1 Hospitalization & Surgical Insurance Product Disclosure Sheet B-Rawat (Please read this Product Disclosure Sheet before you decide to take out this insurance. Be sure to also read the general terms and conditions.) 1. What is this product about? This product provides for hospitalization & surgical expenses incurred due to accident and illnesses covered by the product. 2. Who is eligible? Any Malaysian from age 19 to 55 and is renewable up to 70 years old. Dependent children (unmarried & unemployed) must be 1 year old and under the age of 19 or up to the age of 23 for those registered full time students at a recognized educational institution in Malaysia. 3. What are the covers / benefits provided? 2 Duration of cover is for one year. You need to renew your cover annually. 4. How much premium do I have to pay? Premium is based on your age, plan and subject to underwriting requirements of the insurance company. Payment can be made by cash, credit card or cheque (cheque should be made payable only in the name of BERJAYA SOMPO INSURANCE BERHAD). Premium rates are not guaranteed. Factors which lead to increase in premium are your claim experience, medical inflation and our overall experience in underwriting Hospitalization and Surgical insurance. 5. What are the fees and charges that I have to pay? 3 6. What is the Period of Cover and Renewal Option? The Policy shall become effective as of the date stated in the Schedule. The Policy Anniversary shall be one year after the effective date and annually thereafter. On each such anniversary, this Policy is renewable at the premium rates in effect at that time as notified by the Company. The renewal premium is not guaranteed and the Company reserves the right to revise the premium rate applicable at the time of renewal. Factors contributing to the increase of renewal premium are your age and your claim experience. Renewal will not be invited once you attain the maximum age limit of this product. 7. What are some of the key terms and conditions that I should be aware of? You should provide sufficient and accurate information to us or our intermediary to enable us to advise you on the Hospitalization and Surgical insurance which suits your needs. IMPORTANCE OF DISCLOSURE Pursuant to Paragraph 5 of Schedule 9 of the Financial Services Act 2013, you have a duty to take reasonable care not to make a misrepresentation in answering the questions in the Proposal Form. You must answer the questions fully and accurately. Failure to take reasonable care in answering the questions may result in avoidance of your contract of insurance, refusal or reduction of your claim(s), change of terms or termination of your contract of insurance. The above duty of disclosure shall continue until the time your contract of insurance is entered into, varied or renewed with us. In addition to answering the questions in the Proposal Form, you are required to disclose any other matter that you know to be relevant to our decision in accepting the risks and determining the rates and terms to be applied. You also have a duty to tell us immediately if at any time after your contract of insurance has been entered into, varied or renewed with us any of the information given in the Proposal Form is inaccurate or has changed. PRE-EXISTING ILLNESS Pre-existing Illness shall mean disabilities that existed before the Effective date of Insurance that the Insured Person has reasonable knowledge of. An Insured Person may be considered to have reasonable knowledge of a pre-existing condition where the condition is one for which:- a) the Insured Person had received or is receiving treatment; b) medical advice, diagnosis, care or treatment has been recommended; c) clear and distinct symptoms are or were evident; or d) its existence would have been apparent to a reasonable person in the circumstances. SPECIFIED ILLNESSES Specified Illnesses shall mean the following disabilities and its related complications, occurring within the first 120 days of Insurance of the Insured Person: a) Hypertension, diabetes mellitus and Cardiovascular disease. b) All tumours, cancer, cysts, nodules, polyps, stones of the urinary system and biliary system. c) All ear, nose (including sinuses) and throat condition. 3

