1 PRODUCT DISCLOSURE SHEET (Read this Product Disclosure Sheet before you decide to participate in the IKHLAS Fidelity Guarantee Takaful. Be sure to also read the general terms and conditions.) 1) What is this product about? TAKAFUL IKHLAS BERHAD IKHLAS FIDELITY GUARANTEE TAKAFUL Date: 07/05/2014 This Takaful Certificate indemnifies you as an employer against any direct loss of pecuniary that you may sustain through act(s) of dishonesty by your employees such as act(s) of forgery, embezzlement, larceny or fraudulent conversion. The coverage is not limited to loss or money but also covers loss of real property and stock-in-trade belonging to you. 2) What are the Shariah concepts applicable? Tabarru shall mean donation for the purpose of solidarity and cooperation among the Participants and to be used to help Participants in times of misfortune. In the context of Takaful IKHLAS, Tabarru will be allocated into the Risk Fund. Wakalah shall mean agency. In this context, it refers to a contract between Company and the Participant where the Participant authorizes the Company to manage the fund on his/her behalf, which allows the Company to charge a Wakalah fee. Note: Please refer to the Takaful Certificate contract for more information. 3) What are the covers / benefits provided? This Takaful Certificate covers your pecuniary loss or goods belonging to you due to the act of fraud or dishonesty committed by your employees: during the Period of Takaful; during the uninterrupted continuance of employment of such employee; in connection with the occupation and duties of such employees; and discovered during the aforesaid Period of Takaful or within 6 months thereafter or within 6 month after the death, dismissal or retirement of such employee whichever shall happen first. Duration of cover is for one year. You need to renew your Takaful Certificate annually. 4) How much contribution do I have to pay? The total Contribution that you have to pay may vary depending on the guarantee amount and our underwriting requirements : Guarantee Amount : RM Rate applicable : % No. of Employees Covered : Rate per Capita : RM The estimated total Contribution that you have to pay is : RM 1

2 5) What are the fees and charges that I have to pay? The following charges are applicable: What you have to pay in addition to the Contribution amount :- Service tax (if any) : 6% of Contribution Stamp duty : RM10.00 What is included in the contribution amount :- Wakalah Fees - Commissions : up to max. 25% of Contribution Management Expenses : 15% of Contribution What you have to pay if you cancel the Certificate :- Cancellation Fee : RM10.00 Note: Our charges exclude tax that would be imposed by the government and/or the authority in the future (including Goods and Services Tax (GST)). Upon implementation of the GST, we shall be entitled to recover the GST from you in respect of this Takaful Certificate that is required under the law. 6) What are some of the key terms and conditions that I should be aware of? Importance of Disclosure You must disclose all material facts that you know or ought to know; otherwise your Takaful Certificate may be invalidated; If this Takaful Certificate be continued in force for more than one period of Takaful, our liability in respect of any One Claim shall not be accumulated or increased thereby and our aggregate liability during any number of Period of Takaful and for any number of losses forming the basis of any One Claim whether under this Takaful Certificate or any similar Takaful Certificate in substitution for or substituted by this Takaful Certificate shall not exceed the amount of guarantee under the current Takaful Certificate. Surplus The Company shall charge a