1 SOMPO MOTOR (PRIVATE CAR COMPREHENSIVE INSURANCE POLICY) Product Disclosure Sheet (Please read this Product Disclosure Sheet before you decide to take up this insurance. Be sure to also read the general terms and conditions.) 1. What is this product about? This policy provides comprehensive cover only. The coverage of the policy as per the table below: Types Comprehensive Cover Cover Loss / damage to your own vehicle due to accident Loss / damage to your own vehicle due to accidental fire / theft Liabilities to third party for injury, death and property loss / damage Loss / damage to your own vehicle caused by convulsions of nature, where the claim amount not exceeding RM10,000 or 20% of sum insured, whichever is lower. Coverage for all drivers, where no compulsory excess is applicable except where the authorised driver at time of accident is under twenty-one (21) years old, or holds a provisional (P) or learner (L) driver licence. Repairs warranty of twelve (12) months against defects on new parts replaced and workmanship carried out by our Panel of Repairers. Liabilities to driver and passengers of own vehicle (property, bodily injury, death) Optional benefits that you may wish to purchase by paying additional premium: Cover for Windscreens, Windows and Sunroof Legal Liability to Passengers* Separate Cover for Accessories Fixed to Your Car Current Year NCD Relief Compensation for Assessed Repair Time (CART) Waiver of Betterment for vehicle age not more than fifteen (15) years Unlimited Towing Costs Top Up for Full Convulsion of Nature Cover Private Hire Car for vehicles used for e-hailing services *Note: It is an offence under the laws of Singapore to enter the country without extending passenger liability cover to your motor insurance. 2. What is the period of cover and renewal option? The duration of cover is for one (1) year. You need to renew your insurance cover policy annually. 3. How much premium do I have to pay? The total premium that you have to pay may vary depending on the sum insured of the vehicle, cubic capacity, no-claim-discount (NCD) entitlement and other underwriting criteria that we may impose, for example, policyholder age and vehicle age. 4. What are the fees and charges that I have to pay? What you have to pay in addition to the premium Amount Service Tax 6% of premium Stamp Duty RM What is included in the premium Commission paid to the insurance intermediaries (if any) Amount 10% of premium 5. What are some of the key terms and conditions that I should be aware of? a) Duty of Disclosure Pursuant to Schedule 9 of the Financial Services Act 2013, you must take reasonable care to ensure that all your answers to the questions are full, complete, correct and honest and to the best of your knowledge. You also have a duty to inform us of any change in the information given to us earlier before we issue the policy to you, before you renew or change any of the terms of your policy. If you do not, your policy may be cancelled, or treated as if it never existed, or your claim rejected or not fully paid. (SMotor/PDS ) 1

2 b) Change in Risk - You must disclose any changes to the material facts which you know or ought to know which could affect the risk profile from time to time, by giving us seven (7) days written notice and you shall pay such additional premium to us as may be required. c) Cash Before Cover - Full premium must be paid before the effective date of the policy. d) Compulsory Excess - Compulsory Excess of RM400 for Private Car policy, this is the amount of loss you have to bear if your vehicle is driven by a person who is under the age of twenty-one (21) years old or the holder of a provisional (P) or learner (L) driver s license. e) Voluntary Excess - This is the amount of loss you have to bear before we pay for the balance of your vehicle damage claim. Voluntary Excess can be selected by you (range from 0% of the sum insured to 3% of the sum insured), the higher the Voluntary Excess, the lower the premium. f) Contribution - We shall only be liable for our rateable proportion of the loss in the event you have more than one (1) policy to cover your vehicle. Note: This list is non-exhaustive. Please refer to the Policy Wordings for the full list of terms and conditions under this policy. 