4 d) Hernias, haemorrhoids, fistulae, hydrocele, varicocele. e) Endometriosis including disease of the Reproduction System. f) Vertebro-spinal disorders (including disc) and knee conditions. WAITING PERIOD Waiting Period shall mean the first 30 days between the beginning of an Insured Person s disability and the commencement of this Policy date/reinstatement date and is applied only when the person is first covered. This shall not be applicable after the first year of cover. However, if there is a break in insurance, the Waiting Period will apply again. UPGRADED ROOM & BOARD CO-PAYMENT If the Insured Person is hospitalized at a published Room & Board rate which is higher than his/her eligible benefit, the Insured Person shall bear 20% of the other eligible benefits described in the Schedule of Benefits. RESIDENCE OVERSEAS No benefit whatsoever shall be payable for any medical treatment received by the Insured outside Malaysia, if the Insured resides or travels outside Malaysia for more than ninety (90) consecutive days. OVERSEAS TREATMENT If the Insured Person seeks treatment overseas, benefits in respect of the treatment shall be covered subject to the exclusions, limitations and conditions specified in this Policy and all benefits will be payable based on the official exchange rate ruling on the last day of the Period of Confinement and shall exclude the cost of transport to the place of treatment provided; a) an Insured Person travelling abroad for a reason other than for medical treatment, needs to be confined to a Hospital outside Malaysia as a consequence of a Medically Emergency. b) an Insured Person upon recommendation of a Physician and has to be transferred to a Hospital outside Malaysia because the specialised nature of the treatment, aid, information or decision required can neither be rendered nor furnished nor taken in Malaysia. Overseas Treatment of a disease, sickness or injury which is diagnosed in Malaysia and nonemergency or chronic conditions where treatment can reasonably be postponed until return to Malaysia are excluded. REASONABLE AND CUSTOMARY CHARGES Reasonable and Customary Charges shall mean charges for medical care which is medically necessary shall be considered reasonable and customary to the extent that it does not exceed the general level of charges being made by others of similar standing in the locality where the charge is incurred, when furnishing like or comparable treatment, services or supplies to individual of the same sex and of comparable age for a similar sickness, disease or injury and in accordance with accepted medical standards and practice could not have been omitted without adversely affecting the Insured Person s medical condition. COOLING-OFF PERIOD If this Policy shall have been issued and for any reason whatsoever the Insured Person shall decide not to take up the Policy, the Insured Person may return the Policy to the Company for cancellation provided such request for cancellation is delivered by the Insured Person to the Company within fifteen (15) days from the date of issue of the Policy. The Insured Person is entitled to the return of the full premium paid less deduction of medical expenses incurred by the Company in the issue of the Policy. 4

