1 HOUSEOWNER/HOUSEHOLDER INSURANCE Product Disclosure Sheet (Please read this Product Disclosure Sheet before you decide to take up this insurance. Be sure to also read the general terms and conditions.) 1. What is this product about? This policy provides you with coverage for your building and contents and covers loss or damage by fire, lightning, explosion, flood, burst pipe or by any perils mentioned in the insurance policy. 2. What are the covers / benefits provided? This policy covers: Loss or damage to your building or contents due to fire, lightning and explosion caused by gas used for domestic purposes. Loss or damage to your building or contents by aircraft, road vehicles or animals, bursting or overflowing of water tanks or pipes, theft by forcible and violent entry, windstorm, earthquake and flood. Loss of rent. Liability to third parties for accidents in your house. Property temporarily removed for sale or exhibition or to furniture depositories. Damage to mirrors. Compensation for fatal injury occurring by visible violence caused by thieves or by fire. Servants property. You may extend coverage to the following risks by paying additional premium: Subsidence, landslip, Riot, strike and malicious damage, Bush/Lalang fire, Damage by falling trees or branches and objects; Hurricane, cyclone, typhoon, windstorm to metal smoke stacks, awnings, blinds, signs and other outdoor fixtures and fittings including gates and fences; Alterations, repairs and additions; Extended theft cover; Increased limit of liability to third parties for accidents in your house. 3. What is the Period of Cover and Renewal Option? Duration of cover is usually for one year. You need to renew your insurance policy annually. 4. How much premium do I have to pay? The total premium you have to pay may vary depending on the underwriting requirements of the insurance company: Sum Insured : RM Rate applicable : % Estimated total premium : RM Note: This policy is subject to minimum premium of RM What are the fees and charges I have to pay? What you have to pay in addition to the premium Amount Goods and Services Tax (GST) 6% of premium Stamp Duty RM What is included in the premium Commission paid to the insurance intermediaries (if any) Amount Up to 15% of premium 6. What are some of the key terms and conditions that I should be aware of? a) DUTY OF DISCLOSURE Consumer Insurance Contract Pursuant to Paragraph 5 of Schedule 9 of the Financial Services Act 2013, if you are applying for this Insurance wholly for purposes unrelated to your trade, business or profession, you have a duty to take reasonable care not to make a misrepresentation in answering the questions in the Proposal Form and you must answer the questions fully and accurately. Failure to take reasonable care in answering the questions may result in avoidance of your contract of insurance, refusal or reduction of your claim(s), change of terms or termination of your contract of insurance. You are also required to disclose any other matter that you know to be relevant to our decision in accepting the risks and determining the rates and terms to be applied. Non Consumer Insurance Contract Pursuant to Paragraph 4(1) of Schedule 9 of the Financial Services Act 2013, if you are applying for this Insurance for purposes related to your trade, business or profession, you have a duty to disclose any matter that you know to be relevant to our decision in accepting the risks and determining the rates and terms to be applied and any matter a reasonable person in the circumstances could be expected to know to be relevant, otherwise it may result in avoidance of your contract of insurance, (CIMB-HOHH/PDS ) 1

