2 Overview What is Open Enrollment? Open Enrollment is your annual opportunity to review available benefits and update elections. It takes place every spring, with your new benefits elections taking effect on June 1. The benefits Plan Year is always June 1 May 31. See the Timeline and Resources sections of this document to learn more. Please note that the only other time you can update your benefits elections is if you experience a qualifying life event, such as marriage, birth or adoption. For most people, that makes Open Enrollment your once-a-year chance to choose benefits. What is the ADP TotalSource, Inc. Health and Welfare Plan? It s the benefits plan offered to you through ADP TotalSource. The plan lets you choose the benefits that fit your specific needs. Your premium contributions are deducted each pay period before federal income and Social Security taxes are calculated. That means your taxes are lower and you keep more of what you earn. Which benefits can I enroll in? When you enroll online, you ll see the specific benefits options available to you. Generally, plan options include: Medical coverage Dental coverage Vision coverage Flexible Spending Accounts Health Savings Account (applicable with a High Deductible Health Plan) Additional Benefits Enrollment Opportunity During Open Enrollment, you may also have the opportunity to sign up for select TotalChoice TM Voluntary Benefits available through ADP TotalSource s partnership with Mercer. Depending on your eligibility, the following voluntary benefits may be elected during the enrollment window without proof of good health: critical illness accident hospital indemnity insurance term life insurance (up to $100,000) Additionally, eligible employees can elect short-term disability (up to 60% benefit coverage level) with evidence of insurability completion (medical questionnaire) or group legal services coverage. To enroll in these benefits, access the Mercer Marketplace through MyTotalSource.com.

3 Two Important Steps to Take Now First, provide your address. Your annual Open Enrollment period will be here before you know it, which means your benefits enrollment kit is coming! You ve got two choices: Wait for a kit to arrive at your home by U.S. mail, or receive your benefits information by . Here s how to make enrolling easier: Log in to MyTotalSource.com and click Myself > Personal Profile. Enter your address. Click Setup > Benefits Setup. Click on Benefit Paperless Opt-In. After completing these steps, you ll receive s with personalized enrollment videos throughout your enrollment window. See the Resources section of this document to learn more about the videos. Second, verify your information. Now is the time to verify whether the personal information on My TotalSource and your paycheck is accurate.