5 Unless renewed, the coverage will cease on expiry date and the insurance company shall strictly not be liable for any expenses that take place after the expiry date. You are advised to keep the receipt as proof of payment of premium. It may not be advantageous to switch insurance policy / insurer because you may be subjected to new terms and conditions of the new policy or of the new insurer. 8. What are the General Exclusions under this policy? This Policy does not cover:- a) Pre-existing illness. b) Specified Illnesses occurring during the first 120 days of continuous cover. c) Any medical or physical conditions arising within the first 30 days of the Insured Person s cover or date reinstatement whichever is latest except for accidental injuries. d) Care or treatment for which payment is not required or to the extent which is payable by any other insurance or indemnity covering the Insured and Disabilities arising out of duties of employment or profession that is covered under a Workman's Compensation Insurance Contract. e) Plastic/Cosmetic surgery, circumcision, eye examination, glasses and refraction or surgical correction of nearsightedness(radial Keratotomy or Lasik) and the use or acquisition of prosthetic appliances or devices such as artificial limbs, hearing aids, implanted pacemakers and prescriptions thereof. f) Dental conditions including dental treatment or oral surgery except as necessitated by Accidental Injuries to sound natural teeth occurring wholly during the Period of Insurance. g) Private nursing, rest cures or sanitaria care, illegal drugs, intoxication, sterilization, venereal disease and its sequelae, AIDS (Acquired Immune Deficiency Syndrome) or ARC (AIDS Related Complex) and HIV related diseases, and any communicable diseases required quarantine by law. h) Any treatment or surgical operation for congenital abnormalities or deformities including hereditary conditions. i) Pregnancy, child birth (including surgical delivery) and its related complications, miscarriage, abortion and prenatal or postnatal care and surgical, mechanical or chemical contraceptive methods of birth control or treatment pertaining to infertility. Erectile dysfunction and tests or treatment related to impotence or sterilization. j) Psychotic, mental or nervous disorders, (including any neuroses and their physiological or psychosomatic manifestations). k) Hospitalization primarily for investigatory purposes, all diagnostic tests including and not limited to Positron Emission Tomography (PET) Scan, Computed Tomography (CT) Scan, Computed Axial Tomography (CAT) Scan, Magnetic Resonance Imaging (MRI), X-ray examination, general physical or medical examinations, not incidental to treatment or diagnosis of a covered Disability or any treatment which is not Medically Necessary and any preventive treatments, preventive medicines or examinations carried out by a Physician, and treatments specifically for hyperhidrosis, weight reduction or gain. l) Costs/expenses of services of a non-medical nature, such as television, telephones, telex services, radios or similar facilities, admission kit/pack and other ineligible non-medical items. m) Sickness or Injury arising from racing of any kind (except foot racing), hazardous sports such as but not limited to skydiving, water skiing, underwater activities requiring breathing apparatus, winter sports, professional sports and illegal activities. 5

