. Fees vary for each session to accommodate the length of session, guest artists, and materials needed. T-Shirts A limited number of printed t-shirts will be available for purchase on the first and last day of camp each week. Bowdoin Summer Art Camp Registration To register, please send the registration materials with your deposit check to: Bowdoin Summer Art Camp Events and Summer Programs 3200 College Station Brunswick, Maine Please use a separate form for each child and enclose a non-refundable deposit of $ per session for each camper. Make checks payable to Bowdoin College. If you wish to pay in full, that is perfectly acceptable. All outstanding balances must be paid by the first day of your child s class. Upon receiving the registration materials, an information sheet will be ed to confirm your child s enrollment. In the event that a particular session is full, we will notify you by phone or . If this is the case, your child may be put on a waiting list, or you may have your deposit returned to you. In the event that a session is under-enrolled, Bowdoin Summer Art Camp reserves the right to cancel the session. For more information please call (207) , or artcamp@bowdoin.edu. 3 2018 Bowdoin Summer Art Camp Release and Indemnity Agreement Participant s Name In consideration of Bowdoin College (the College ) accepting the Participant into the Bowdoin Summer Art Camp ( the Summer Program ), the undersigned agrees as follows: 1. The undersigned understands that the Participant s involvement in the Summer Program at the College poses risks of personal injury, loss or damage to property, or loss of life. The undersigned knowingly assumes any and all risks connected with Participant s involvement in the Summer Program and hereby waives any right to make any claim against the College, its officers, employees, Trustees, or agents. 2. The undersigned, on behalf of himself/herself and the Participant, hereby releases the College, its employees, and agents from any claim. The undersigned further agrees to indemnify and hold harmless the College, its officers, employees, Trustees, or agents from any such claim. 3. This indemnity, waiver and release includes, without limitation, any claim arising out of negligence or alleged negligence on the part of the College, its officers, employees, Trustees, or agents. 4. For purposes of this Agreement, the term claim means any claim, cause of action, damage or liability, specifically including attorney s fees, relating to or in connection with the Participant s involvement in the Summer Program. 5. This Agreement shall be governed by Maine law and shall be binding on the heirs, personal representatives and successors of the undersigned. If any part of this Agreement is found to be unenforceable by a court or other body having jurisdiction, the provisions shall be altered and not eliminated as may be considered reasonable, and as amended shall be enforced. 6. As a condition of, and in consideration of, the acceptance of this application, therefore, the undersigned parent/guardian consents to have administrators of the Summer Program act in his/her behalf should an emergency arise, and hereby grants permission to said administrators to authorize medical attention and treatment. The consenting guardian or parent understands that any medical treatment is not payable by the College. 7. The undersigned understands that it is a requirement of the Summer Program that all Participants be covered by health insurance. 5 2018 BOWDOIN SUMMER ART CAMP PHOTO RELEASE (OPTIONAL FORM) I understand that the photograph(s) taken of me or my child or children by agents, employees or representatives of Bowdoin College shall be used in connection with the College s dissemination of information on its academic and other programs to the general public. I hereby irrevocably authorize Bowdoin College and its agents, employees or representatives to copy, exhibit, publish or distribute any and all such photographs of me or my child, or wherein I appear, including composite or artistic representations, and to use the said photographs in all forms and media for purposes of publicizing College programs or for any other lawful purpose. In addition, I waive any right to inspect or approve the finished product, including written copy, wherein my photograph(s) appears. I hereby hold harmless and release and forever discharge Bowdoin College and all of its agents, employees and representatives, and their successors, from all claims, demands and causes of action which I, my heirs, representatives, executors, administrators or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.
