The existing empanelled transporters are also required to apply afresh. Terms & Conditions, application form / documents and other details of pre-qualification for the tender may be downloaded from our website or under Procurement News Application complete in all respect in the prescribed format together with the required documents should reach to the Assistant General Manager, State Bank of India, Circle Stationery Department, 4 th Floor, Tilak Marg, Jaipur (Rajasthan) on or before 04:00 PM on the day of 23 rd January Applications received after the aforementioned time and date will not be accepted/ entertained. Technical bids will be opened before the interested bidders who will remain present on at SBI, Local Head Office, Tilak Marg, Jaipur at AM.. Empanelment of transporter will be at sole discretion of the State Bank of India who reserves the right to accept or reject any or all the applications without assigning any reason thereof. Interested vendors are requested to submit their application as per Annexure I to IV of the tender notice available on the Bank s site. The procedure for submission of applications is as under: Envelope I i. Terms & Conditions for Empanelment (Annexure I) duly signed in. ii. 4 Annexure I STATE BANK OF INDIA Circle Stationery Department, 4 th Floor, Local Head Office, Tilak Marg, Jaipur EMPANELMENT OF TRANSPORTERS FOR TRANSPORTATION OF STATIONERY BAGS FROM CSD JAIPUR TO BRANCHES/OFFICES IN THE GEOGRAPHICAL AREA OF THE BANK'S JAIPUR CIRCLE, JAIPUR Ph FAX TERMS & CONDITIONS FOR EMPANELMENT 1. ELIGIBILITY CRITERION: The criterion for empanelment to reputed transporters will be as under: (I) The interested transporters should have all the required statutory licenses/ permissions to run/ continue business in the declared line of activity which should be the primary activity. (II) Applicant vendors should be an Income Tax Assesses with a PAN/TAN No. and its Tax Returns must be up-to-date. (III) The transporter should be a Proprietary Firm / Partnership Firm/ Private Limited / Public Limited Co. / Co-operative Society / Govt. Undertaking. The applicant must have minimum 5 years experience in the line of activity. (IV) Turnover of the applicant for the last two years should be Rs.200 Lacs & above in the line of activity per annum. (V) The applicant should be an IBA approved transporter. (VI) Applicant should not have been barred or blacklisted by SBI and other nationalized bank, any other financial institutions, Public Sector Undertaking, State Government or Central Government etc. 2. The application will be submitted either by registered post or by hand. The procedure for submission of applications is as under: A. Envelope 1 Technical Bid- Envelope to be super scribed as Quotation for TRANSPORTERs Technical Bid Pre-qualification application for empanelment in the prescribed format, information, supporting documents and other details of the interested vendor along with non refundable demand draft of Rs.500/- (Rs. Five Hundred only) in favour of State Bank of India, Circle Stationery Department, Jaipur. B. Envelope 2 Financial Bid Envelope to be super scribed as Quotation for TRANSPORTERs Financial Bid The freight charges to be quoted by the bidder per bag exclusive of taxes, if any, separately for three different sectors mentioned in Para 11 of Annexure-II and selected by the interested bidder. Envelope 1 shall be opened first on the date fixed for opening the tender and Envelope 2 shall be opened on the same day only of those transporters who found

5 technically acceptable to the Bank. 3. Interested Transporters are requested to send their application with other required documents and best competitive quotation to: The Assistant General Manager (Procurement) State Bank of India Circle Stationery Department 4 th Floor, Tilak Marg Jaipur All application should reach latest by before 4:00 PM. by post or may be dropped in the Tender Box kept at the above address before the aforesaid date and time. Applications received after the aforesaid time and date will not be entertained. 4. All the vendors should give Income Tax PAN No. allotted by Income Tax authorities and Registration No. allotted by Sales Tax authorities. Xerox copies of the certificates of Income Tax Authority and Sales Tax Authority duly self certified, should be enclosed with the application form. 5. The Bank will have the right to relax the minimum pre-qualification criterion in exceptional circumstances. 6. Mere fulfilment of the eligibility criterion does not necessarily ensure short listing/ empanelment of such vendor or it will not give any right or claim for empanelment with the Bank. 7. The Bank will be within its right to empanel and distribute/ split the work order between L1 and L2 transporters under the above empanelment, if considered necessary, in the ratio of 60:40 in the interest of the Bank, provided L2 transporter matches the freight charges with that offered by L1 transporter. 8. The Bank reserves the right to reject all or any quotation without assigning any reason; or to call for fresh quotations at any time at its own. 9. Quotation(s) not fulfilling any or all of the conditions prescribed or which are incomplete is liable to be rejected. 10. Canvassing in connection with quotations is strictly prohibited. 11. Vendor participation is desirable at the time of opening of Technical and Financial Bids, as stated hereunder. However, if representatives do not turn up at the appointed date and time, the quotations will be opened in his absence and he will have no right to question the process adopted for opening bids. 12. Please note that two separate covers for Transport Operators Part A & B should be submitted along with the enclosures as mentioned in Para 2 above. If part 'A' or Part 'B' is submitted alone or if part 'A' and/or part 'B' is submitted in one envelope instead of separate envelopes, then the tender will be rejected summarily. 13. That this agreement shall be in force for a period of two years and may be renewed for further periods upon mutual consent by both the parties. 14. That the consignment will be picked up by the Contractor from the office or the place instructed by the Bank within a reasonable period but not later than 2 days from the date of such requisition. 15. That the goods so accepted for consignment will be delivered at the consignee's address within a reasonable period, in any case not exceeding six (6) days from the destination (within Jaipur District.), and not exceeding twelve (12) days for destination