6 n) Suicide, attempted suicide or intentionally self-inflicted injury while sane or insane. o) Private flying other than as a fare-paying passenger in any commercial scheduled airlines licensed to carry passengers over established routes. p) War or any act of war, declared or undeclared, criminal or terrorist activities, active duty in any armed forces, direct participation in strikes, riots and civil commotion or insurrection. q) Ionising radiation or contamination by radioactivity from any nuclear fuel or nuclear waste from process of nuclear fission or from any nuclear weapons material. r) Expenses incurred for donation of any body organ by an Insured Person and costs of acquisition of the organ including all costs incurred by the donor during organ transplant and its complications. s) Expenses incurred for sex changes t) Investigation and treatment of sleep and snoring disorders, hormone replacement therapy, stem cell treatment and alternative therapy such as treatment, medical service or supplies, including but not limited to chiropractic services, acupuncture, acupressure, reflexology, bonesetting, herbalist treatment, massage or aroma therapy or other alternative treatment. 9. Can I cancel my policy? You may cancel your policy by giving a written notice to the insurance company provided that no claims have been made during the current policy year. You are entitled to a refund of premium based on the premium refund table as set out in the policy. 10. What do I need to do if there are changes to my contact details? It is important that you inform us of any change in your contact details to ensure that all correspondences reach you in a timely manner. 11. How do I make claim? By notifying us within 30 days of any occurrences and submitting the claim form, medical report, original itemised bills, receipts and other relevant claims documents. 12. Produk ini melindingi bayaran penghospitalan dan pembedahan yang ditanggung akibat kemalangan dan penyakit yang dilindungi di bawah produk ini. 2. Siapa yang layak? Semua rakyat Malaysia yang berumur 19 hingga 55 tahun dan boleh diperbaharui sehingga umur 70 tahun. Anak-anak di bawah tanggungan (bujang dan tidak bekerja) mestilah berumur 1 tahun dan di bawah 19 tahun atau sehingga berumur 23 tahun bagi mereka yang masih menuntut di institusi pengajian yang diiktiraf di Malaysia. 3. Pembayaran boleh dibuat dengan tunai, kad kredit atau cek (pembayaran melalui cek hendaklah dibuat di atas nama BERJAYA SOMPO INSURANCE BERHAD). Kadar premium adalah tidak terjamin. Faktor-faktor yang menyebabkan peningkatan premium adalah pengalaman tuntutan anda, inflasi perubatan dan pengalaman keseluruhan kami dalam pengunderaitan insurans Penghospitalan dan Pembedahan. 9

10 5. Apakah kos dan caj yang saya perlu bayar? Apa yang anda perlu bayar sebagai tambahan kepada premium Amaun Duti Setem RM Cukai Barangan dan Perkhidmatan (CBP) 6% dari premium Apa yang dimasukkan dalam premium Komisen untuk pengantara insurans (jika ada) Amaun 15% dari premium (polisi individu) 10% dari premium (polisi kumpulan) 6. Apakah tempoh perlindugan polisi dan opsyen pembaharuan? Polisi akan berkuatkuasa mengikut tarikh yang dinyatakan pada Jadual Polisi. Ulang Tahun Polisi adalah satu tahun selepas tarikh kuatkuasa dan setiap tahun berikutnya. Pada setiap ulang tahun, polisi boleh dibaharui pada kadar premium yang berkuatkuasa pada masa itu seperti yang diberitahu oleh Syarikat. Faktor yang menyumbang kepada peningkatan premium pembaharuan adalah umur anda dan pengalaman tuntutan anda. Pembaharuan tidak akan dijemput sekiranya anda mencapai had umur produk ini. Polisi boleh dibaharui mengikut pilihan pemegang polisi tertakluk kepada terma dan syarat dan penamatan pada setiap ulangtahun tarikh Polisi. Premium pembaharuan yang dibayar tidak terjamin dan Syarikat berhak menyemak semula kadar premium pada masa pembaharuan itu. 7. Apakah terma-terma dan syarat-syarat yang perlu diberi perhatian? Anda harus memberikan maklumat yang lengkap dan tepat kepada kami atau pengantara kami untuk membolehkan kami menasihati anda tentang Insurans Penghospitalan dan Pembedahan yang sesuai dengan keperluan anda. KEPENTINGAN PENDEDAHAN Menurut Perenggan 5 daripada Jadual 9 Akta Perkhidmatan Kewangan 2013, anda mempunyai kewajipan untuk mengambil langkah