Be sure to also read the general terms and conditions.) 1. What is this product about? This policy is designed for Businesses having a total business asset not exceeding RM 10 million. a. Section 1 - Fire and Defined Perils b. Section 2 (Special Pack) - Public Liability - Plate Glass - Money - Fidelity Guarantee - Employer s Liability c. Section 3 (Option Pack) - Burglary - Equipment - All Risks - Workmen s Compensation 2. What are the covers / benefits provided? You may select to insure any of the following: a. Section 1 - Loss or damages to your property insured caused by fire, lighting and/or additional cover for specials perils as selected by paying additional premium b. Section 2 (Special Pack) - Loss or damage to third parties property or bodily injury to third parties due to your negligence - Accidental breakage of glass fitted to your premises - Loss of money during transit or loss of money secured in your premises - Loss due to the infidelity or dishonesty committed by your employees - Your liability to your employees for injury or illness sustained in the course of employment due to your negligence c. Section 3 (Option Pack) - Loss by theft consequent upon forcible entry into your premises including Armed Robbery and Hold-up - Loss or damage to your mobile plant, machinery and equipment used within your premises. - Loss or damage to your office and business equipment (including non-mobile plant & machinery) within the Territorial Limit specified in the policy schedule - Accidental death or permanent disablement and/or medical expenses to your employees sustained in the course of employment 3. What is the Period of Cover and Renewal Option? Duration of cover is usually for one year. You need to renew your insurance policy annually. 4. How much premium do I have to pay? The total premium you have to pay may vary depending on the underwriting requirements of the insurance company. 5. What are the fees and charges I have to pay? What you have to pay in addition to the premium Service Tax Stamp Duty Amount 6% of premium RM What is included in the premium Amount Commission paid to the insurance intermediaries (if any) up to 15% of premium (Section 1) up to 25% of premium (Other Sections) (BIP/PDS ) 1

What are some of the key terms and conditions that I should be aware of? a) DUTY OF DISCLOSURE Non Consumer Insurance Contract Pursuant to Paragraph 4(1) of Schedule 9 of the Financial Services Act 2013, if you are applying for this Insurance for purposes related to your trade, business or profession, you have a duty to disclose any matter that you know to be relevant to our decision in accepting the risks and determining the rates and terms to be applied and any matter a reasonable person in the circumstances could be expected to know to be relevant, otherwise it may result in avoidance of your contract of insurance, refusal or reduction of your claim(s), change of terms or termination of your contract of insurance. This duty of disclosure for Non-Consumer Insurance Contract shall continue until the time the contract is entered into, varied or renewed. You also have a duty to tell us immediately if at any time after your contract of insurance has been entered into, varied or renewed with us, any of the information given in the Proposal Form is inaccurate or has changed. b) You must disclose all material facts which you know or ought to know; otherwise your policy may be invalidated. You must ensure that the proposal form is completed accurately as it forms the basis of the insurance contract. c) You must ensure that your property is insured at the appropriate amount taking into account the renovations made to your property. d) If the amount insured in your policy is less than the actual value at the time of loss (i.e. under-insurance and you are deemed to be self-insuring the difference), the condition of average will apply in the event of a claim. e) You must inform your Insurance intermediary or us in writing on any material changes during the policy period so that the necessary amendments are endorsed to your policy. f) Payment can be made by Cash, Credit Card or GIRO to Berjaya Sompo Insurance Berhad. Insist on a receipt for the premium paid as proof of payment for future reference. g) Depending on the circumstances of the loss or claims made during the term of policy, we may review the policy terms and conditions upon renewal or decline renewal. Note: This list is non-exhaustive. Please refer to the policy contract for the full list of terms and conditions under this policy. 7. What are the major exclusions under this policy? This policy does not cover losses which are specifically excluded under the respective policies. Note: This list is non-exhaustive. Please refer to the policy contract for the full list of exclusions under this policy. 8. Can I cancel my policy? You may cancel your policy at any time by giving written notice to us. Upon cancellation, you are entitled to a refund of the premium less premium based on short period rates for the policy which has been in force, subject to the minimum premium to be retained by us. No refund premium is allowed if there is a claim under the policy. 9. What do I need to do if there are changes to my contact details? It is important that you inform us if there are changes in your contact details to ensure that all future correspondences reach you in a timely manner. 2

Anda boleh memilih untuk menginsuranskan satu daripada yang berikut: a. Seksyen 3 (Pek Opsyen) - Kerugian akibat kecurian pecah masuk secara paksa ke dalam premis anda termasuk Rompakan Bersenjata dan Keganasan - Kehilangan atau kerosakan kepada loji mudah alih, jentera dan peralatan yang digunakan di dalam premis anda. - Kerugian atau kerosakan kepada pejabat dan kelengkapan perniagaan (termasuk loji bukan mudah alih & jentera) dalam Had Wilayah seperti yang dinyatakan dalam jadual polisi - Kematian akibat kemalangan atau hilang upaya kekal dan/atau perbelanjaan perubatan kepada pekerja anda yang dialami semasa penggajian pekerja 3. Apakah Tempoh Perlindungan dan Pembaharuan Pilihan? Tempoh perlindungan biasanya selama satu tahun. Anda perlu memperbaharui polisi insurans anda setiap tahun. 4. Berapakah premium yang saya perlu bayar? Jumlah premium yang anda perlu bayar mungkin berbeza bergantung kepada syarat pengunderaitan syarikat insurans. (BIP/PDS ) 1