Surplus Administration Charge (SAC) of 50% of the gross distributable surplus arising at the end of the financial year. However, the Company may at its discretion and where appropriate charge SAC less than 50% of the gross distributable surplus. Any net distributable surplus arising (after deducting the SAC) from the Risk Fund will be allocated in full (100%) to the Participant. If the amount due to Participant is less than RM10 per Certificate, the amount shall be retained in the Risk Fund. 7) What are the major exclusions under the certificate? This Takaful Certificate does not cover : any indirect or consequential loss; any third party losses; extortion; any loss sustained prior to Takaful Certificate inception or discovered prior to Takaful Certificate inception or discovered subsequent to the termination of the Takaful Certificate period including the discovery period; stock taking or inventory losses; losses rising from ionizing radiations or contamination by radioactivity from any nuclear fuel, nuclear waste or any nuclear weapons material; losses caused by any act of terrorism. 2

You may cancel your Takaful Certificate by giving written notice to us. Upon cancellation, you are entitled to a refund of the pro rata Contribution for the Period of the Takaful Certificate which has been in force. No refund of Contribution is allowed if there is a claim under the Takaful Certificate. 9) What do I need to do if there are changes to my contact details? It is important that you inform us of any change in your contact details to ensure that all correspondences reach you in a timely manner. 10) Where can I get further information? Pastikan juga anda telah membaca terma-terma dan syarat-syarat.) TAKAFUL IKHLAS BERHAD IKHLAS FIDELITY GUARANTEE TAKAFUL Tarikh: 07/05/2014 1) Apakah produk ini? Sijil Takaful ini menanggung rugi anda sebagai seorang majikan daripada mana-mana kerugian kewangan yang langsung akibat kelakuan pekerja-pekerja yang pecah amanah, seperti kejadian pemalsuan, penggelapan wang, larseni atau penukaran fraud. Perlindungan ini tidak terhad kepada kerugian wang tetapi juga melindungi kerugian harta sebenar dan stok perniagaan anda. 2) Apakah konsep Syariah yang digunapakai? Tabarru - bermaksud derma bagi tujuan perpaduan dan kerjasama di kalangan Peserta-peserta dan akan digunakan untuk membantu Peserta yang ditimpa musibah. Di dalam konteks Takaful IKHLAS, Tabarru akan diperuntukan ke dalam Dana Risiko. Wakalah - bermaksud agensi. Di dalam konteks ini, ia merujuk kepada kontrak di antara Syarikat dan Peserta di mana Peserta melantik Syarikat untuk menguruskan dana bagi pihak Peserta, yang membenarkan Syarikat untuk mengenakan yuran Wakalah. Nota: Sila rujuk kepada Sijil kontrak Takaful untuk maklumat lanjut. 3) Apakah perlindungan/manfaat yang ditawarkan? Sijil Takaful ini melindungi kerugian kewangan atau barangan kepunyaan anda yang disebabkan oleh perbuatan penipuan atau ketidakjujuran yang dilakukan oleh pekerja-pekerja anda: Sepanjang Tempoh Takaful; Sepanjang penggajian yang berterusan tanpa berhenti bagi pekerja tersebut; Berkaitan dengan pekerjaan dan tugasan bagi pekerja tersebut; dan Diketahui sepanjang tempoh Takaful seperti dinyatakan atau dalam masa 6 bulan selepas itu atau dalam masa 6 bulan selepas kematian, pembuangan atau persaraan bagi pekerja tersebut, bergantung kepada yang mana berlaku terlebih dahulu. Tempoh perlindungannya ialah selama satu tahun. Anda dikehendaki membaharui Sijil Takaful anda setiap tahun. 4) Berapakah jumlah sumbangan yang perlu dibayar? Jumlah Sumbangan yang anda perlu bayar adalah berbeza bergantung kepada amaun jaminan dan keperluan pengunderaitan pihak kami. Amaun Jaminan : RM Kadar dikenakan : % Bilangan Pekerja yang Dilindungi Anggaran jumlah Sumbangan yang anda perlu bayar ialah : RM 4

