Applications can be obtained from OS/Establishment department, LIC of India, 11 th Floor, Zonal office, 153 Annasalai, Chennai 2 by remitting Rs 250/-+GST(18%) by cash/dd at cash counter during cash hours. If application fee is not remitted, DD drawn in favour of LIC of India for Rs 250/-+GST (18%) payable at Chennai should be enclosed along with Sealed quotations. The tender fee is non-refundable. The application should be returned to us duly filled and signed along with enclosures: 1. Receipt copy for Application fee/dd for Rs 295/- 2. GST registration copy 3. PAN copy 4. Proof for registration under MSME, if applicable 5. Copy of registration of firm with Government authorities 6. Copies of P&L statement for annual sales and ITR for the past 3 years as given 7. 5 TERMS AND CONDITIONS OF THE TENDER 1. The average Annual Turnover should be above 10 Lakhs in the last three financial years. Audited Financial statements for the years , & should be enclosed 2. The Tenderer should have an experience in the respective field of supply and service of CCTV for a minimum period of 3 years. 3. The tenderer should be providing similar services for the minimum of one Govt/Semi Govt/PSU presently. Copy of work order, TDS certificates should be enclosed as a proof. 4. Income tax returns certificates of last three consecutive years as a proof for IT filing(assessment year , & ) 5. Agency should have local office at Chennai with sufficient personnel to attend to work and should possess the authorized dealership for the product 6. A copy of the registration with the Regional Labour Commissioner (Central Government) for engaging contract labour as required under the Contract Labour (Regulation and Abolition) Act, If applicable, the firm will have to submit the labour license within a week of awarding the work. 7. An undertaking to the effect that if the AMC is awarded after warranty period and for the existing CCTVs by calling for tenders from the empanelled vendors. The tenderer will comply to all the requirements conforming to various Acts related to the employment of Contract Labour including, the Employees Provident Funds and Miscellaneous Provisions Act 1952; the Payment of Wages act, 1936; the Central Minimum Wages Act; ESI etc. in so far as the same may be applicable. 8. The contractor should deploy the skilled person or carryout the work in supervision of a skilled person, who have the technical knowledge of the work. 9. For the works that cannot be done from the ground level, necessary means should be provided by the tenderer. Drum or other make shifts must not be used in place of ladders with proper support. 10.Any person found under the influence of alcohol or any intoxicating drugs on duty is unfit for duty and should not be allowed to work. 11. Necessary equipments for installing/maintaining the CCTV should be brought by the technician. The offered system on our order should be as per specification and shall be currently in production. 12. Installation will be in the premises as specified by LIC of India. Page 5 of 7

The contractor will be fully liable to compensate all concerned for any loss, damage or destruction of structures, persons, property etc. including third party risk arising due to causes attributable to the contractor as may be decided by the LIC whose decision in this regard shall be final. No claim shall be entertained by LIC on this account. 14. AMC will cover the quarterly servicing and whenever fault is reported, fault should be attended within 24 hours of registering the complaint. In case the faulty part/unit cannot be repaired within 24 Hours after the first service visit by Tenderer s Engineer, the Tenderer will provide a spare unit till the time faulty part is repaired and put back. If any component/ part/unit needs replacement, the same would be replaced with the current available product from the OEM. However if that product gets obsolete or unavailable in the market, service provider should supply alternate product of same or higher compatible configuration of the existing unit. AMC covers all Labour charges, required spares, wear & tear of components except burnt components and breakage, DVR, monitor, etc. Maintenance also includes all system related, Software related activities, and un-installation. 15.The Firm/Company shall take the workmen compensation policy for the workers engaged in the work on the basis of the monthly salary of the worker. The policy shall also cover the personal accidental death, permanent total disability, permanent partial disability, temporary disablement or any other accident during the execution of the work and medical extension of the worker 16. Tender fees of Rs GST(18%) to be submitted along with application for empanelment, by way of remitting cash at ZO/cash counter or by Demand Draft in favour of LIC of India. 17.The tender should be dispatched so as to reach the office which called for the same, on or before the due date specified. 18. Please note the description and specification of stores carefully when the financial quote is called for and your offer should be in accordance with the same. 19. Any dispute arising out of or relating to this tender shall be deemed to have arisen at the headquarters of the Corporation s Office placing the order and shall be subject to adjudication by a Court in that city. 20. LIC of India is not responsible in any manner for the postal delay/loss/ non receipt of the tender 21. 7 The initial screening for empanelment will be based on the documents submitted, for the following reasons: 1. Tender fee is not remitted/dd is not enclosed 2. Not having minimum running contract with any one Govt./ Semi Govt/PSU and not enclosed proof for the contract 3. Minimum experience in the field of supply, installation and maintenance of CCTV is less than 3 years 4. Average turn over in the past 3 years is less than Rs 10 lacs 5. No service centre with necessary spare parts at Chennai 6. Any Litigation/civil suit is pending 7. Blacklisted/removed/cancelled previously by any your clients 8. Not having registered/not having licence to carry out the work 9. Not registered under GST 10. Not a authorized dealer of the products Page 7 of 7