yang munasabah untuk tidak salah nyata dalam menjawab soalan-soalan dalam Borang Cadangan ini. Anda dikehendaki menjawab soalan-soalan dalam Borang Cadangan ini dengan lengkap dan tepat. Kegagalan untuk mengambil langkah yang munasabah dalam menjawab soalan-soalan, mungkin mengakibatkan pembatalan kontrak insurans anda, keengganan atau pengurangan gantirugi, perubahan terma atau penamatan kontrak insurans anda. Kewajipan pendedahan diatas hendaklah diteruskan sehingga kontrak insurans anda dimeterai, diubah atau diperbaharui dengan kami. Sebagai tambahan kepada soalan-soalan dalam Borang Cadangan ini, anda dikehendaki untuk mendedahkan apa-apa perkara lain yang anda tahu akan mempengaruhi keputusan kami dalam menerima risiko dan menentukan kadar dan terma yang dikenakan. Anda juga mempunyai kewajipan untuk memberitahu kami dengan serta-merta jika pada bila-bila masa selepas kontrak insurans anda ditandatangani, diubah atau diperbaharui dengan kami, apaapa maklumat yang dinyatakan dalam Borang Cadangan ini tidak tepat atau telah berubah. 10

11 PENYAKIT SEDIA ADA Penyakit Sedia Ada bermaksud hilang upaya yang wujud sebelum tarikh berkuatkuasa insurans dimana Orang yang Diinsuranskan mengetahui sewajarnya. Orang yang Diinsuranskan dianggap mengetahui sewajarnya keadaan sedia ada itu apabila:- a) Orang yang Diinsuranskan telah atau sedang menerima rawatan; b) nasihat perubatan, diagnosis, jagaan atau rawatan telah disyorkan; c) gejala yang jelas dan tepat dapat atau telah dilihat dengan nyata; atau d) kewujudannya dapat diperhatikan dengan jelas bagi orang yang mengalami keadaan itu. PENYAKIT TERTENTU Penyakit Tertentu bermaksud hilang upaya berikut dan komplikasi yang berkaitan dengannya, yang wujud dalam tempoh 120 hari pertama lnsurans bagi Orang yang Diinsuranskan:- a) Hipertensi, diabetes melitus dan penyakit kardiovaskular b) Semua tumor, kanser, sista, nodul, polip, batu dalam sistem kencing dan sistem biliari c) Semua penyakit telinga, hidung (termasuk sinus) dan tekak d) Hernia, hemoroid, fistula, hidrosele, varikosele e) Endometriosis termasuk penyakit sistem pembiakan f) Gangguan spina vertebro (termasuk diska) dan penyakit lutut. TEMPOH TANGGUH Tempoh Tangguh bermaksud 30 hari pertama antara masa bermulanya hilang upaya Orang yang Diinsuranskan dan bermulanya tarikh Polisi/tarikh pengembalian semula, dan terpakai hanya apabila orang itu dilindungi pertama kalinya. Hal ini tidak terpakai selepas tahun pertama perlindungan. Walau bagaimanapun, jika insurans terhenti, maka Tempoh Tangguh akan terpakai sekali lagi. BAYARAN BERSAMA BILIK DAN MAKAN DINAIKKAN Jika Orang yang Diinsuranskan dimasukkan ke hospital pada kadar Bilik & Makan yang diumumkan yang lebih tinggi daripada manfaat yang dia layak, Orang yang Diinsuranskan hendaklah menanggung 20% daripada manfaat lain yang dia layak yang diterangkan dalam Jadual Manfaat. TINGGAL DI LUAR NEGARA Tiada manfaat akan dibayar untuk apa-apa rawatan perubatan yang diterima oleh Orang yang Diinsuranskan di luar Malaysia, jika Orang yang Diinsuranskan itu tinggal atau membuat perjalanan di luar Malaysia lebih daripada sembilan puluh (90) hari berturut-turut. RAWATAN LUAR NEGARA Jika Orang yang Diinsuranskan ingin mendapatkan rawatan luar negara, manfaat berkaitan rawatan hendaklah dilindungi tertakluk kepada pengecualian, had dan syarat yang ditetapkan dalam Polisi ini dan semua manfaat akan dibayar berdasarkan keputusan kadar pertukaran mata wang rasmi pada hari terakhir Tempoh Dimasukkan ke hospital dan tidak termasuk kos pengangkutan ke tempat rawatan disediakan; a. Orang yang Dinsuranskan yang pergi ke luar negara atas sebab selain mendapatkan rawatan perubatan, perlu dimasukkan ke Hospital di luar Malaysia kerana Kecemasan Perubatan. b. Orang yang Diinsuranskan, atas syor Pakar Perubatan dan perlu dipindahkan ke Hospital di luar Malaysia disebabkan oleh jenis yang khusus bagi rawatan, bantuan, maklumat atau keputusan yang dikehendaki tidak boieh diberikan atau disediakan atau diambil di Malaysia. Rawatan luar negara bagi penyakit, sakit atau kecederaan yang didiagnosis di Malaysia dan 11

