1 EQUIPMENT INSURANCE Product Disclosure Sheet (Please read this Product Disclosure Sheet before you decide to take out this insurance. Be sure to also read the general terms and conditions.) 1. What is this product about? This policy indemnifies you against fire, theft and accidental damage to your mobile equipment. 2. What are the covers / benefits provided? a) This policy covers loss or damage to your mobile equipment (such as forklift, excavator etc.) and its standard accessories and parts whilst thereon by:- i) Accidental collision or overturning or collision or overturning consequent upon mechanical breakdown or consequent upon wear and tear; ii) Fire external explosion self-ignition or lightning; iii) Burglary housebreaking or theft. b) In addition, if the equipment is disabled by reason of loss or damage insured under this policy, the Company will bear the cost of not more than RM reasonably incurred for its protection and removal to the nearest repairers and for its delivery within the country where the loss or damage was sustained, subject to the sum insured stated in the policy schedule. 3. What is the Period of Cover and Renewal Option? Duration of cover is usually for one year. You need to renew your insurance policy annually. 4. How much premium do I have to pay? The premium you have to pay may vary depending on the sum insured, occupation or nature of business, building construction, insured property, claims experience etc. Sum Insured : RM Rate applicable : % The estimated total premium : RM 5. What are the fees and charges I have to pay? What you have to pay in addition to the premium Amount Goods and Services Tax (GST) 6% of premium Stamp Duty RM What is included in the premium Commission paid to the insurance intermediaries (if any) Amount 25% of premium 6. What are some of the key terms and conditions that I should be aware of? a) DUTY OF DISCLOSURE Pursuant to Paragraph 4(1) of Schedule 9 of the Financial Services Act 2013, if you are applying for this Insurance for a purpose related to your trade, business or profession, you have a duty to disclose any matter that you know to be relevant to our decision in accepting the risks and determining the rates and terms to be applied and any matter a reasonable person in the circumstances could be expected to know to be relevant, otherwise it may result in avoidance of your contract of insurance, refusal or reduction of your claim(s), change of terms or termination of your contract of insurance. The above duty of disclosure shall continue until the time your contract of insurance is entered into, varied or renewed with us. You also have a duty to tell us immediately if at any time after your contract of insurance has been entered into, varied or renewed with us any of the information given in the Proposal Form (or when you applied for this insurance) is inaccurate or has changed. b) Premium Warranty - Premium must be paid within 60 days from the inception date of the cover, otherwise the cover will automatically be cancelled. Please take note that you are still responsible to pay the proportion of the premium for the 60 days. c) Sum Insured - Please ensure that your property is adequately insured. (Equipment/PDS ) 1

2 d) Claims - Upon the happening of an accident which gives rise to a claim, you shall notify us immediately. e) GOODS AND SERVICES TAX (GST) CLAUSE - Please ensure that you have taken consideration of this GST when you decide on the amount to be insured in your policy: GST CLAUSE The amount of premium payable by You for this Policy includes an amount on account of the GST on the premium. When We pay a claim, Your GST status will determine the amount We pay. When You are: (a) Non-GST registered person, We will pay in full (including 6% GST) up to sum insured/limit of liability or the other limits of insurance cover. (b) GST registered person, We will pay (excluding 6% GST) up to sum insured/limit of liability or the other limits of insurance cover. You are to claim Your Input Tax Credit entitlement from the Royal Malaysian Customs Department directly. You must advise Us of Your correct entitlement to an Input Tax Credit on Your Premium and the correct entitlement to an Input Tax Credit on each item of the property to be insured. Definitions under the Section on Glossary:- GST means goods and services tax and has the meaning assigned to it in the Goods and Services Tax Act 2014 ( GST Act ). Registered person means a person who is registered under Part IV of the GST Act and a non registered person shall mean a person who is not registered under the GST Act. Input Tax means the GST incurred on any purchase or acquisition of goods and services by a taxable person for the purpose of making a taxable supply in the course or furtherance of business. Input Tax Credit means the input tax claimable by a registered person Note: This list is non-exhaustive. Please refer to the policy contract for the full list of terms and conditions under this policy. 