2 2 ESIC Model Hospital, Rajajinagar, Bangalore invites sealed quotations from the reputed firms for supplying Laundry Chemicals under Rate Contract. Minimum eligibility: 1. The agency / Service providers should be Bangalore based. 2. Should have minimum of 3 years service in the field 3. Should not have been blacklisted by the Govt. / PSUs The tender document can be downloaded from the website & which should be accompanied by the requisite Earnest Money Deposit(EMD) of Rs. 5,000/- in the form of demand draft/pay order payable at Bangalore in favour of the ESIC MH Fund Account No. 1. Period of downloading of tender form: 19/06/2014 to 09/07/2014. The bid should be accompanied with the EMD of Rs. 5,000/- The tender, complete in all respects, should be dropped in the sealed tender box available in the General branch II, ESIC Model Hospital, Rajaji Nagar, Bangalore 10. Last date of submission of Bid : 09/07/2014 at Hrs. If it happens to be holiday, on next working day at same time. Late / delayed tenders shall not be opened/entertained under any circumstances. Date and time of opening of Bids: The Bids shall be opened on 09/07/2014 at Hrs, if it happens to be holiday, then on next working day at same time and same venue by the duly authorized officer/team of officers of ESIC Model Hospital, Rajaji nagar in the presence of all such bidders who wish to be present. Authorized representatives of the quoting parties may be present for the opening of the bids, if they desire so. 3 3 INSTRUCTIONS TO THE BIDDERS 1. ESIC Model Hospital, Rajajinagar, Bangalore invites sealed quotations from the reputed firms for supplying Laundry Chemicals under Rate Contract for one year under two bid system. 2. The details of Laundry Chemicals to be supplied are furnished in the Annexure II with specifications. 3. Minimum eligibility: 1. The firm providers should be Bangalore based. 2. Should have minimum of 3 years service in the field 3. Should not have been blacklisted by the Govt. / PSUs 4. Bid Security/ Earnest Money Deposit (EMD) a) EMD for Rs.5,000/- (Rupees Five Thousand only) in the form of Demand Draft/Bankers Cheque from any Schedule bank drawn in favour of ESI Fund A/c-1 payable at Bangalore shall accompany the bid. b) EMD shall remain valid for a period of 30 days beyond the final validity period of bids (120 days). c) A bid received without Bid security (EMD) shall be rejected at the bid opening stage. d) The earnest money shall be refunded to the unsuccessful Tenderers after finalization of the contract. e ) It shall be refunded to the successful Tenderer on receipt of Security deposit or shall be allowed to be adjusted towards Security Deposit. f) No interest is payable on the EMD. g) The bid security may be forfeited: (i) If a bidder withdraws his bid during the specified period of bid validity specified in the bid document. (ii) In the case of successful bidder, if the bidder fails to : o Submit the surety bond after awarding of the contract o furnish the Performance security within the time specified in the document 5. Submission of Bid a) The tenderers are required to submit bid in two separate bid i.e. Technical bid and Financial bid, as per prescribed proforma. The two Bids should be submitted in two separately sealed envelopes superscribed Technical Bid for supply of Laundry Chemicals in ESICMH, Rajajinagar Bangalore -10 and Financial Bid for supply of Laundry Chemicals in ESICMH, Rajajinagar, Bangalore -10. Both sealed envelopes should be put in a third sealed envelope superscribed Tender for supply of linen items in ESICMH Bangalore -10. b) The sealed envelope for the Technical bid should contain the following: i) Technical bid tender form in Annexure I ii) Earnest Money Deposit of Rs 5,000/- in the form of Demand Draft/Pay Order from a commercial bank drawn in favour of ESI Fund A/c-1, payable at Bangalore. iii) The envelope should be superscribed as Technical bid for supply of Laundry Chemicals in ESICMH, Rajajinagar, Bangalore 10 iv) The details of Income Tax returns filed for the past three years. v) Copy of PAN number.

