Bids shuld be prepared in a straight frward and brief manner in rder t ensure the mst effective and equitable evaluatin f all materials received. Failure, in whle r in part, by a bidder t respnd t a specific mandatry requirement, as defined in Appendix F f this ITB, may result in a request fr crrective actin during the evaluatin prcess r rejectin f the bid. The Authrity reserves the right t amend this ITB at any time prir t the bid due date by issuing written addenda. Shuld this ccur, the Authrity may revise the bid due date, based upn the timing and magnitude f the amendment. Bidders shuld als reference the prcurement guidelines utlined in ITB subsectin 1.3, discussins f the prcurement prcess in ITB subsectin 1.2, pay particular attentin t the anticipated schedule presented in ITB subsectin 1.2.2, and cmply with Central Purchasing ITB Frm #2. A descriptin f the evaluatin prcess and methdlgy is presented in ITB Sectin TECHNICAL BID REQUIREMENTS The Technical Bid shall include eight (8) separate sectins (with named tabs) presented in the fllwing rder. The frmat and cntents fr the material t be included under each f these headings is described belw. Each subsectin within the Technical Bid must include all items listed under a heading because evaluatin f the bids shall be dne n a sectin-bysectin r functinal area basis. N reference t, r inclusin f, cst infrmatin shall appear in any sectin f the Technical Bid r the Transmittal Letter. Such inclusin can be reasn fr rejectin f the Bid. All mandatry bid submissin requirements are included n the Mandatry Bid Submissin Checklist included in Appendix F f this ITB. This checklist must be cmpleted, as described in subsectin Failure t adequately meet any ne mandatry requirement may cause the entire bid t be deemed nn-respnsive and it may be rejected frm further cnsideratin. Hwever, the state reserves the right t waive minr irregularities and minr instances f nn-cmpliance. In additin, as described in subsectin 4.2.5, all bidders are required t cmplete the MMIS Requirements List, als included in Appendix F f this ITB, as a cmpnent f their technical bid, Sectin E: MMIS Technical And Functinal Capabilities. Pages in excess f these limits will be remved during the Mandatry Requirements Phase f the evaluatin prcess and will nt be evaluated TRANSMITTAL LETTER - TECHNICAL BID SECTION A The fllwing describes the cntents f what is required fr the Technical Bid, Sectin A The Transmittal Letter The Transmittal Letter shall be submitted n fficial business letterhead by the prime cntractr and shall be signed by an individual authrized t legally bind the cmpany t the scpe f wrk prpsed. It shall be part f the Technical Bid (with named and numbered tabs). The letter is t identify all material and enclsures being submitted in respnse t the ITB. 4 an itemizatin f all materials and enclsures being frwarded in respnse t the ITB; a reference t all ITB amendments received by the bidder (by amendment issue date), t warrant that the bidder is aware f all such amendments in the event that there are any; if nne have been received by the bidder, a statement t that effect must be included; a statement f agreement that system enhancements prpsed 1) will meet the specificatins set frth in the ITB, 2) will meet HCFA certificatin requirements, and 3) that all cntract perfrmance standards set frth in the ITB will be met. a statement which warrants that pricing was arrived at withut any cllusin r cnflict f interest; a statement that identifies wh the bidder is that will be the prime cntractr and the name f the crpratin r ther legal entity submitting the bid. It shall als include a statement identifying any and all subcntractrs. The bidder will assume sle and exclusive respnsibility fr all f the cntractr respnsibilities and wrk indicated in the ITB (including any and all ITB addenda); identity f the Federal tax identificatin number f the prime cntractr and all subcntractrs; a statement f cmpliance with Affirmative Actin and Equal Emplyment Opprtunity regulatins, cnfirming that the bidder des nt discriminate in emplyment practices with regard t race, clr, ancestry, physical cnditin, creed, religin, age, sex, marital status, develpmental disability, arrest recrd r cnvictin, sexual rientatin, natinal rigin, r handicap; cnfirmatin that n cst r pricing infrmatin has been included in this letter r the Technical Bid; an expressin f the bidder's willingness t enter int an agreement with the state which includes the terms and cnditins f the cntract included in Sectin 6 f this ITB including an itemized list f any desired variances frm r negtiatin f these terms; Acknwledgement and acceptance that all requirements included in the ITB are mandatry. The bidder selected under this ITB may request nn-substantive changes t the cntract language in its Transmittal Letter, but OHCA reserves the sle right t accept r reject any requested changes BID SUBMISSION REQUIREMENTS CHECKLIST - TECHNICAL BID SECTION B Appendix F f this ITB cntains the Mandatry Bid Submissin Requirements Checklist each bidder must submit as part f the Technical Bid. The cmpleted Mandatry Bid Submissin Requirements Checklist must be included in this sectin f the bid. Agreement r acknwledgement f a requirement is shwn by a check mark (3) r "X" next t the requirement and a signature at the bttm f the checklist. The length f the Executive Summary is limited t a ttal f 20 pages. Bidders shuld address the fllwing three (3) items in their respnse t this sectin: summarize yur verall apprach and the cmmitments that yu are ffering t OHCA in this bid. Specifically, discuss the quality prpsed in yur apprach t: transfer f the MMIS, design, mdificatin, and implementatin f needed enhancements, design, develpment and implementatin f the DSS/Data Warehuse, and nging MMIS Cntractr peratins and respnsibilities, describe any risks that yu have identified with OHCA s apprach t transferring an MMIS, taking int cnsideratin the schedule defined fr accmplishing this effrt. 7 highlight the bidder s crprate resurces, including previus relevant experience, staff, cmputer facilities, financial stability, and crprate cmmitment t perfrming this cntract PROJECT MANAGEMENT PLAN (TECHNICAL BID SECTION D) The Prject Management Plan shall describe the bidder s verall plan and activities required t successfully cmplete this prject within budget and the defined schedule. The Prject Plan administratin activities include the set-up f all the internal management prcesses fr the cntractr and its subcntractrs, as well as the implementatin f all department and cntractr management prcesses and reprting requirements. The bjective f these cntrls is t ensure the smth administratin f the prject. Bidders must prpse an apprach t prject administratin that includes and describes the activities that shall be in effect thrughut the life f the prject fr each f the majr prject phases. The system demnstratin may be held in Oklahma r n-site at a lcatin f the bidder's chice. In this sectin, the bidder shall specify the preferred site fr a system demnstratin, if s required by the state. The bidder shall bear in mind that mre f the selectin/evaluatin team can bserve a system demnstratin held in Oklahma. The bidder must bear the cst and preparatin time fr such a demnstratin, except fr any required travel by the designated state evaluatrs. 11 4.2.6 STAFF QUALIFICATIONS AND EXPERIENCE (TECHNICAL BID SECTION F) Bidders shall submit a staff skill matrix in their wn frmat t summarize relevant experience f the prpsed staff. Additinally, bidders shall prvide a narrative descriptin f experience each key staff member has in the areas relevant t this prject. Bidder and subcntractr staff experience shall be shwn separately.