2 refusal or reduction of your claim(s), change of terms or termination of your contract of insurance. This duty of disclosure for Consumer and Non-Consumer Insurance Contract shall continue until the time the contract is entered into, varied or renewed. You also have a duty to tell us immediately if at any time after your contract of insurance has been entered into, varied or renewed with us, any of the information given in the Proposal Form is inaccurate or has changed. b) You must ensure that your property is insured at the appropriate amount taking into account the renovations made to your property. c) You may select to insure your property on Market Value or Reinstatement Value basis: Market value we will pay the cost of repairing the damaged property less the amount for wear, tear and depreciation. Reinstatement value we will pay the full cost of repairing the damaged property without any deductions being made for wear, tear or depreciation, provided that the sum covered is adequate to cover the total cost of reinstating the property. d) If the amount insured in your policy is lower than the actual value at the time of loss, you will be deemed as self-insuring for the difference, i.e. in the event of loss/damage, you will have to bear a proportional share of loss. Should you insure your property at a higher sum than its actual value at the time of loss, the maximum compensation you will receive is the actual value which is the lesser amount than your sum insured. e) Depending on the circumstances of the loss or claims made during the term of the policy, we may review the policy terms and conditions upon renewal or decline renewal. g) Payment can be made by Cash, Credit Card or GIRO to Berjaya Sompo Insurance Berhad. Insist on a receipt for the premium paid as proof of payment for future reference. h) Depending on the circumstances of the loss or claims made during the term of policy, we may review the policy terms and conditions upon renewal or decline renewal. i) Premium Warranty - The premium due must be paid and received by the insurer within sixty (60) days from the inception date of this policy/endorsement/renewal certificate. Failure to pay the premium within this period, the contract is automatically cancelled and insurer is entitled to the pro rata premium on the period they have been on risk. Note: This list is non-exhaustive. Please refer to the policy contract for the full list of terms and conditions under this policy. 7. What are the major exclusions under this policy? This Policy does not cover:- a) War, Civil War and any act of Terrorism. b) Radioactive and nuclear energy risks. c) Date recognition. d) Property damage to data or software. e) The Liability to the Public section does not cover any asbestos related injury or damage involving the use, presence, existence, detection, removal, elimination or avoidance of asbestos or exposure or potential exposure to asbestos; Note: This list is non-exhaustive. Please refer to the policy contract for the full list of exclusions under this policy. 8. Can I cancel my policy? You may cancel your policy at any time by giving written notice to us. Upon cancellation, you are entitled to a refund of the premium less premium based on short period rates for the policy which has been in force, subject to the minimum premium to be retained by us. No refund premium is allowed if there is a claim under the policy. 9. What do I need to do if there are changes to my contact details? It is important that you inform us if there are changes in your contact details to ensure that all future correspondences reach you in a timely manner. 10. Where can I get further information? 