6. What are the major exclusions under this policy? This policy does not cover certain losses, such as: Your own death or bodily injury due to a motor accident. Your liability against claims from passengers in your vehicle. Theft of non-factory fitted vehicle accessories unless otherwise declared. Consequential loss, depreciation, wear and tear, mechanical or electrical breakdown, failures or breakages. Any loss or damage to your vehicle caused by flood, typhoon, hurricane, storm, tempest, volcanic eruption, earthquake, landslide, landslip, subsidence or sinking of the soil / earth or other convulsions of nature, where the claim amount exceeds RM10,000 or 20% of sum insured, whichever is lower. Note: This list is non-exhaustive. Please refer to the Policy Wordings for the full list of exclusions under this policy. 7. Can I cancel my policy? You may cancel your policy by giving written notice to us. Upon cancellation, any refund of the premium would be based on the conditions stipulated in the Policy Wordings. No refund is allowed if there is a claim under the policy. 8. What do I need to do if there are changes to my contact details? It is important that you inform us if there are any changes to your contact details to ensure that all correspondence reach you in a timely manner. You may inform our branch office or our Customer Service Centre. 9. How do I make a claim? Report to the police within twenty-four (24) hours and immediately us in writing with full details. Repairs must be conducted by the Authorized Panel Workshop selected and approved by us if you make an own damage claim against your own comprehensive policy. If you are not at fault in the accident, you can submit the claim either directly to the insurance company of the party at fault, or if you have a comprehensive policy, you are encouraged to submit the claim to us for speedier claims processing without losing your NCD entitlement. You will lose your entire NCD entitlement once an own damage or a third party claim is made against your policy. 10. Where can I get further information? Pastikan anda juga telah membaca terma dan syarat am.) 1. Apakah produk ini? Polisi ini memberikan perlindungan komprehensif sahaja. Perlindungan polisi adalah seperti jadual di bawah: Jenis Komprehensif Perlindungan Kerugian / kerosakan kepada kenderaan sendiri akibat kemalangan Kerugian / kerosakan kepada kenderaan sendiri akibat kebakaran yang tidak disengajakan atau kecurian Liabiliti kepada pihak ketiga akibat kecederaan, kematian dan kerugian / kerosakan harta benda Kerugian / kerosakan kepada kenderaan sendiri disebabkan oleh apa-apa bentuk konvulsi alam, di mana amaun tuntutan hanya terhad kepada RM10,000 atau 20% dari amaun yang diinsuranskan, yang mana lebih rendah. Perlindungan untuk semua pemandu, dimana tiada kelebihan wajib dikenakan kecuali dimana pemandu yang dibenarkan memandu kenderaan pada masa kemalangan adalah di bawah umur dua puluh satu (21) tahun, atau memegang lesen memandu percubaan (P) atau belajar memandu (L), Waranti Pembaikan tempoh dua belas (12) bulan terhadap kecacatan pada bahagian ganti baru dan mutu kerja yang dilakukan oleh bengkel panel kami Liabiliti kepada pemandu dan penumpang kenderaan sendiri (harta benda, kecederaan tubuh badan, kematian) Manfaat pilihan yang anda ingin beli dengan membayar premium tambahan: Perlindungan untuk Cermin Depan / Belakang, Tingkap dan Bumbung Matahari Liabiliti dari segi Undang-undang terhadap Penumpang* Perlindungan Berasingan bagi Aksesori yang dipasang pada Kereta Anda Pampasan Diskaun Tanpa Tuntutan (NCD) Tahun Semasa Pampasan untuk Tempoh Pembaikan yang Dianggarkan (CART) Pengecualian daripada Kos Penambahbaikan "Betterment" usia kenderaan tidak melebihi lima belas (15) tahun Kos Menunda Tanpa Had Tambahan untuk Perlindungan Penuh untuk Konvulsi Semula Jadi Alam Kereta Sewa Persendirian untuk kenderaan yang digunakan untuk perkhidmatan e-hailing *Nota: Adalah menjadi satu kesalahan di bawah undang-undang Singapura untuk memasuki negara tersebut tanpa perlindungan liabiliti penumpang bagi insurans motor anda. 2. Apakah Tempoh Perlindungan dan Pilihan Pembaharuan? Tempoh perlindungan adalah selama satu (1) tahun. Anda perlu memperbaharui polisi insurans anda setiap tahun. 3. Berapakah premium yang saya perlu bayar? Jumlah premium yang anda perlu bayar adalah berbeza dan bergantung kepada amaun yang diinsuranskan, keupayaan enjin, kelayakan Diskaun Tanpa Tuntutan (NCD) dan kriteria pengunderaitan yang kami kenakan seperti umur pemegang polisi dan usia kenderaan. 4. Apakah yuran dan caj yang saya perlu bayar? Apa yang anda perlu bayar sebagai tambahan kepada premium Jumlah Cukai Perkhidmatan 6% daripada premium Duti Setem RM (SMotor/PDS ) 1