5 5. Apakah yuran dan caj yang saya perlu bayar? Apa yang anda perlu bayar sebagai tambahan kepada premium Cukai Perkhidmatan (CP) Duti Setem Apa yang termasuk dalam premium Komisen yang dibayar kepada pengantara insurans (jika ada) Jumlah 6% daripada premium RM Jumlah Sehingga 15% daripada premium (Seksyen 1) Sehingga 25% daripada premium (Seksyen Lain) 6. Apakah terma dan syarat utama yang saya harus ketahui? a) KEWAJIPAN PENDEDAHAN Kontrak Insurans Komersial Menurut Perenggan 4(1) Jadual 9 Akta Perkhidmatan Kewangan 2013, jika anda memohon insurans ini untuk tujuan yang berkaitan dengan perdagangan, perniagaan atau profesion anda, anda berkewajipan untuk mendedahkan apa-apa perkara yang anda tahu akan mempengaruhi keputusan kami dalam menerima risiko dan menentukan kadar dan terma yang dikenakan, dan apa-apa perkara yang munasabah yang boleh dijangka, jika tidak ia boleh menyebabkan pembatalan kontrak insurans, keengganan atau pengurangan ganti rugi, perubahan terma atau penamatan kontrak insurans anda. Kewajipan pendedahan untuk Kontrak Insurans Komersial hendaklah diteruskan sehingga kontrak insurans anda dimeterai, diubah atau diperbaharui dengan kami. Anda juga mempunyai kewajipan untuk memberitahu kami dengan serta-merta jika pada bila-bila masa selepas kontrak insurans anda ditandatangani, diubah atau diperbaharui dengan kami, apa-apa maklumat yang dinyatakan dalam Borang Cadangan ini tidak tepat atau telah berubah. b) Anda mesti mendedahkan semua fakta material yang anda tahu atau patut tahu; jika tidak, polisi anda mungkin tidak sah. Anda mesti pastikan borang cadangan dilengkapkan dengan tepat kerana ia merupakan asas kepada kontrak insurans. c) Anda harus memastikan harta anda diinsuranskan pada amaun yang berpatutan dengan memasukkan pengubahsuaian yang dilakukan pada harta. d) Sekiranya jumlah diinsuranskan dalam polisi anda adalah kurang daripada nilai sebenar pada waktu kerugian berlaku (contohnya insurans terkurang dan anda dianggap menanggung sendiri terhadap perbezaannya), syarat purata akan dikenakan jika berlakunya tuntutan. e) Anda perlu memaklumkan kepada pengantara insurans anda atau kami secara bertulis tentang sebarang perubahan material sepanjang tempoh polisi supaya pindaan-pindaan yang wajar diendorskan pada polisi anda. f) Bayaran boleh dibuat melalui Tunai, Kad Kredit atau GIRO kepada Berjaya Sompo Insurance Berhad. Sentiasa meminta resit untuk premium yang dibayar sebagai bukti pembayaran untuk rujukan masa depan. g) Bergantung kepada keadaan kerugian atau tuntutan dibuat semasa tempoh polisi, kami boleh mengkaji semula terma-terma dan syarat-syarat polisi pada masa pembaharuan atau menolak pembaharuan. Nota: Senarai ini bukannya senarai yang lengkap. Sila rujuk kepada kontrak polisi untuk senarai penuh bagi terma dan syarat di bawah polisi ini. 7. Apakah pengecualian utama di bawah polisi ini? Polisi ini tidak melindungi kerugian yang dikecualikan secara spesifik di bawah polisi masing-masing. Nota: Senarai ini bukannya senarai yang lengkap. Sila rujuk kepada kontrak polisi untuk senarai penuh bagi pengecualian di bawah polisi ini. 8. Bolehkah saya membatalkan polisi saya? Anda boleh membatalkan polisi anda pada bila-bila masa dengan memberi notis bertulis kepada kami. Selepas pembatalan, anda layak mendapat pemulangan premium ditolak premium berdasarkan kadar tempoh pendek untuk polisi yang telah berkuatkuasa, tertakluk kepada premium minimum yang ditahan oleh pihak kami. Tiada pemulangan premium dibenarkan jika terdapat tuntutan di bawah polisi ini. 2

Adalah penting bagi anda untuk memaklumkan pihak kami tentang sebarang perubahan mengenai maklumat anda bagi memastikan semua surat-menyurat dapat disampaikan kepada anda tepat pada masanya. 10.