12 keadaan bukan kecemasan atau kronik yang rawatannya boleh ditangguhkan sewajarnya hingga pulang ke Malaysia dikecualikan. BAYARAN MUNASABAH DAN BIASA Bayaran Munasabah dan Biasa bermaksud bayaran rawatan perubatan yang perlu dari segi perubatan, yang dianggap munasabah dan biasa selagi bayaran itu tidak melebihi had biasa bayaran yang dibuat oleh pihak lain yang sama kedudukannya di tempat bayaran itu dikenakan, apabila memberikan rawatan, khidmat atau bekalan seumpamanya atau yang serupa kepada individu dengan jantina yang sama dan kategori umur yang sama untuk sakit, penyakit atau kecederaan serta selaras dengan piawai dan amalan perubatan yang diterima dan tidak boleh diabaikan tanpa menjejaskan keadaan perubatan Orang yang Diinsuranskan. TEMPOH IHSAN Jika Polisi ini telah dikeluarkan dan atas apa-apa alasan, Orang yang Diinsuranskan membuat keputusan untuk tidak mengambil Polisi itu, Orang yang Diinsuranskan itu boleh mengembalikan Polisi itu kepada Syarikat untuk pembatalan dengan syarat permohonan untuk pembatalan diserahkan oleh Orang yang Diinsuranskan kepada Syarikat dalam tempoh lima belas (15) hari dari tarikh penyerahan Polisi. Orang yang Diinsuranskan itu layak menerima pulangan penuh premium yang telah dibayar tolak belanja perubatan yang ditanggung oleh Syarikat untuk mengeluarkan Polisi tersebut. Perlindungan akan tamat pada tarikh luput dan syarikat insurans tidak akan bertanggungjawab terhadap sebarang perbelanjaan yang terlibat melainkan perlindungan diperbaharui. Anda dinasihatkan supaya menyimpan resit sebagai bukti pembayaran premium. Penukaran polisi insurans atau syarikat insurans mungkin tidak membawa manfaat kerana anda mungkin tertakluk kepada terma dan syarat baru yang dikenakan oleh polisi baru atau syarikat insurans baru. 8. Apakah pengecualian utama di bawah polisi ini? Polisi ini tidak melindungi:- a) Penyakit sedia ada. b) Penyakit yang ditetapkan yang berlaku dalam tempoh 120 hari pertama bagi perlindungan berterusan. c) Apa-apa keadaan perubatan atau fizikal yang berlaku dalam tempoh 30 hari pertama perlindungan atau tarikh pengembalian semula bagi Orang yang Diinsuranskan, mengikut manamana yang terakhir kecuali untuk kecederaan akibat kemalangan. d) Jagaan atau rawatan yang pembayarannya tidak dikehendaki atau setakat yang ia dibayar oleh mana-mana insurans lain atau tanggung rugi yang melindungi Pengambil Insurans dan Hilang Upaya yang timbul daripada tugas dalam pekerjaan atau kerjaya yang dilindungi di bawah Kontrak lnsurans Pampasan Pekerja. e) Pembedahan plastik/kosmetik, khatan, pemeriksaan mata, cermin mata dan pembetulan penglihatan dekat melalui pembiasan atau pembedahan (Keratotomi radial atau Lasik) dan penggunaan atau pemerolehan perkakas atau alat prostetik seperti anggota tiruan, alat pendengaran, perentak yang diimplankan dan preskripsinya. f) Penyakit pergigian termasuk rawatan pergigian atau pembedahan oral kecuali apabila diperlukan kerana Kecederaan Akibat Kemalangan pada gigi asli yang sihat yang berlaku sepenuhnya dalam Tempoh lnsurans. 12

