1 HELAIAN PENERANGAN PRODUK Sila baca Helaian Keterangan Produk ini sebelum membuat keputusan untuk mengambil Skim Takaful Kemasukan Hospital dan Pembedahan Pekerja Asing (SKHPPA). Sila pastikan juga bahawa anda meneliti terma-terma dan syaratsyarat asas. 1) Produk apakah ini? TAKAFUL IKHLAS BERHAD TAKAFUL IKHLAS BERHAD adalah dilesenkan di bawah Akta Takaful 1984 dan dikawal selia oleh Bank Negara Malaysia. Skim Takaful Kemasukan Hospital dan Pembedahan Pekerja Asing (SKHPPA) Tarikh: 07/05/2014 Skim Takaful Kemasukan Hospital dan Pembedahan Pekerja Asing (SKHPPA) adalah Skim Takaful kemasukan hospital dan pembedahan yang diperkenalkan oleh Kerajaan Malaysia sebagai suatu inisiatif untuk mengurangkan beban kewangan pihak Majikan ke atas pekerja asing sekiranya pekerja asing tersebut dimasukkan ke hospital Kerajaan dalam Malaysia yang bukan diperbadankan disebabkan oleh kemalangan ataupun penyakit. Golongan yang layak untuk perlindungan di bawah sijil ini adalah pekerja asing yang bekerja pada masa sekarang/ masa hadapan secara sepenuh masa dengan peserta, berumur dari lapan belas (18) tahun sehingga lima puluh sembilan (59) tahun, yang masih aktif bekerja pada tarikh orang itu layak untuk menyertai sijil ini. 2) Apakah konsep Shariah yang diguna pakai? Tabarru bermaksud derma bagi tujuan perpaduan dan kerjasama di kalangan peserta-peserta dan akan digunakan untuk membantu Peserta yang ditimpa musibah. Di dalam konteks Takaful IKHLAS, Tabarru akan diperuntukkan ke dalam Dana Risiko. Wakalah bermaksud agensi. Di dalam konteks ini, ia merujuk kepada kontrak di antara Syarikat dan Peserta di mana Peserta melantik Syarikat untuk menguruskan dana bagi pihak Peserta, yang membenarkan Syarikat untuk membantu mereka yang memerlukan. 3) Apakah perlindungan /manfaat yang ditawarkan? Pelan ini menyediakan manfaat-manfaat berikut secara pembayaran balik dengan manfaat dikenakan had tahunan seperti di bawah: Perkara Jadual Manfaat Jumlah (RM) 1(a) 1(b) Bilik Hospital & Makan Harian (Maksimum sehingga 30 hari) Unit Rawatan Rapi (Maksimum sehingga 15 hari) 2 Bekalan & Khidmat Hospital 3 Bilik Bedah 4 Bayaran Pembedahan (Tidak merangkumi transplant organ) 5 Bayaran Pakar Bius 6 7 Lawatan Pakar Perubatan Dalam Hospital (Maksimum sehingga 30 hari) Lawatan Rundingan Pakar Dalam Hospital (Maksimum sehingga 30 hari) Bayaran yang dikenakan mengikut bayaran yang selaras dengan Bilik & Makan Kelas Ketiga (ke-3) sehingga maksimum RM60 sehari di Hospital Kerajaan di dalam Malaysia dan Bukan Korporat mengikut Akta Fi 1951, Perintah Fi (Perubatan)

2 8 Bayaran Ambulans/Bayaran Laporan Perubatan Had Tahunan Keseluruhan Maksimum (Perkara 1-8) Bagi Peserta RM 10, Di penghujung tempoh Takaful, sijil ini boleh diperbaharui setiap tahun mengikut pilihan Takaful Ikhlas Berhad (selepas ini disebut Syarikat ) sehingga 60 tahun. Nota: Penerangan di atas adalah makluman ringkas untuk rujukan yang cepat dan mudah. Syarat dan terma yang tepat yang diguna pakai adalah sepertimana yang dinyatakan di dalam kontrak sijil. 4) Berapakah jumlah sumbangan yang perlu dibayar? Sumbangan tahunan adalah sebanyak RM ke atas setiap pekerja asing yang dilindungi termasuk fi yang dikenakan oleh Managed Care Organisation (MCO). Syarat Pembayaran: Pembayaran Tunai Sebelum Perlindungan (Cash Before Cover CBC) 5) Apakah yuran dan caj yang perlu dibayar? Yuran Wakalah Jenis Yuran Managed Care Organisation (MCO) Duti setem RM Cukai kerajaan (bukan individu) Jumlah 25% daripada RM (sumbangan tahunan tolak fi MCO) terdiri dari: 10% daripada RM (sumbangan tahunan tolak fi MCO) akan ditolak sebagai komisen. dan 15% daripada RM (sumbangan tahunan tolak fi MCO) akan ditolak sebagai Perbelanjaan Pengurusan RM15 untuk setiap pekerja asing 6% daripada sumbangan tahunan (Walaubagaimanapun, ianya dikecualikan oleh Kastam Diraja Malaysia sekiranya sumbangan dibayar oleh pekerja.) Nota: Caj kami tidak termasuk cukai yang dikenakan oleh kerajaan dan/atau pihak berkuasa pada masa akan datang (termasuk Cukai Barangan dan Perkhidmatan (GST). Setelah perlaksanaan GST, kami berhak mengenakan caj GST bagi maksud Sijil Takaful ini yang dikehendaki dibayar di bawah undang-undang. 