The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. Discounts are available from our packages pricelist (attached). A casket must be purchased through Mehl s Colonial Chapel to receive the package discount. An additional 2% discount will be given when a casket is purchased and the balance is paid, by cash or check, within 10 days, with at least half prior to date of service. This discount cannot be given when payment is made with a charge card. Prior to drafting any contract for goods or services, the responsible party or the decedent s survivor who is handling the funeral arrangements is entitled to receive a copy of any preneed agreement in the possession of the funeral establishment that has been signed and paid for, in full or in part, by or on behalf of the decedent. TRADITIONAL FUNERAL SERVICE *MINIMUM SERVICES OF THE FUNERAL DIRECTOR AND STAFF: $ 1, Our services include: personnel available 24 hours a day, 365 days a year to respond to questions and initial call; conducting the arrangements conference; planning the funeral; consulting with family and clergy; sheltering of remains; preparing and filing of necessary notices; obtaining necessary authorizations and permits; coordinating with the cemetery, crematory, church or other third parties. In addition, this fee includes a proportionate share of our basic overhead costs of doing business, such as taxes, licenses and insurance required. This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials, and forwarding or receiving remains.) ADDITIONAL SERVICES OF THE FUNERAL DIRECTOR AND STAFF AND USE OF THE FACILITIES: Use of the Chapel and Staff to coordinate and direct the funeral ceremony at the chapel. (This includes 2 hrs. of visitation prior to the service, service at the place of final disposition or delivery to place of disposition). $ Use of The Staff to coordinate and direct the funeral ceremony at the church, temple, or other facility. If you do not want embalming, you usually have the right to choose an arrangement which does not require you to pay for it, such as a closed casket, direct cremation, immediate burial or less than 1 hour private family viewing. FORWARDING OF REMAINS TO ANOTHER FUNERAL HOME (at family request) $2, Our charges include: basic services of the funeral director and staff, local removal of remains during regular business hours, and embalming, if relevant. This price does not include: transportation to the airport or out of town funeral home and/or any shipping charges, the cost of a casket or alternative container, permit fee or the cost of an air shipping tray that is required by the airlines to protect the casket. See casket price list for these prices. No additional discounts are applicable. RECEIVING OF REMAINS FROM ANOTHER FUNERAL HOME $2, Our charges include: basic services of the funeral director and staff, a proportionate share of the overhead costs, storage of the remains (four days) and transportation to the cemetery. This cost is for delivery to the cemetery only. It does not include the cost of the use of our automotive equipment to go to the airport, visitation or funeral services, casket or permit fees. No additional discounts are applicable.. SHIPOUT TO MEXICO $3, These charges include: a portion of the basic services of the funeral director and staff charge, a proportionate share of the overhead costs, local removal during regular business hour, two hour visitation during regular business hours, embalming, other preparation of the body, acquiring needed notaries and clerk verification, translations of documents, transportation to airport, air tray and unionall. This charge does not include the cost of the casket, funeral or memorial services, certified copies of the death certificates, permit fee or fees from county and state for authentication of documents or use of staff to take documents to Consulate or Secretary of State Offices. If funeral or memorial services are requested, the full amount of the service charge will apply. No additional discounts are applicable. 10 FUNERAL PAYMENT POLICIES FOR MEHL S COLONIAL CHAPEL 1. If the funeral bill is paid by the date of funeral, (payment must be made either by check or in cash) there will be a two percent (2%) discount given on funeral services and merchandise purchased (not on cash advances). This discount cannot be given if the casket is purchased somewhere other than Mehl s or if payment is made with a credit card. Discount will not apply to Direct Cremation prices. 2. Insurance Policies will be accepted as payment (partial or full payment depending on the value of the policy) only if the insurance company accepts an assignment to the funeral home. If the insurance company does not accept an assignment of the policy to the funeral home then Mehl s cannot accept the policy as payment. 3. Monthly payments may be made when half of the funeral bill and all of the cash advances are paid by the time of the funeral service. This option is available only after Mehl s has run a credit check and has approved the good credit rating of the person signing the funeral bill. Upon credit approval, full payment is extended to 90 days. 4. When burial is to take place outside of California, all funeral expenses must be paid before the person is taken to the airport or transported outside of the state or country.