4 4 c) The envelope for the Financial bid should contain the Financial bid tender form in Annexure II and the envelope should be superscribed as Financial bid for supply of Laundry Chemicals in ESICMH, Rajajinagar, Bangalore 10. d) The sealed envelopes containing Technical bid and the Financial bid should be put into a third sealed envelope and superscribed a s Tender for supply of Laundry Chemicals in ESICMH Bangalore -10. e) Further samples of all quoted materials should be submitted by the participating firm in a sealed cover superscribed as Sample of Laundry Chemicals and the name of the tendering firm should be mentioned on the cover. N.B. All the documents submitted in the bid must be legible and self attested along with the seal of the firm. Otherwise the bid is likely to be rejected 6. Deadline for submission of bids 1. The above envelopes shall be sealed and addressed to The Deputy Director (General Branch - II), ESIC Model Hospital, Rajajinagar, Bangalore 10 and must reach on or before the 09/07/2014 by 2.00 p.m. 2. No bids will be received/accepted after the expiry of the prescribed date and time for submission of the bids. 3. The Medical Superintendent, ESIC Model Hospital, Rajajinagar, Bangalore - 10, may, at his discretion, extend the deadline for submission of bids. 7. Bid Opening and Evaluation The Technical bid shall be opened by the Officers authorized by the Medical Superintendent at p.m. on 09/07/2014 in the presence of the bidders or their representatives duly authorized by the bidder who wish to be present All the Technical bids will be scrutinized, relevant documents and sample given will be checked for their quality/authenticity and the tenderer whose technical tenders are accepted will be informed in writing about the date and time for opening the Financial bids. 8. Contacting the officials 12.1 No bidder shall try to influence the Purchaser on any matter relating to its bid, from the time of the bid opening till the time contract is awarded Any effort by the bidder to influence the Purchaser in the Purchaser s bid evaluation, bid comparison or contract award decision shall result in the rejection of the bid. 9. Non transferability : This tender is non transferrable 10. Right to accept / reject any or all Bids The Medical Superintendent, ESIC Model Hospital, Rajajinagar, Bangalore 10 reserves the right to accept or reject any bid including the lowest and to annul the bidding process and reject all bids, at any time prior to award of contract, without thereby incurring any liability to the affected bidder or bidders or any obligation to inform the affected bidder or bidders of the grounds for the said action.

Bid Prices a) The price quoted should be valid for the one year. b) Any conditional acceptance is liable for rejection of tender. c) The rates/ prices including the taxes,if any, should be quoted in Indian Rupees only in words as well as figures. d) The Contractor has to quote the price in the financial bid and who has quoted the least in comparison to the other tenderers shall be considered as the lowest tenderer item wise. 12. Awarding of contract a) The successful tenderer has to deposit Security Deposit of Rs. 40,000/- in the form of Demand Draft in favour of ESIC A/c No. 1 payable at State Bank of India, Rajajinagar, Bangalore 10 till completion of supply of materials. 13. successful tenderer will have to s u p p l y t h e m a t e r i a l w i t h i n s i x w e e k s f r o m t h e d a t e o f i s s u e o f s u p p l y o r d e r o n t h e r a t e s q u o t e d b y t h e m a n d a p p r o v e d b y t h e c o m p e t e n t a u t h o r i t y o f t h i s H o s p i t a l. 14. The Goods shall be supplied F.O.R. destination. 15. In case of delay / failure in supply, the indented items shall be purchased from the market at the risk and cost of the Contractor. The extra expenditure involved in procuring supplies from elsewhere will be recoverable from the Contractor in full at discretion of the Medical Superintendent. 16. The recovery thus due shall be deducted from any sum then due to the Contractor from the ESIC Model Hospital from this or any other contract placed with said contractor by the ESIC Model Hospital. 17. The Security Deposit will be forfeited if the Laundry Chemicals are not supplied as per sample provided / supply order. 18. Mode of Payment details : The payment shall be made through RTGS only. Hence the Bank details are to be submitted alongwith the bills. 19. The Medical Superintendent has the right to authorize / appoint / depute any person to supervise the services. 20. The Medical Superintendent, ESICMH, Rajajinagar, Bangalore -10 reserves the right to withdraw/relax any of the terms and conditions mentioned above. 21. The Medical Superintendent also reserves the right to terminate the contract before the contract period expiry with one month notice. 22. For any dispute regarding quality / service and rates the decision of the Medical Superintendent shall be final and binding to the Contractor. 23. All disputes relating to this tender can be legally resolved through courts in Bangalore only. 24. The Medical Superintendent, ESIC Model Hospital, Bangalore -10 is the Competent Authority to renew Contract or to terminate contract. 25. The Medical Superintendent, ESICMH, Rajajinagar, Bangalore -10 the right to reject/cancel all or any tender without assigning any reason thereof. Read and Accepted : Signature of Tenderer with seal