6) Apakah terma dan syarat utama yang perlu diketahui? Kepentingan Pendedahan Anda harus mendedahkan semua fakta-fakta yang berkaitan dengan Peserta seperti keadaan kesihatan, pekerjaan serta tahap kesihatan mereka yang tepat. Kenyataan Mengikut Seksyen 28 Akta Takaful Anda hendaklah memberi maklumat secara penuh dan jujur segala butir-butir yang anda tahu atau patut tahu di atas borang cadangan ini dan jika tidak sijil yang dikeluarkan menurut cadangan ini akan menjadi tidak sah. Wilayah Geografi Semua manfaat yang dinyatakan di sini adalah terpakai di dalam Malaysia sahaja 24 jam sehari. Perlindungan terhenti sebaik sahaja Peserta meninggalkan Malaysia dan bersambung semula sekembalinya Peserta ke Malaysia. Had Manfaat- Semua manfaat yang disediakan di dalam sijil ini adalah hanya berbayar jika Peserta dimasukkan ke hospital Kerajaan Malaysia yang bukan diperbadankan. 2

3 Tempoh Ihsan - Ini adalah sijil Tunai Sebelum Perlindungan. Meskipun terdapat syarat Tunai sebelum Perlindungan, suatu tempoh ihsan selama empat belas (14) hari dari tarikh perlu dibayar dibenarkan untuk pembayaran sebarang sumbangan setelah Tahun Pertama sijil. Dalam tempoh selama empat belas (14) hari tersebut, Syarikat akan bertanggungjawab jika pada akhir tempoh berkenaan, sumbangan telah dibayar. Sekiranya sebarang sumbangan berkaitan dengan sijil Kontrak ini tidak berbayar sebelum tarikh tamat tempoh ihsan, sijil Kontrak ini dianggap sebagai terbatal pada tarikh luput sijil ini. Syarikat berhak untuk tidak menerima sebarang permohonan di mana pekerja terlibat dengan aktiviti-aktiviti yang tidak mematuhi syariah. Pihak Syarikat akan mengenakan Caj Pengurusan Lebihan (SAC) sebanyak 50% daripada lebihan bolehagih kasar dari Dana Risiko pada setiap akhir tahun. Walau bagaimanapun, pihak Syarikat boleh mengenakan caj SAC kurang daripada 50% daripada lebihan boleh-agih kasar mengikut budi bicara dan kesesuaian. Sebarang lebihan boleh-agih bersih (selepas ditolak SAC) daripada Dana Risiko akan diperuntukkan seluruhnya (100%) kepada Peserta. Sekiranya jumlah untuk Peserta kurang daripada RM10 untuk setiap sijil, jumlah tersebut akan terus disimpan di dalam Dana Risiko. Nota: Senarai di atas adalah tidak lengkap. Sila rujuk kepada kontrak sijil untuk terma-terma dan syarat-syarat lengkap di bawah sijil ini. 7) Apakah pengecualian utama yang terdapat di dalam sijil ini? Sijil ini tidak melindungi mana-mana penghospitalan, pembedahan atau bayaran yang disebabkan oleh manamana satu (1) kejadian yang berikut: Pembedahan plastik/kosmetik; Rawatan pergigian atau pembedahan oral; Rawatan atau operasi pembedahan untuk keabnormalan atau kecacatan kongenital termasuk penyakit keturunan; Kehamilan atau keguguran; Rawatan yang tidak memerlukan keperluan perubatan; Bunuh diri atau kecederaan diri sendiri yang disengajakan ketika siuman atau tidak siuman; Sakit atau kecederaan yang timbul daripada apa-apa jenis perlumbaan atau sukan berbahaya; Penyakit kardiovaskular dan semua jenis kanser yang berlaku dalam masa seratus dua puluh (120) hari pertama perlindungan ini berkuatkuasa. Penyakit sedia ada kecuali sekiranya Pekerja telah lulus pemeriksaan perubatan serta telah disahkan oleh Fomema Sdn. Bhd. (FOMEMA) dalam masa tiga puluh (30) hari ketibaan Peserta ke Malaysia. Nota: Senarai di atas adalah tidak lengkap. Sila rujuk kontrak sijil untuk senarai penuh pengecualian di bawah sijil ini. 8) Bolehkah saya membatalkan sijil saya? Ya, anda (Peserta) boleh