4 INSURANS EMPUNYA RUMAH / ISI RUMAH Lembaran Pendedahan Produk (Sila baca Lembaran Pendedahan Produk ini sebelum anda membuat keputusan untuk membeli insurans ini. Pastikan anda juga telah membaca terma dan syarat am.) 1. Apakah produk ini? Polisi ini memberi perlindungan kepada bangunan anda dan kandungan di dalamnya dan melindungi kerugian atau kerosakan akibat kebakaran, kilat, letupan, banjir, paip pecah atau oleh apa-apa peril yang dinyatakan dalam polisi insurans ini. 2. Apakah perlindungan/faedah yang diberikan? Polisi ini melindungi: Kerugian atau kerosakan bangunan anda atau kandungan di dalamnya yang disebabkan oleh kebakaran, kilat dan letupan yang berpunca daripada gas yang digunakan untuk tujuan domestik. Kerugian atau kerosakan bangunan anda atau kandungan di dalamnya yang disebabkan oleh pesawat udara, kenderaan di jalan raya atau binatang, tangki air atau paip pecah atau melimpah, menerobos masuk atau pecah masuk oleh pencuri, ribut angin, gempa bumi dan banjir. Kehilangan sewa. Liabiliti kepada pihak ketiga akibat kemalangan di dalam rumah anda. Harta dipindahkan sementara untuk jualan atau untuk pameran atau ke tempat simpanan perabot. Kerosakan pada cermin. Pampasan terhadap kecederaan maut yang berlaku akibat kekerasan ketara oleh pencuri atau kebakaran. Harta orang gaji. Anda boleh melanjutkan perlindungan ke atas risiko berikut dengan membayar premium tambahan : Tanah tenggelam, tanah runtuh, rusuhan, mogok dan kerosakan niat jahat, kebakaran belukar/lalang, kerosakan akibat pokok atau dahan pokok dan objek yang tumbang; Kerosakan disebabkan oleh hurikan, siklon, taufan atau ribut angin kepada cerobong asap logam, bidai, kajang, papan tanda dan lekapan atau lengkapan luar yang lain termasuk pintu pagar dan pagar; Perubahan, pembaikan dan penambahan; Perlindungan yang dilanjutkan ke atas kecurian; Menambahkan had liabiliti kepada pihak ketiga akibat kemalangan di dalam rumah anda. 3. Apakah Tempoh Perlindungan dan Pembaharuan Pilihan? Tempoh perlindungannya ialah selama satu tahun. Anda dikehendaki membaharui polisi insurans anda setiap tahun. 4. Berapakah premium yang saya perlu bayar? Jumlah premium yang anda perlu bayar mungkin berbeza-beza bergantung kepada keperluan pengunderaitan syarikat insurans: Jumlah Diinsuranskan : RM Kadar yang dikenakan : % Anggaran jumlah premium : RM Nota: Polisi ini tertakluk kepada premium minimum sebanyak RM Apakah yuran dan caj yang saya perlu bayar? Apa yang anda perlu bayar sebagai tambahan kepada premium Jumlah Cukai Barangan dan Perkhidmatan (CBP) 6% daripada premium Duti Setem RM Apa yang termasuk dalam premium Komisen yang dibayar kepada pengantara insurans (jika ada) Jumlah Sehingga 15% daripada premium 6. Apakah terma dan syarat utama yang saya harus ketahui? a) KEWAJIPAN PENDEDAHAN Kontrak Insurans Pengguna Menurut Perenggan 5 daripada Jadual 9 Akta Perkhidmatan Kewangan 2013, jika anda memohon insurans ini sepenuhnya untuk tujuan yang tidak berkaitan perdagangan, perniagaan atau profesion anda, anda mempunyai kewajipan untuk mengambil langkah yang munasabah untuk tidak salah nyata dalam menjawab soalan-soalan dalam Borang Cadangan ini. Anda dikehendaki menjawab soalan-soalan dalam Borang Cadangan ini dengan lengkap dan tepat. Kegagalan untuk mengambil langkah yang munasabah dalam menjawab soalan-soalan, mungkin mengakibatkan pembatalan kontrak insurans anda, keengganan atau pengurangan gantirugi, perubahan terma atau penamatan kontrak insurans anda. Anda dikehendaki untuk mendedahkan apa-apa perkara lain yang anda tahu akan mempengaruhi keputusan kami dalam menerima risiko dan menentukan kadar dan terma yang dikenakan. (CIMB-HOHH/PDS ) 1

