ALL PLAYERS AND AT LEAST ONE PARENT OR GUARDIAN MUST ATTEND. NOTE SINCE YOU DID NOT REGISTER AS A MEMBER OF XCELERATION WITH THE NCVA PRIOR TO TRYOUTS YOU MUST GO BACK INTO THE NCVA SITE AND AFFILIATE YOURSELF WITH XCELERATION. FORMS THAT NEED TO BE FILLED OUT ARE IN THIS SIGNING PACKAGE, OR ON OUR WEBSITE ( UNDER FORMS. PRINT AND COMPLETELY FILL OUT ALL FORMS PRIOR TO COMING TO THE MEETING TO HELP THINGS RUN SMOOTHLY! PRINT OUT and COMPLETE the NCVA Letter of Commitment and USAV Medical Release forms (click links). These are also available on the Xceleration website. FIRST INSTALLMENT AND TRAVEL DEPOSITS (IF APPLICABLE) MUST BE PAID (checks or Paypal with transaction/convenience fee accepted). PLAYERS WILL BE TRYING ON UNIFORMS AND SWEATS FOR SIZING. 3 COMMITMENT CONTRACT By signing below, player and parent agree to the following with the Xceleration Volleyball Club: 1. The player commits to playing with Xceleration Volleyball Club 2. To pay all fees associated with the Club for dues per the schedule received on Signing Night. If the player chooses to leave the club during the season for any reason, the full season s fees remain the responsibility of the player and parents. 3. To pay all fees associated with the club for travel by the due dates. In the event that a player does not attend a travel tournament, they are still responsible for their share of the coaches and team costs. If air travel is involved, once ticketed the air fare is non-refundable. 4. No refunds can be given for dues or travel for any reason. 5. 4 WAIVER OF LIABILITY FOR GYM USE I/We hereby understand and acknowledge that the training, programs and events held by the Xceleration VBC may expose me to many inherent risks, including accidents, injury, illness, or even death. I/We assume all risk of injuries associated with participation including, but not limited to, falls, contact with other participants, the effects of the weather, including high heat and/or humidity, and all other such risks being known and appreciated by me. I/We hereby acknowledge my responsibility in communicating any physical and psychological concerns that might conflict with participation in activity. I/We acknowledge that I am physically fit and mentally capable of performing the physical activity I choose to participate in. After having read this waiver and knowing these facts, and in consideration of acceptance of my participation and the Xceleration VBC furnishing services to me, I agree, for myself and anyone entitled to act on my behalf, to HOLD HARMLESS, WAIVE AND RELEASE the Xceleration VBC, Xceleration Sports Facility/NorCal Courts Martinez, their officers, agents, employees, organizers, representatives, and successors from any responsibility, liabilities, demands, or claims of any kind arising out of my participation in the Xceleration VBC training, programs and/or events.