6 other than Jaipur District. Except in cases where there is delay due to reasons / factors beyond the control of the Contractor i.e., due to floods, cyclones, bandhs, accidents, curfew etc. Suitable penalty be imposed if delay takes place beyond the permissible Rs. 5/- per day per bag for delay within Jaipur & Jaipur District. 16. That the goods consigned will be properly taken care of against all weather and fire hazards and will be adequately protected from thefts, burglary etc., In spite of that if there is still loss on account of fire or theft such claim will be settled by the Contractor. 17. That in the event of any damage caused to consignment in transit or due to omission/ commission of the vendor or its agent/ employee, the Vendor shall be liable for such damages and shall fully compensate the Bank for the loss on the basis of the claim put up by the Bank as per its record of dispatch which would not be disputed by the vendor and will be accepted as correct, except in cases where there is damage due to reasons/factors beyond the control of Contractor. The decision of the Bank will be final in this regard. 18. Any claim arising out of acts, omission or Commission of Contractor or its agent/ employee will be settled within 30 days from the date of receipt of the valid claim documents from the Bank. 19. The contractor shall keep with the Bank a deposit of Rs.1,00,000/- (Rupees One Lac Only) in form of TDR in favour of State Bank of India A/c of the transporter as security deposit which shall be released by the Bank to the contractor after fulfilment of the contract. 20. As and when required by the Bank, the contractor shall provide the vehicles desired by the Bank at the place or places directed by the Bank with proper authority/ licence to complete the consignments. 21. That the contractor shall supervise proper loading of the goods to avoid any damages to the same in course of transit. 22. That during the course of transit, the contractor shall take adequate and appropriate protective measures to protect the goods from damages or loss in any manner including tie up with ropes and tarpaulin covering. 23. That the contractor shall promptly deliver the consignment notes with the Bank's correct invoice Nos./Private marks to the Bank's dispatch section and any mistake shall be immediately rectified by the Contractor. 24. That the contractor shall promptly deliver the consignment to the consignees at the address mentioned and obtain a proper acknowledgement duly dated on the contractor's copy of the consignment note. 25. That the contractor shall advise the Bank the date of delivery of the consignment without any delay. 26. That in case of loss or damage of the consignment in course of transit, open delivery to be effected and the actual quantum of loss, as assessed by the consignees as per Bank's relative invoice, to be advised to the Bank immediately and without any delay. 27. That the bills for payment at the Bank's approved rates shall be submitted by the contractor to the consignee Bank branch accompanied with the acknowledged consignment note for issuing cheque/draft in favour of the contractor in due course. Delivery of such cheques/drafts shall be taken by the contractor or his agent duly authorized in writing from Bank's office. 28. That the door delivery shall be affected during the office hours of the consignees. 29. That in case of piece meal consignments proper intimation to the consignees must be given in writing at the destination just after arrival of goods and delivery thereof shall be affected on demand. 30. That if actual transportation is entrusted to the other agencies, at any stage, then it will be the sole responsibility of the Contractor to ensure proper adherence to all the above conditions and that the Contractor shall be primarily liable for any loss or damage

7 arising out of such transportation. 31. That for any delay on the part of Contractor for delivery to places other than Jaipur District, beyond six days from the scheduled delivery date if complained by any consignee Bank Branch/ office, the Contractor shall be liable to pay the entire cost of consignment with packing and other associated charges to the Bank. It shall be at the discretion of the Bank reserving further the right to forfeit security deposit and to take other actions, provided such delay is not on account of factors/reasons over which the Contractor has no control. 32. That on the basis of day to day experience, in case it is felt necessary to make additions/alterations of any of the above conditions, it may be done by exchange of letters, which will be acceptable to the Contractor. 33. Contractor shall indemnify and keep indemnified the bank without any demur against any loss or damage that may be suffered / incurred by the Bank by reason of Contractor's handling the transportation of the said goods from time to time as aforesaid. 34. All the goods consigned on the terms of TO PAY basis only. 35. All Disputes are subject to the jurisdiction of Jaipur city.
