2 Dear 8 th and 9 th grade students and family members, We will be going up to Camelback Ski Area this year on Tuesdays. Our program runs for 5 consecutive Tuesdays. You and your family are invited to ski and snowboard with us this year. Junior High students will be using district transportation to Camelback. We will depart from the Junior High School Auditorium exit at approximately 3:15 pm. If for some reason you will not be riding the bus to Camelback, please make sure you arrive at the mountain by 4:00 pm. Parents or guardians will be responsible for your trip home each Tuesday. All ski club members must be signed out in the main lodge at our designated table by 8:00 pm. There will be NO EXCEPTIONS to this rule. We encourage car pools. However, you must be signed out by an adult, you cannot sign yourself out. If you fail to be signed out by 8:00, you will forfeit your next trip to Camelback. If you are interested in participating with us this year; please use the provided instructions for on-line purchases which can be made starting October 22nd, complete the participant registration form, season pass contract & agreement not to sue, and return your forms in an envelope with the following information: 1. Participants Name, 2. ID#, 3. Grade, 4. Homeroom Number/Teacher Name., and 5. Parent or guardian address. Envelopes must be returned by Friday, November 16th. Please return the forms to Mr. Jacobs Rm G202, or the Main office. Ski/Snowboard Dates are: 1/8, 1/15, 1/22, 1/29, 2/5 Make-ups 2/12 & 2/26. Please note: We strongly advise club participants who purchased season passes to pick up their passes prior to the start of the ski program. If doing so, parents will need to accompany their child to Camelback to pick up the season pass. Lift Tickets are valid until 10:00 pm; however, parents assume responsibility for their children by signing them out at 8:00 PM in the main lodge, at the designated table. Students cannot sign themselves out. Prepaid multi-week cards must be used by March 9th, They will not scan after this date. Missed lessons are to be made up after the 5 week program ends, on the same night and at the same lesson time originally scheduled for the group. Group participants may switch from skis/snowboard or vice versa however, this must be done prior to their third trip. Camelback only issue pro-rated refunds for group members that have been injured during the course of the program. Refunds will be issued in the manner in which they were paid and sent directly to the source of the payment. Discounted helmets are available through The Loft. If you wish to purchase a helmet for your child, please see the attached coupon on the last page of the packet. If purchasing a package that contains a rental, the rental will be added in a future step and the total cost will be calculated at the end of the sale. Once item is selected: Add to Cart -If the products you purchased included rentals the rental will need to be added to the cart. Cart Contents gives you the ability to review your purchase. If you need to purchase additional items click continue shopping. Once you are done shopping you will need to assign a guest for each item added to your cart. If you have ever purchased anything online from either the Camelback or Camelbeach website, you are an Existing Guest. Enter your first and last name and click Look Me Up. You will need to put in the Password you originally created or click Forgot Password and you will be issued a new password via . Passwords are case sensitive. Click Verify
4 For existing guest Once you have located the existing guest, the Guest Details will be displayed. Click continue if nothing has changed. Click edit and update if there are changes. Click Save. Click Continue. If rentals were purchased you will need to add/change guest for the rental item as well. Click on add/change guest. Click on the participants name under Logged in Guest. Click edit, scroll down and update the rental information. Click Save. Click Continue. If you have never purchased products online from either Camelback or Camelbeach, you are a New Guest. Click on Create New Guest under the heading of New Guests. Complete the Guest Edit section. Click Save. The Guest Details page will display. Click Continue. If rentals were purchased you will need to add/change guest for the rental item. Click on add/change guest. Click on the participants name under Logged in Guest. Click edit and update the rental information. Click Save. Click Continue. If everything is correct, Click Proceed to Checkout The next screen will give you one more chance to review your purchase. (Ignore the Have A Promo Code? ) Review Terms and Conditions and check the box if accepted. NOTE: Our website is temporarily unable to display the Terms and Conditions. Your group leader will provide you with a copy. The box must be checked to be able to proceed with the order. Scroll down the Please Enter or verify Purchaser Details page Verify billing name and address. This would be information pertaining to the Credit Card holder or parent/guardian. Enter Payment Information If paying in full, with a credit card, enter credit card information and proceed as directed. 6 CAMELBACK RESORT WINTER SEASON PASS CONTRACT RELEASE, AGREEMENT NOT TO SUE AND ACKNOWLEDGEMENT OF RISKS THIS IS A CONTRACT READ IT! This AGREEMENT is between Camelback Resort, its Operating Divisions, CBH2O, LP, and CBK Lodge, LP, and their owners, officers, directors, partners, members, investors, operators, management companies, agents, servants and employees (hereinafter collectively referred to as "Camelback") and the Patron, Parent/Guardian or Accompanying Adult identified on the signature line below. IN CONSIDERATION of the payment of the applicable season pass rate, Camelback hereby grants to the undersigned Patron, family member or ward thereof the privilege and license to use all lifts and ski facilities at Camelback during the applicable winter season(s) and during all operating hours as specified by the applicable pass upon the following terms and conditions: 1. Passes are not transferable or assignable. They may not be given or loaned to anyone under any circumstances. Violation of this condition shall result in revocation of the pass without refund. Pass holders not in possession of their season passes are required to purchase a lift ticket at full price in order to use the ski facilities. 