5 5) Apakah yuran dan caj yang perlu dibayar? Berikut adalah yuran dan bayaran yang dikenakan: Bayaran yang perlu dijelaskan disamping jumlah Sumbangan Cukai Perkhidmatan (jika ada) : 6% dari Sumbangan Duti Setem : RM10.00 Bayaran lain yang termasuk dalam jumlah sumbangan Fi Wakalah - Komisen : sehingga maksima 25% dari Sumbangan Perbelanjaan Pengurusan : 15% dari Sumbangan Bayaran yang perlu dijelaskan jika anda membatalkan Sijil ini :- Caj Pembatalan : RM10.00 Nota: Caj kami tidak termasuk cukai yang dikenakan oleh kerajaan dan/atau pihak berkuasa pada masa akan datang (termasuk Cukai Barangan dan Perkhidmatan (GST)). Setelah perlaksanaan GST, kami berhak mengenakan caj GST bagi maksud Sijil Takaful ini yang dikehendaki dibayar di bawah undang-undang. 6) Apakah terma dan syarat utama yang perlu diketahui? Kepentingan Pendedahan Anda dikehendaki mendedahkan semua fakta material yang anda tahu atau patut tahu; jika tidak, Sijil Takaful ini akan dianggap tidak sah; Jika Sijil Takaful ini terus berkuatkuasa melebihi satu tempoh insurans, liabiliti pihak kami berhubung dengan mana-mana satu tuntutan tidak akan terkumpul atau bertambah dengan cara itu, dan liabiliti agregat pihak kami sepanjang mana-mana bilangan tempoh takafuldan untuk mana-mana bilangan kerugian yang membentuk dasar kepada mana-mana satu tuntutan tidak kira di bawah Sijil Takaful ini atau mana-mana Sijil Takaful yang serupa sebagai penggantian untuk atau digantikan oleh Sijil Takaful ini, tidak boleh melebihi amaun jaminan di bawah Sijil Takaful yang sedia ada; Lebihan Pihak Syarikat akan mengenakan Caj Pengurusan Lebihan (SAC) sebanyak 50% daripada lebihan boleh-agih kasar dari Dana Risiko pada setiap akhir tahun kewangan. Walau bagaimanapun, pihak Syarikat boleh mengenakan caj SAC kurang daripada 50% daripada lebihan boleh-agih kasar mengikut budi bicara dan kesesuaian. Sebarang lebihan boleh-agih bersih (selepas ditolak SAC) daripada Dana Risiko akan diperuntukkan seluruhnya (100%) kepada Peserta. Sekiranya jumlah untuk Peserta kurang daripada RM10 untuk setiap Sijil, jumlah tersebut akan terus disimpan di dalam Dana Risiko. 7) Apakah pengecualian utama yang terdapat di dalam sijil ini? Sijil Takaful ini tidak melindungi kerugian, kemusnahan atau kerosakan: Sebarang kerugian tidak langsung atau kerugian turutan; Sebarang kerugian pihak ketiga; Pemerasan; Sebarang kerugian yang dialami sebelum tempoh takafulatau diketahui sebelum Tempoh Takaful atau diketahui pada masa perberhentian sijil takaful termasuk tempoh ketahui; Pengambilan stok atau kehilangan inventori; Kerugian yang terbit daripada pengionan radiasi atau kontaminasi oleh radioaktif daripada sebarang tenaga nuklear, bahan buangan nuklear atau sebarang senjaya nuklear; 5

6 Kerugian akibat sebarang tindakan keganasan. Nota: Senarai ini bukannya senarai yang lengkap. Sila rujuk Sijil kontrak Takaful untuk senarai penuh bagi pengecualian di bawah Sijil Takaful ini. 8) Bolehkah saya membatalkan sijil Takaful ini? Anda boleh membatalkan Sijil Takaful ini dengan memberi notis bertulis kepada pihak kami. Selepas pembatalan, anda layak mendapat pulangan Sumbangan untuk kadar pro rata bagi tempoh Sijil Takaful yang telah berkuatkuasa. Tiada pulangan Sumbangan dibenarkan jika ada sebarang tuntutan di bawah Sijil Takaful ini. 9) Apakah yang perlu dilakukan sekiranya terdapat perubahan kepada maklumat untuk dihubungi? Adalah penting bagi anda untuk memaklumkan kepada pihak kami tentang sebarang perubahan mengenai maklumat anda bagi memastikan semua surat-menyurat dapat disampaikan kepada anda tepat pada masanya. 10) Bagaimanakah saya boleh mendapatkan maklumat lanjut?