5 Kontrak Insurans Komersial Menurut Perenggan 4(1) Jadual 9 Akta Perkhidmatan Kewangan 2013, jika anda memohon insurans ini untuk tujuan yang berkaitan dengan perdagangan, perniagaan atau profesion anda, anda berkewajipan untuk mendedahkan apa-apa perkara yang anda tahu akan mempengaruhi keputusan kami dalam menerima risiko dan menentukan kadar dan terma yang dikenakan, dan apa-apa perkara yang munasabah yang boleh dijangka, jika tidak ia boleh menyebabkan pembatalan kontrak insurans, keengganan atau pengurangan ganti rugi, perubahan terma atau penamatan kontrak insurans anda. Kewajipan pendedahan untuk Kontrak Insurans Pengguna dan Kontrak Insurans Komersial hendaklah diteruskan sehingga kontrak insurans anda dimeterai, diubah atau diperbaharui dengan kami. Anda juga mempunyai kewajipan untuk memberitahu kami dengan serta-merta jika pada bila-bila masa selepas kontrak insurans anda ditandatangani, diubah atau diperbaharui dengan kami, apa-apa maklumat yang dinyatakan dalam Borang Cadangan ini tidak tepat atau telah berubah. b) Anda harus memastikan harta anda diinsuranskan pada amaun yang berpatutan dengan mengambil kira pengubahsuaian yang dilakukan pada harta tersebut. c) Anda boleh menginsuranskan harta anda berasaskan dasar Nilai Pasaran atau Nilai Pengembalian Semula: Dasar Nilai Pasaran pihak kami akan membayar kos pembaikan ke atas harta yang rugi atau rosak setelah ditolak amaun susut nilai. Dasar Nilai Pengembalian Semula pihak kami akan membayar kos penuh untuk pembaikan harta yang rosak tanpa menolak susut nilai, dengan syarat jumlah perlindungan adalah mencukupi untuk menampung jumlah kos pengembalian semula harta tersebut. d) Sekiranya jumlah diinsuranskan dalam polisi anda adalah kurang daripada nilai sebenar pada waktu kerugian berlaku (contohnya insurans terkurang), anda dianggap harus menanggung sendiri terhadap perbezaannya. Syarat purata akan dikenakan jika berlakunya tuntutan. e) Bergantung kepada keadaan kerugian atau tuntutan dibuat semasa tempoh polisi, kami boleh mengkaji semula terma-terma dan syarat-syarat polisi pada masa pembaharuan atau menolak pembaharuan. f) Bayaran boleh dibuat melalui Tunai, Kad Kredit atau GIRO kepada Berjaya Sompo Insurance Berhad. Sentiasa meminta resit untuk premium yang dibayar sebagai bukti pembayaran untuk rujukan masa depan. h) Bergantung kepada keadaan kerugian atau tuntutan dibuat semasa tempoh polisi, kami boleh mengkaji semula terma-terma dan syarat-syarat polisi pada masa pembaharuan atau menolak pembaharuan. j) Premium Jaminan - Premium yang perlu dibayar mesti dibayar dan diterima oleh penanggung insurans dalam masa enam puluh (60) hari dari tarikh permulaan polisi/pindaan/sijil pembaharuan. Jika premium tidak dibayar dalam tempoh tersebut, kontrak akan dibatalkan secara automatik dan penanggung insurans adalah berhak menuntut premium secara pro-rata bagi tempoh mereka menanggung risiko. Nota: Senarai ini bukannya senarai yang lengkap. Sila rujuk kepada kontrak polisi untuk senarai penuh bagi terma dan syarat di bawah polisi ini. 7. Apakah pengecualian utama di bawah polisi ini? Polisi ini tidak meliputi:- a) Perang, perang saudara dan apa-apa tindakan keganasan b) Risiko radioaktif dan tenaga nuclear c) Pengenalpastian tarikh d) Kerosakan harta kepada data atau perisian e) Seksyen Liabiliti Terhadap Awam tidak melindungi apa-apa kecederaan yang dianggap berkaitan dengan asbestos atau kerosakan yang melibatkan penggunaan, kehadiran, kewujudan, pengesanan, pengalihan, pembuangan atau penghindaran asbestos atau terdedah atau potensi terdedah kepada asbestos. Nota: Senarai ini bukannya senarai yang lengkap. Sila rujuk kepada kontrak polisi untuk senarai penuh bagi pengecualian di bawah polisi ini. 8. Bolehkah saya membatalkan polisi saya? Anda boleh membatalkan polisi ini dengan memberi notis bertulis kepada pihak kami. Selepas pembatalan, anda layak mendapat pemulangan premium ditolak premium pada kadar tempoh singkat bagi tempoh polisi yang telah berkuatkuasa, tertakluk kepada premium minimum yang ditahan oleh pihak kami. Tiada pemulangan premium dibenarkan jika timbulnya tuntutan di bawah polisi ini. 9. Apakah yang harus saya lakukan jika terdapat perubahan dalam butiran peribadi saya? Adalah penting bagi anda untuk memaklumkan pihak kami tentang sebarang perubahan mengenai maklumat anda bagi memastikan semua surat-menyurat dapat disampaikan kepada anda tepat pada masanya. 2