7. What are the major exclusions under this policy? This policy does not cover:- a) Loss/damage arising from war, terrorism and nuclear risks. b) Equipment licensed for road use and for which a Certificate of Motor Insurance is required. c) Equipment operating on board any waterborne vessel unless approved by us. d) Loss/damage due to overloading or strain. e) Loss of accessories and parts unless the equipment is stolen at the same time. f) Loss/damage due to the willful or dishonest act of the Insured s employees or any person to whom the equipment is entrusted. g) Loss/damage caused by atmospheric conditions, depreciation, mechanical or electrical breakdown/failures/breakages or wear and tear. h) Loss/damage caused by the operator who is under the influence of alcohol or drugs. i) Damage to tyres unless the equipment is damaged at the same time. Note: This list is non-exhaustive. Please refer to the policy contract for the full list of exclusions under this policy. 8. Can I cancel my policy? You may cancel your policy at any time by giving written notice to us. Upon cancellation, any refund premium would be based on the conditions as stated in the policy contract. 9. What do I need to do if there are changes to my contact details? It is important that you inform us if there are changes in your contact details to ensure that all future correspondences reach you in a timely manner. 2

Pastikan anda juga telah membaca terma dan syarat am.) 1. Apakah produk ini? Polisi ini mengganti rugi terhadap kebakaran, kecurian dan kerosakan kepada peralatan mudah alih anda. 2. Apakah perlindungan / faedah yang disediakan? a) Polisi ini melindungi kerugian atau kerosakan terhadap peralatan mudah alih anda (seperti forklift, penggali dan lainlain), bahagian aksesori dan alat gantinya akibat oleh: - i) perlanggaran atau terbalik akibat kemalangan atau perlanggaran atau terbalik berbangkit kerosakan mekanikal atau akibat haus dan lusuh; ii) Kebakaran luaran letupan diri pencucuhan atau kilat ; iii) Pecah rumah atau kecurian. b) Selain itu, jika peralatan yang dilumpuhkan oleh sebab kehilangan atau kerugian yang diinsuranskan di bawah polisi ini, Syarikat akan menanggung kos yang tidak melebihi RM semunasabahnya dilakukan bagi perlindungan dan pemindahannya kepada pembaik yang terdekat dan untuk penyerahan di dalam negara ini di mana kerugian atau kerosakan yang berterusan, tertakluk kepada jumlah yang diinsuranskan yang dinyatakan dalam jadual polisi. 3. Apakah Tempoh Perlindungan dan Pembaharuan Pilihan? Tempoh perlindungan biasanya selama satu tahun. Anda perlu memperbaharui polisi insurans anda setiap tahun. 4. Berapakah premium yang saya perlu bayar? Premium yang anda perlu bayar mungkin berbeza bergantung kepada jumlah yang diinsuranskan, pekerjaan atau jenis perniagaan, pembinaan bangunan, harta yang diinsuranskan, pengalaman tuntutan dan lain-lain. Jumlah Diinsuranskan : RM Kadar yang dikenakan : % Anggaran jumlah premium : RM 5. Apakah yuran dan caj yang saya perlu bayar? Apa yang anda perlu bayar sebagai tambahan kepada premium Jumlah Cukai Barangan dan Perkhidmatan (CBP) 6% daripada premium Duti Setem RM Apa yang termasuk dalam premium Komisen yang dibayar kepada pengantara insurans (jika ada) Jumlah Sehingga 25% daripada premium 6. Apakah terma dan syarat utama yang saya harus ketahui? a) KEWAJIPAN PENDEDAHAN Menurut Perenggan 4(1) Jadual 9 Akta Perkhidmatan Kewangan 2013, jika anda memohon insurans ini untuk tujuan yang berkaitan dengan perdagangan, perniagaan atau profesion anda, anda berkewajipan untuk mendedahkan apa- 3 apa perkara yang anda tahu akan mempengaruhi keputusan kami dalam menerima risiko dan menentukan kadar dan terma yang dikenakan dan apa-apa perkara yang munasabah yang boleh dijangka, jika tidak ia boleh menyebabkan pembatalan kontrak insurans, keengganan atau pengurangan ganti rugi, perubahan terma atau penamatan kontrak insurans anda. Kewajipan pendedahan diatas hendaklah diteruskan sehingga kontrak insurans anda dimeterai, diubah atau diperbaharui dengan kami. Anda juga mempunyai kewajipan untuk memberitahu kami dengan serta-merta jika pada bila-bila masa selepas kontrak insurans anda ditangatangani, diubah atau diperbaharui dengan kami, apa-apa maklumat yang dinyatakan dalam Borang Cadangan ini tidak tepat atau telah berubah. 