membatalkan sijil ini bila-bila masa dengan memberikan notis secara bertulis kepada syarikat. Di atas pembatalan tersebut,anda adalah layak untuk pembayaran balik sumbangan seperti jadual di bawah, dengan syarat anda tidak pernah membuat tuntutan dalam tahun sijil semasa. Be sure to also read the general terms and conditions.) TAKAFUL IKHLAS BERHAD TAKAFUL IKHLAS BERHAD is licensed under the Takaful Act 1984 and regulated by Bank Negara Malaysia. Foreign Worker Hospitalization & Surgical Takaful Scheme (SKHPPA) Date: 07/05/2014 1) What is this product about? Foreign Worker Hospitalization & Surgical Takaful Scheme (SKHPPA) is a yearly renewable hospital and surgical Takaful scheme introduced by the Malaysian government as an initiative to reduce the financial burden of the employer of foreign workers in the event of hospital admission of their foreign workers to a Non- Corporatised Malaysian Government Hospital due to an accident or illness. Eligible persons for coverage under this certificate are those present and future full-time foreign worker employees of participant, from the age of eighteen (18) to fifty-nine (59), who are actively engaged at their usual work on the date the persons are eligible to join this certificate. 2) What are the Shariah concepts applicable? Tabarru shall mean donation for the purpose of solidarity and cooperation among the participants and to be used to help participants in times of misfortune. In the context of Takaful IKHLAS, Tabarru will be allocated into the Risk Fund. Wakalah shall mean agency. In this context, it refers to a contract between Company and the Participant where the Participant authorizes the Company to manage the fund on his/her behalf, which allows the Company to charge a Wakalah fee. 3) What are the covers / benefits provided? This plan provides the following benefits on a reimbursement basis with the total benefits payable subjected to the annual limit stated below: Item Benefits Amount (RM) 1(a) Daily Hospital Room & Board (Maximum up to 30 days) 1(b) Intensive Care Unit (Maximum up to 15 days) 2 Hospital Supplies & Services 3 Operating Theatre 4 Surgical fess 5 Anesthetist Fees 6 In-Hospital Physician Visits (Maximum up to 30 days) In-Hospital Specialist Consultation Visits (Maximum up 7 to 30 days) 8 Ambulance Fees/Medical Report Fees As charged in accordance to charges consistent with Third (3rd) Class Room and Board to a maximum of RM60.00 per day in a Non- Corporatised Malaysian Government Hospital in conformance to the charges specified under Fees Act 1951, Fees (Medical) Order Maximum Overall Annual Limit (Item 1-8) per participant RM 10,

6 On each certificate anniversary, this certificate is renewable annually at the option of the Takaful IKHLAS (hereinafter called "the Company") up to age 60. Note: The description on the available cover is only a brief summary for quick and easy reference. The precise terms and conditions that apply are stated in the certificate contract. 4) How much contribution do I have to pay? The annual contribution is RM per foreign worker covered inclusive of RM15 Managed Care Organisation (MCO) fee. Terms of Payment: Cash Before Cover Payment 5) What are the fees and charges that I have to pay? Type Wakalah fees Managed Care Organisation (MCO) fees Stamp Duty Government Tax (non-individual) Amount 25% of RM (annual contribution less MCO fees) of which: 10% of RM (annual contribution less MCO fees) will be deducted for commission; and 15% of RM (annual contribution less MCO fees) will be deducted for Administrative Expense RM15 per foreign worker You have to pay RM in addition of the contribution for stamp duty. 6% of the Gross Contribution (RM120.00) (Exempted by Kastam Diraja Malaysia provided the contribution is paid by the employee.) Note: Our charges