5 Apa yang termasuk dalam premium Komisen yang dibayar kepada pengantara insurans (jika ada) Jumlah 10% daripada premium 5. Apakah terma dan syarat utama yang saya harus ketahui? a) Kewajipan Pendedahan - Menurut Jadual 9 Akta Perkhidmatan Kewangan 2013, anda mempunyai kewajipan untuk mengambil langkah yang munasabah untuk tidak salah nyata dalam menjawab soalan-soalan dalam Borang Cadangan ini. Anda bertanggungjawab untuk memastikan jawapan anda, pada pengetahuan terbaik, adalah penuh, tepat dan jujur. Anda juga mempunyai kewajipan untuk memberitahu kami jika ada perubahan kepada maklumat yang anda berikan kepada kami sebelum kami mengeluarkan polisi anda, sebelum anda memperbaharui atau menukar mana-mana terma polisi anda. Sekiranya tidak, polisi anda mungkin dibatalkan, atau dianggap tidak wujud, atau tuntutan anda ditolak atau dikurangkan. b) Perubahan Risiko - Anda mesti mendedahkan mana-mana perubahan kepada maklumat penting yang anda patut tahu yang mungkin menukar profil risiko dari semasa ke semasa, dengan memberi notis secara bertulis kepada kami dalam masa tujuh (7) hari dan hendaklah membayar premium tambahan sekiranya perlu. c) Pembayaran Sebelum Berkuatkuasa - Pembayaran premium mesti dibuat sebelum polisi ini berkuatkuasa. d) Lebihan Wajib - Lebihan wajib RM400 untuk polisi kereta persendirian, ini adalah amaun kerugian yang anda harus tanggung sendiri jika kenderaan anda dipandu oleh seseorang di bawah umur dua puluh satu (21) tahun atau memegang lesen memandu percubaan (P) atau belajar memandu (L). e) Lebihan Sukarela - Jumlah kerugian yang anda perlu tanggung sebelum kami membayar baki tuntutan kerosakan kenderaan anda. Anda boleh memilih Lebihan Sukarela (lingkungan dari 0% daripada amaun yang diinsuranskan sehingga ke 3% daripada amaun yang diinsuranskan), di mana semakin tinggi Lebihan Sukarela, semakin rendah premium. f) Sumbangan - Kami hanya akan bertanggungjawab ke atas kadar kerugian kami yang berpatutan apabila anda mempunyai lebih daripada satu (1) polisi untuk melindungi kereta anda. Nota: Senarai ini bukannya senarai yang lengkap. Sila rujuk kepada Polisi untuk senarai penuh bagi terma dan syarat. 6. Apakah pengecualian utama di bawah polisi ini? Polisi ini tidak melindungi kerugian tertentu, seperti: - Kematian atau kecederaan diri anda sendiri akibat kemalangan motor. Liabiliti anda terhadap tuntutan daripada penumpang di dalam kenderaan anda. Kecurian aksesori kenderaan yang bukan dipasang di kilang, melainkan sekiranya diisytiharkan. Segala kerugian turutan, susut nilai, haus dan lusuh, kegagalan / kerosakan mekanikal atau elektrik. Sebarang kehilangan atau kerosakan kepada kenderaan anda yang disebabkan oleh banjir, taufan, ribut, letusan gunung berapi, gempa bumi, tanah runtuh, gelinciran tanah, penenggelaman atau tenggelam tanah / bumi atau konvulsi alam semula jadi, di mana jumlah tuntutan melebihi RM10,000 atau 20% dari amaun yang diinsuranskan, yang mana lebih rendah. Nota: Senarai ini bukannya senarai yang lengkap. Sila rujuk kepada Polisi untuk senarai penuh bagi pengecualian. 7. Bolehkah saya membatalkan polisi saya? Anda boleh membatalkan polisi pada bila-bila masa dengan memberi notis bertulis dan Sijil Insurans asal anda kepada kami. Selepas pembatalan, bayaran balik premium adalah tertakluk kepada syarat yang telah ditetapkan dalam Polisi. Tiada pengembalian premium dibenarkan sekiranya terdapat tuntutan bawah polisi ini. 8. Apakah yang harus saya lakukan jika terdapat perubahan dalam butiran peribadi saya? Adalah penting bagi anda untuk memaklumkan pihak kami tentang sebarang perubahan mengenai maklumat peribadi anda bagi memastikan semua surat-menyurat dapat disampaikan kepada anda tepat pada masanya. 2

6 9. Bagaimanakah caranya untuk saya membuat tuntutan? Buat laporan polis dalam masa dua puluh (24) jam dan maklumkan kami secara bertulis dengan memberi butir-butir lengkap dengan segera. Pembaikan hendaklah dikendalikan oleh Bengkel Panel Disahkan yang telah dipilih dan kami luluskan sekiranya anda membuat tuntutan kerosakan sendiri terhadap polisi komprehensif anda sendiri. Sekiranya anda bukan pihak yang bersalah dalam kemalangan tersebut, anda boleh mengemukakan tuntutan sama ada secara terus kepada syarikat insurans pihak yang bersalah atau, sekiranya anda mempunyai polisi komprehensif, anda digalakkan mengemukakan tuntutan anda kepada kami untuk mempercepatkan pemprosesan tuntutan tanpa kehilangan kelayakan NCD anda. Anda akan kehilangan keseluruhan kelayakan NCD anda apabila tuntutan kerosakan sendiri atau pihak ketiga dibuat terhadap polisi anda. 10. Di manakah saya boleh dapatkan maklumat lanjut?