5 b) Waranti Premium Premium mesti dibayar dalam masa 60 hari dari tarikh bermulanya perlindungan, jika tidak perlindungan secara automatik akan dibatalkan. Sila ambil perhatian bahawa anda masih bertanggungjawab untuk membayar perkadaran premium bagi 60 hari. c) Jumlah Diinsuranskan Anda harus memastikan harta anda diinsuranskan pada amaun yang berpatutan. d) Tuntutan Apabila berlakunya kemalangan yang menimbulkan suatu tuntutan, anda hendaklah memberitahu kami dengan segera. e) Fasal Cukai Barangan Dan Perkhidmatan (CBP) Sila pastikan bahawa anda telah mengambil pertimbangan CBP ini apabila anda membuat keputusan mengenai jumlah yang diinsuranskan dalam polisi anda: FASAL CBP Jumlah premium yang perlu dibayar oleh Anda bagi Polisi ini termasuk jumlah CBP yang dikenakan pada premium. Status CBP Anda akan menentukan jumlah Kami bayar keatas tuntutan. Apabila Anda adalah: (a) orang tidak berdaftar dibawah CBP, Kami akan membayar sepenuhnya (termasuk 6% CBP) sehingga kepada jumlah diinsuranskan / had liabiliti atau had lain perlindungan insurans. (b) orang berdaftar dibawah CBP, Kami akan membayar (tidak termasuk 6% CBP) sehingga jumlah diinsuranskan / had liabiliti atau had lain perlindungan insurans. Anda hendaklah menuntut secara langsung Kredit Cukai Input yang layak daripada Jabatan Kastam Diraja Malaysia. Anda hendaklah memaklumkan kepada Kami kelayakan Kredit Cukai Input Anda yang betul atas Premium Anda dan kelayakan Kredit Cukai Input yang betul pada setiap item harta yang diinsurans. GLOSARI:- CBP" bermaksud cukai barangan dan perkhidmatan dan mempunyai erti yang diberikan kepadanya dalam Akta Cukai Barangan dan Perkhidmatan 2014 ("Akta CBP"). "Orang berdaftar" bermaksud seseorang yang didaftarkan di bawah Bahagian IV Akta CBP dan orang tidak berdaftar bermaksud seseorang yang tidak didaftarkan di bawah Akta CBP. "Cukai Input" bermaksud CBP yang ditanggung ke atas sebarang pembelian atau perolehan barangan dan perkhidmatan oleh orang yang dikenakan cukai bagi maksud membuat suatu pembekalan bercukai bagi meneruskan perniagaan. "Kredit Cukai Input" bermaksud cukai input yang boleh dituntut oleh orang berdaftar. Nota: Senarai ini bukannya senarai yang lengkap. Sila rujuk kepada kontrak polisi untuk senarai penuh bagi terma dan syarat di bawah polisi ini. 7. Apakah pengecualian utama di bawah polisi ini? Polisi ini tidak melindungi: a) Kerugian / kerosakan yang timbul daripada peperangan, keganasan dan risiko nuklear. b) Peralatan yang dilesenkan untuk kegunaan di jalan raya dan yang mana Sijil Insurans Motor diperlukan. c) Peralatan yang beroperasi di mana-mana vesel bawaan air melainkan jika diluluskan oleh kami. d) Kerugian / kerosakan disebabkan oleh muatan yang berlebihan atau terlalu berat. e) Kerugian aksesori dan alat kecuali peralatan yang dicuri pada masa yang sama. f) Kerugian / kerosakan yang disebabkan oleh tindakan sengaja atau tidak jujur pekerja Pihak Diinsuranskan atau manamana orang yang kepadanya peralatan yang diamanahkan. g) Kerugian / kerosakan yang disebabkan oleh keadaan atmosfera, susut nilai, kerosakan / kegagalan / pecahnya mekanikal atau elektrik atau haus dan lusuh. h) Kerugian / kerosakan yang disebabkan oleh pengendali yang berada di bawah pengaruh alkohol atau dadah. i) Kerosakan kepada tayar kecuali peralatan yang rosak pada masa yang sama. Nota: Senarai ini bukannya senarai yang lengkap. Sila rujuk kepada kontrak polisi untuk senarai penuh bagi pengecualian di bawah polisi ini. 2

6 8. Bolehkah saya membatalkan polisi saya? Anda boleh membatalkan polisi anda pada bila-bila masa dengan memberi notis bertulis kepada kami. Selepas pembatalan, sebarang bayaran balik premium akan berdasarkan pada syarat-syarat seperti yang dinyatakan dalam kontrak polisi. 9. Apakah yang harus saya lakukan jika terdapat perubahan dalam butiran peribadi saya? Adalah penting bagi anda untuk memaklumkan pihak kami tentang sebarang perubahan mengenai maklumat anda bagi memastikan semua surat-menyurat dapat disampaikan kepada anda tepat pada masanya. 10.