13 g) Penjagaan peribadi, rehat pulih atau jagaan kebersihan, dadah yang tidak dibenarkan, intoksikasi, pensterilan, penyakit venereal dan sekuelanya, AIDS (Sindrom Kurang Daya Tahan Penyakit) atau ARC (Kompleks Berkaitan AIDS) dan penyakit berkaitan HIV, dan apa-apa penyakit berjangkit yang memerlukan kuarantin oleh undang-undang. h) Apa-apa rawatan atau operasi pembedahan untuk keabnormalan atau kecacatan kongenital termasuk penyakit keturunan. i) Kehamilan, melahirkan anak (termasuk kelahiran secara pembedahan), dan komplikasi yang berkaitan dengannya, keguguran, menggugurkan kandungan dan jagaan serta pembedahan pranatal atau postnatum, kaedah kawalan kelahiran kontraseptif mekanikal atau kimia atau rawatan berkaitan ketaksuburan, disfungsi erektil dan ujian atau rawatan berkaitan impoten atau pensterilan. j) Psikotik, gangguan mental atau saraf (termasuk apa-apa neurosis dan manifestasi fisiologi atau psikosomatiknya). k) Penghospitalan terutamanya untuk tujuan penyiasatan, semua ujian diagnosis termasuk dan tidak terhad kepada Positron Emission Tomography (PET) Scan, Computed Tomography (CT) Scan, Computed Axial Tomography (CAT) Scan, Magnetic Resonance Imaging (MRI) pemeriksaan sinar-x, pemeriksaan fizikal atau perubatan am, tidak berkaitan dengan rawatan atau diagnosis Hilang Upaya yang dilindungi yang tidak Perlu Dari Segi Perubatan dan apa-apa rawatan pencegahan, ubat atau pemeriksaan pencegahan yang dijalankan oleh Pakar Perubatan, dan rawatan khusus untuk hiperhidrosis, mengurangkan atau menaikkan berat badan. l) Kos/belanja bagi khidmat yang bersifat bukan perubatan, seperti televisyen, telefon, khidmat teleks, radio atau kemudahan yang serupa, kit/pek kemasukan dan barang bukan perubatan lain yang tidak layak. m) Sakit atau kecederaan yang timbul daripada apa-apa jenis perlumbaan (kecuali perlumbaan jalan kaki), sukan berbahaya seperti tetapi tidak terhad kepada terjun di udara, luncur air, aktiviti dalam air yang memerlukan alat pernafasan, sukan musim sejuk, sukan profesional dan aktiviti yang tidak dibenarkan. n) Bunuh diri, percubaan bunuh diri atau kecederaan diri sendiri yang disengajakan ketika siuman atau tidak siuman. o) Penerbangan peribadi selain sebagai penumpang yang membayar tambang dalam mana-mana penerbangan komersil berjadual untuk mengangkut penumpang melalui laluan yang ditetapkan. p) Perang atau apa-apa tindakan perang, diisytiharkan atau tidak diisytiharkan, aktiviti jenayah atau pengganas, bergiat cergas dalam mana-mana angkatan bersenjata, penyertaan secara langsung dalam mogok, rusuhan dan kekecohan awam atau penderhakaan tentera. q) Radiasi pengionan atau pencemaran melalui radioaktiviti daripada mana-mana bahan api nuklear atau sisa nuklear daripada proses pembelahan nuklear atau daripada apa-apa bahan senjata nuklear. r) Belanja yang ditanggung untuk menderma mana-mana organ tubuh oleh Orang yang Diinsuranskan dan kos pemerolehan organ termasuk semua kos yang ditanggung oleh penderma ketika transplan organ dan komplikasinya. s) Belanja yang ditanggung untuk menukar jantina. t) Penyiasatan dan rawatan gangguan tidur dan dengkur, terapi penggantian hormon, rawatan stem sel dan terapi pilihan seperti rawatan, khidmat atau bekalan perubatan, termasuk tetapi tidak terhad kepada khidmat kiropraktik, akupunktur, akutekanan, refleksologi, pengikatan tulang, rawatan pakar herba, urut atau terapi aroma atau rawatan pilihan yang lain. 13

14 9. Bolehkah saya membatalkan polisi saya? Anda boleh membatalkan polisi dengan memberikan notis bertulis kepada syarikat insurans dengan syarat tiada tuntutan dibuat dalam tahun polisi semasa. Anda akan menerima bayaran balik premium mengikut jadual bayaran balik premium seperti tertera di polisi. 10. Apakah yang harus saya lakukan jika terdapat perubahan dalam maklumat hubungan saya? Adalah penting bagi anda untuk memaklumkan pihak kami tentang sebarang perubahan mengenai maklumat hubungan anda bagi memastikan semua surat-menyurat dapat disampaikan kepada anda tepat pada masanya. 11. Bagaimana saya membuat tuntutan? Anda boleh membuat tuntutan dengan memberitahu kami dalam jangka masa 30 hari dari sebarang kejadian dan menghantarkan borang tuntutan, laporan perubatan, salinan asal bil terperinci, resit dan dokumen-dokumen lain yang berkaitan. 12.