2. If a pass is lost or stolen, I agree to report the loss in writing immediately. Failure to do so may result in revocation of the pass, without refund. A $75 charge will be imposed for a replacement pass. A defective pass will be replaced free of charge upon surrender of the pass. 3. This pass does not include or guarantee automobile parking. I understand that, on some busy days, parking may not be available. 4. I agree to comply with all rules and regulations pertaining to season passes that may, from time to time, be adopted by Camelback, and to abide by the Skiers Responsibility Code when using Camelback s ski facilities. I acknowledge receipt of a copy of the current rules and regulations. 5. The pass may be suspended and/or revoked by Camelback, without refund, in the event of a violation of any rule or regulation, misrepresentation of facts to obtain an applicable season pass rate, or for violation of the Skiers Responsibility Code. RELEASE AND AGREEMENT NOT TO SUE 6. IN CONSIDERATION OF BEING ALLOWED TO USE CAMELBACK S FACILITIES, I AGREE THAT I WILL NOT SUE CAMELBACK AND WILL RELEASE CAMELBACK FROM ANY AND ALL LIABILITY IF I OR ANY MEMBER OF MY FAMILY IS INJURED WHILE USING ANY OF THE CAMELBACK FACILITIES OR WHILE BEING PRESENT AT THE FACILITIES, EVEN IF I CONTEND THAT SUCH INJURIES ARE THE RESULT OF NEGLIGENCE, INCLUDING GROSS NEGLIGENCE, SOLE OR JOINT, OR ANY OTHER IMPROPER CONDUCT FOR WHICH A RELEASE IS NOT CONTRARY TO PUBLIC POLICY, ON THE PART OF CAMELBACK. I FURTHER AGREE THAT I WILL INDEMNIFY AND HOLD HARMLESS CAMELBACK FROM ANY LOSS, LIABILITY, DAMAGE OR COST OF ANY KIND THAT MAY OCCUR AS THE RESULT OF ANY INJURY TO MYSELF, TO ANY MEMBER OF MY FAMILY OR TO ANY PERSON FOR WHOM I AM SIGNING THIS AGREEMENT, EVEN IF IT IS CONTENDED THAT SUCH INJURIES ARE THE RESULT OF NEGLIGENCE, SOLE OR JOINT, INCLUDING GROSS NEGLIGENCE, OR ANY OTHER IMPROPER CONDUCT FOR WHICH A RELEASE IS NOT CONTRARY TO PUBLIC POLICY, ON THE PART OF CAMELBACK. I FURTHER ACKNOWLEDGE THAT THIS AGREEMENT EXTENDS TO MY PRESENCE AT CAMELBACK PURSUANT TO ALL COMPLIMENTARY, PURCHASED, TRADES, EMPLOYEE DISCOUNTED AND VOLUNTEER DISCOUNTED TICKETS OR ANY OTHER TICKETS, PASSES, OR CARDS, PROMOTIONAL OR OTHERWISE THAT I HAVE OR MAY OBTAIN. 7. Notwithstanding the foregoing, if I sue Camelback, I agree that I will only bring suit, whether it be on my own behalf or on behalf of a family member, in the Court of Common Pleas of Monroe County or in the United States District Court for the Middle District of Pennsylvania and further agree that any and all disputes which might arise between Camelback and myself or my family member(s) shall be litigated exclusively in one of said Courts. I understand and agree that this Agreement Not to Sue is governed by the laws of Pennsylvania. I further agree that if any part of this Agreement Not to Sue is determined to be unenforceable, all other parts shall be given full force and effect. ACKNOWLEDGEMENT OF RISKS 8. I understand that there are inherent and other risks in the sports of skiing, snowboarding, snowtubing and other snow-related sports and associated activities (hereinafter referred to collectively as snow sports ), including the use of lifts and other forms of conveyance. These risks include, but are not limited to, variations in snow, steepness and terrain, trail side drop-offs, ice and icy conditions, moguls, rocks, trees, and other forms of forest growth or debris (above or below the surface), bare spots, lift towers, utility lines, poles and guy wires, snowmaking equipment and component parts, trail fences and control nets, and the absence of such fences and nets, Terrain Park features and elements (such as halfpipe, jumps, rails, rollers, hits, etc.), and other forms of natural or manmade obstacles on and/or off designated trails, as well as collisions with equipment, including snowmobiles, obstacles or other skiers, participants or observers. I understand trail fences are located along the side of some but not all trails; such fences are not designed to protect you from injury if you run into them or go through, over or under them. Likewise, I understand trail conditions vary constantly because of weather changes and skier use. I understand that a helmet does NOT ELIMINATE THE RISK of injury or death and helmets have only limited capability for shock absorption and that, although the helmet may reduce or mitigate the severity of injuries to the head, it is not a guarantee of the participant s safety. I understand that all of the inherent and other risks of participating in snow sports present the risk of injury, including death, that skiing, snowboarding and snowtubing are DANGEROUS risk sports, and that injuries can be serious and even fatal. I HEREBY ACKNOWLEDGE, ACCEPT, AND ASSUME THESE RISKS AND ALL OTHER RISKS THAT CAN AND DO EXIST WHILE PARTICIPATING IN THE SPORTS OF SKIING, SNOWBOARDING AND SNOWTUBING WHILE ENGAGED IN OR OBSERVING ANY SUCH ACTIVITY AT CAMELBACK. * If the Season Pass Holder is under eighteen (18) years of age, this Contract must be executed by a parent/guardian or Accompanying Adult. I have read and understood the above RELEASE, AGREEMENT NOT TO SUE and ACKNOWLEDGEMENT OF RISKS, and I am voluntarily signing below, manually or electronically, with the intent to be legally bound by this Contract. I represent and warrant that I am signing this Contract with the consent of my Spouse (if any) and understand that I may be giving up the rights of my child and spouse to sue as well as giving up my own right to sue.
