2 Clemson University Insurance New Employee Orientation As a full-time employee of Clemson University, you are provided with an extensive benefits package. Clemson University has created the following presentation to provide a brief summary of insurance benefits. Included is a link to the Employee Insurance Program presentation for details. The plan document governs all state insurance programs, and should be referenced for all specifics. It is each employee s responsibility to review these presentations to make informed decisions regarding their benefits. Information is sent to eligible employees via with instructions to view this online orientation and employees are invited to attend a counseling session at the Clemson University Office of Human Resources to make their insurance elections. Insurance elections must be made within 31 days of your hire date per state guidelines. You are responsible for your benefits, so please do not assume that anything regarding enrollment is automatic.

3 Who is Eligible? Employee Rehired Retirees Spouse Children Employees and rehired retirees in a permanent position are eligible for insurance. They must work at least 30 hours per week and some Grant employees may be eligible. A wedded spouse or common law spouse can be covered, but the spouse cannot be an eligible state employee. You are eligible to cover an ex-spouse by court order only. Children can be covered under the insurance as long as they meet the following criteria: Under age 25 Not employed with benefits Approved incapacitation Per a court order Proof of full-time student status is only required for Dependent Life Child Insurance Proof of relationship is required for any dependent coverage (ex. Marriage license or child birth certificate)

4 Effective Dates HIRE DATE EFFECTIVE DATE After the 1 st working day 1 st of the following month of the month. 1 st day of the month 1 st day of current month 1 st working day of the month 1 st day of current month that did not fall on the or following month 1 st day of the month. The effective date for insurance coverage for an employee with a hire date after the first day of the month will be the first day of the following month. If the employee begins active employment on the first day of the month, insurance coverage begins on that day. If the employee begins active employment on the first working day of the month, but not on the first day of the month, then the employee may choose when the insurance coverage begins (either the first day of that month, or the first day of the following month).

5 Premiums Rates Deductions 2 nd paycheck of each month Deduct one month in advance Miscellaneous Other Deductions (MODs) Please review the Employee Insurance Program presentation for rates. At Clemson University, premiums for all insurance programs are deducted the second check date each month for the following month s coverage. Miscellaneous Other Deductions or MODs may be deducted as needed to catch up your premiums.

6 Health Insurance Options Savings State Health Plan (High deductible) Standard State Health Plan BlueChoice HMO Cigna HMO Tobacco Surcharge The state offers four health plans. Two PPOs, The Health Savings plan which is a high deductible plan and The Standard State Health Plan. The state also offers two HMOs, BlueChoice and Cigna. Beginning 1/1/2010 the state installed a monthly tobacco surcharge to be added to health insurance premiums for subscribers who use tobacco or dependents covered by a subscriber who uses tobacco.

7 Dental Insurance Options Basic State Dental Plan Dental Plus The state offers two dental plans, a Basic Dental Plan and Dental Plus which is a supplemental plan. The Basic Dental Plan provides basic dental coverage Dental Plus is offered at an additional dental premium and covers higher allowable amounts and the actual charge if the dentist participates in the Dental Plus provider network. If a subscriber enrolls in Dental Plus, the coverage and the premium is in addition to the Basic Dental Plan as the two work together.

8 Vision Insurance Options Vision Care Discount Eye Med Vision Insurance Employees eligible for benefits are eligible for Eye Med Vision Insurance which has a monthly premium. All employees can participate in the vision care discount program. You do not need to be enrolled in a health plan. Employee ID may be requested by the provider to verify eligibility for the program. This program offers eye exams and eye wear with participating providers at a discounted price. A subscriber may not use the Vision Care Discount Program and the Eye Med Vision Insurance at the same time.

9 Life Insurance Options Basic Life Insurance Optional Life Insurance Spouse Dependent Life Insurance Child Dependent Life Insurance An employee enrolled in a health plan is automatically enrolled in the basic $3,000 life insurance policy at no cost. For additional life coverage, an employee can enroll in an optional life plan through Metlife. Coverage levels are based on salary and premiums are based on coverage level and age. An employee may enroll a spouse or eligible dependent child in the dependent life plan through Metlife.

10 Disability Insurance Options Basic Long Term Disability Supplemental Long Term Disability An employee enrolled in a health plan is provided with a basic long term disability plan at no extra charge. For an additional charge, based on age and salary, you can enroll in a long term disability plan. The state does not offer short term disability.

11 Pre-Existing Condition Period 12 month waiting period Certificate of Coverage When you enroll in a health plan, a certificate of credible coverage from a former employer or insurance company may reduce or eliminate the period when the new plan doesn t cover pre-existing conditions if there has been no significant break in coverage A significant break in coverage is a period of more than 62 days in which you did not have health insurance. A pre-existing condition is any illness or injury for which medical advice, doctor care, or treatment was recommended or received during the 6 months before you enrolled in the plan. The plan will not pay for treatment of a pre-existing condition until 12 months after enrollment. If you are a late entrant, the plan will not pay any benefits for 18 months after enrollment. Pregnancy is not considered a pre-existing condition.

12 Enrollment Changes 31 day deadline Special Eligibility Situations Enrollment Period Annual (every October) -change health plans Open (October of odd years) -Enroll -Add/Drop dependents Money Plus You must make insurance elections within 31 days of your hire date, special eligibility situation or during enrollment periods. Special eligibility situations include Marriage Divorce Birth, adoption or placement of a child Involuntary loss of coverage Gaining other coverage Annual enrollment is held every year in October. During annual enrollment, you may make health plan carrier changes. Open enrollment is held in October (odd years) During open enrollment you may enroll as a late entrant and add or drop coverage or dependents Other options for annual enrollments will be announced in the Fall. Employees must enroll or re-enroll in Money Plus Medical and/or Dependent Care Spending every October to participate in the following year. An employee is eligible for the Medical Spending account feature of Money Plus after working for one full year as of January 1 st of the participating year.

13 Joseph F Sullivan Center Located in Edwards Hall Health Management Flu Vaccines Physical Exams Women s Health CU for Health The Joseph F Sullivan Center is located at Edwards Hall and offers health management to all faculty and staff of the University. State health program participants are eligible for a $15 health risk appraisal including; blood profile, height and weight measurement, blood pressure and follow up counseling.

15 Other Voluntary Insurance Programs AFLAC Agent: Ginny Murdock (864) AFLAC also offers several voluntary insurance plans at Clemson University to include cancer insurance, personal hospital intensive care, and personal accident expense coverage. There are individual and family plans available and benefits are not coordinated.
