What is this product about? This policy is designed to protect the Joint Management Body (JMB) or Management Corporation (MC) and the individual JMB committee members for condominium/apartment. It offers the flexibility to choose the insurance protection at an affordable premium. 2. What are the covers / benefits provided? This policy provides cover on: Section I. : Fire Section II. : Special Pack (A) Money (B) Fidelity Guarantee (C) Plate Glass (D) Burglary (E) Public Liability (F) Personal Accident Section III. : Machinery & Equipment Section IV. : Error and Omission 3. What is the Period of Cover and Renewal Option? Duration of cover is usually for one year. You need to renew your insurance policy annually. 4. How much premium do I have to pay? This is an annual policy. The premium charges are based on the package premium according to the plan chosen by insured. 5. What are the fees and charges I have to pay? What you have to pay in addition to the premium Amount Goods and Services Tax (GST) 6% of premium Stamp Duty RM What is included in the premium Commission paid to the insurance intermediaries (if any) Amount Up to 15% of premium (all sections) 6. What are some of the key terms and conditions that I should be aware of? a) DUTY OF DISCLOSURE Non-Consumer Insurance Contract Pursuant to Paragraph 4(1) of Schedule 9 of the Financial Services Act 2013, if you are applying for this Insurance for purposes related to your trade, business or profession, you have a duty to disclose any matter that you know to be relevant to our decision in accepting the risks and determining the rates and terms to be applied and any matter a reasonable person in the circumstances could be expected to know to be relevant, otherwise it may result in avoidance of your contract of insurance, refusal or reduction of your claim(s), change of terms or termination of your contract of insurance. This duty of disclosure for Non-Consumer Insurance Contract shall continue until the time the contract is entered into, varied or renewed. You also have a duty to tell us immediately if at any time after your contract of insurance has been entered into, varied or renewed with us, any of the information given in the Proposal Form is inaccurate or has changed. b) Premium Warranty Premium must be paid within 60 days from the inception date of the cover, otherwise the cover will automatically be cancelled. Please take note that you are still responsible to pay the proportion of the premium for the 60 days. c) Sum Insured You must ensure that your property is insured at the appropriate amount. d) Claims Upon the happening of an accident which gives rise to a claim, you shall notify us immediately. 2 e) Payment can be made by Cash, Credit Card or GIRO to Berjaya Sompo Insurance Berhad. Insist on a receipt for the premium paid as proof of payment for future reference. f) Depending on the circumstances of the loss or claims made during the term of policy, we may review the policy terms and conditions upon renewal or decline renewal. Note: This list is non-exhaustive. Please refer to the policy contract for the full list of terms and conditions under this policy. 7. What are the major exclusions under this policy? This policy does not cover :- a) Theft during or after the occurrence of a fire; b) Burning of property by order of any Public Authority; c) Subterranean Fire, Pollution or contamination; d) Earthquake, volcanic eruption or other convulsion of nature; e) Typhoon, hurricane, tornado, cyclone or other atmospheric disturbance; f) War, Civil War and any act of Terrorism; g) Radioactive and nuclear energy risks; h) Date recognition i) Property damage to data or software Note: This list is non-exhaustive. Please refer to the policy contract for the full list of exclusions under this policy. 8. Can I cancel my policy? You may cancel your policy at any time by giving written notice to us. Upon cancellation, any refund premium would be based on the conditions as stated in the policy contract. 9. What do I need to do if there are changes to my contact details? Apakah terma dan syarat utama yang saya harus ketahui? a) KEWAJIPAN PENDEDAHAN Kontrak Insurans Komersial Menurut Perenggan 4(1) Jadual 9 Akta Perkhidmatan Kewangan 2013, jika anda memohon insurans ini untuk tujuan yang berkaitan dengan perdagangan, perniagaan atau profesion anda, anda berkewajipan untuk mendedahkan apa-apa perkara yang anda tahu akan mempengaruhi keputusan kami dalam menerima risiko dan menentukan kadar dan terma yang dikenakan, dan apa-apa perkara yang munasabah yang boleh dijangka, jika tidak ia boleh menyebabkan pembatalan kontrak insurans, keengganan atau pengurangan ganti rugi, perubahan terma atau penamatan kontrak insurans anda. Kewajipan pendedahan untuk Kontrak Insurans Komersial hendaklah diteruskan sehingga kontrak insurans anda dimeterai, diubah atau diperbaharui dengan kami. Anda juga mempunyai kewajipan untuk memberitahu kami dengan serta-merta jika pada bila-bila masa selepas kontrak insurans anda ditandatangani, diubah atau diperbaharui dengan kami, apa-apa maklumat yang dinyatakan dalam Borang Cadangan ini tidak tepat atau telah berubah. b) Waranti Premium - Premium mesti dibayar dalam masa 60 hari dari tarikh bermulanya perlindungan, jika tidak perlindungan secara automatik akan dibatalkan. 4 c) Jumlah Diinsuranskan - Anda harus memastikan harta anda diinsuranskan pada amaun yang berpatutan. d) Tuntutan - Apabila berlakunya kemalangan yang menimbulkan suatu tuntutan, anda hendaklah memberitahu kami dengan segera. e) Bayaran boleh dibuat melalui Tunai, Kad Kredit atau GIRO kepada Berjaya Sompo Insurance Berhad. Sentiasa meminta resit untuk premium yang dibayar sebagai bukti pembayaran untuk rujukan masa depan. f) Bergantung kepada keadaan kerugian atau tuntutan dibuat semasa tempoh polisi, kami boleh mengkaji semula terma-terma dan syarat-syarat polisi pada masa pembaharuan atau menolak pembaharuan. Nota: Senarai ini bukannya senarai yang lengkap. Sila rujuk kepada kontrak polisi untuk senarai penuh bagi terma dan syarat di bawah polisi ini. 7. Apakah pengecualian utama di bawah polisi ini? Polisi ini tidak melindungi:- a) Kecurian berlaku semasa atau selepas kebakaran; b) Pembakaran harta dengan perintah daripada pihak berkuasa awam; c) Kebakaran bawah tanah atau pencemaran; d) Gempa bumi, letupan gunung berapi atau konvulsi alam semula jadi; e) Taufan, hurikan, puting beliung, siklon atau gangguan atmosfera yang lain; f) Perang, perang saudara dan mana-mana tindakan keganasan; g) Radioaktif dan nuklear risiko tenaga; h) Tarikh pengiktirafan i) Kerosakan harta kepada data atau perisian Nota: Senarai ini bukannya senarai yang lengkap. Selepas pembatalan, sebarang bayaran balik premium akan berdasarkan pada syarat-syarat seperti yang dinyatakan dalam kontrak polisi. 9. Adalah penting bagi anda untuk memaklumkan pihak kami tentang sebarang perubahan mengenai maklumat anda bagi memastikan semua surat-menyurat dapat disampaikan kepada anda tepat pada masanya. 10.