exclude tax that would be imposed by the government and/or the authority in the future (including Goods and Services Tax (GST)). Upon implementation of the GST, we shall be entitled to recover the GST from you in respect of this Takaful Certificate that is required under the law. 6) What are some of the key terms and conditions that I should be aware of? Importance of disclosure You must disclose all material facts of all the covered members such as their medical condition, occupation, and state their ages correctly. Statement Pursuant to Section 28 of the Takaful Act 1984: You are to disclose in the proposal form, fully and faithfully all the facts which you know or ought to know, otherwise the certificate issued may be invalidated. Geographical Territory All benefits provided in this certificate are applicable within Malaysia only for twenty-four (24) hours a day. Cover ceases from the time the Participant leaves Malaysia and resumes upon his/her return to Malaysia. Limitation of Benefits All benefits provided in this certificate are only payable in the event the Participant is confined in a non-corporatised Malaysian Government Hospital. 6

7 Grace Period This is a Cash-Before-Cover certificate. Notwithstanding the Cash-Before-Cover condition, a grace period of fourteen (14) days from its due date will be allowed for payment of each contribution after the first certificate year. During such fourteen (14) days, the Company shall remain liable there under if by the last of such days, the contribution is actually paid. If any contribution is not paid in respect of this certificate contract before the end of the Grace Period, this certificate contract shall be deemed as terminated at the expiry date of this certificate. The Company has the right not to accept any proposal where the worker is involved in any non-shariah compliance activities. The Company shall charge a Surplus Administration Charge (SAC) of 50% of the gross distributable surplus arising at the end of the financial year. However, the Company may at its discretion and where appropriate charge SAC less than 50% of the gross distributable surplus. Any net distributable surplus arising (after deducting the SAC) from the Risk Fund will be allocated in full (100%) to the Participant. If the amount due to Participant is less than RM10 per Certificate, the amount shall be retained in the Risk Fund. Note: The list above is non-exhaustive. Please refer to the certificate contract for the full terms and conditions under this certificate. 7) What are the major exclusions under this certificate? This certificate does not cover any hospitalization, surgery or charges caused by any one of the following occurrences: Plastic/Cosmetic surgery; Dental treatment or oral surgery; Treatment or surgical operation for congenital abnormalities or deformities; Pregnancy or miscarriage; Treatment which is not medically necessary; Suicide or self-inflicted injury while sane or insane; Accidental injuries or illnesses arising from racing or hazardous sports. Cardiovascular diseases and all cancers occurring within the first one hundred and twenty (120) days of Takaful coverage of the Participant. Pre-existing conditions unless the Participant passes the medical examination as continued by FOMEMA Sdn Bhd (FOMEMA) within thirty (30) days from the Participant s arrival to Malaysia. Note: The list above is non-exhaustive. Please refer to the certificate contract for the full list of exclusions under this Certificate. 7

8 8) Can I cancel my certificate? Yes, you (the Participant) may cancel this certificate at any time by giving written notice to us. Upon cancellation, you are entitled to a refund of the contribution as per the schedule below, provided that you have not made a claim during the current certificate year.
