1 Using TriNet s Open Enrollment Wizard: Step-by-Step Instructions Enrolling online is fast and easy. Need a guide? These steps will walk you through the Open Enrollment wizard. Getting There: Entering the OE Wizard Step 1 Click the Open Enrollment Icon To get started, from the home page of HR Passport, just click on the green Benefits Open Enrollment icon. 2 Step 2 Enter the Open Enrollment Wizard The home page of TriNet s Open Enrollment wizard is organized into three sections: Before Electing or Waiving: This section includes most of the information you need to research and consider the TriNet benefits options available to you, including a link to What s Changing, planning tools (such as flexible spending account (FSA) calculators and medical and dental plan comparison charts), and an overview of TriNet s additional (voluntary) benefits options. When You re Ready to Elect or Waive: In this section, choose Elect or Waive Now to walk through the step-by-step wizard and make your TriNet benefits elections for the new benefits plan year. All the comparison and planning tools are available on the relevant pages as you move through the Elect or Waive Now process. If You Don t Elect or Waive: This important section tells you what will happen to your TriNet benefits for the new benefits plan year if you don t actively elect or waive coverage. Click on the links to the information or section where you d like to start, and when you re ready to enroll, click Elect or Waive Now to get started. 3 You re In! Making Your Elections Your Dependents Step 3 Review or Add Dependents You re not required to cover your dependents. However, if you do plan to cover dependents under a TriNet medical, dental, vision, or life insurance plan, you need to enter your dependents on this page. You ll need each dependent s legal name, relationship to you, date of birth, and Social Security number to enroll him or her in any of TriNet s benefits options. Remember, entering a dependent s information will not automatically enroll the dependent in TriNet s benefits. In order to cover a dependent, you ll need to click the checkbox next to each eligible dependent you wish to cover when you elect TriNet s medical, dental, vision, and life coverage. 4 Health Coverage Step 4 Confirm, Elect or Waive Medical Coverage To confirm or elect your TriNet medical coverage, click the checkboxes next to the dependents you want to cover and then click the radio (round) button next to the TriNet medical benefit plan you want to elect. Note that the plan costs, along with the amounts your company and you will pay per pay period, will adjust based on the dependents you elect to cover. Still not sure about which plan to elect? Use TriNet s Medical Plan Comparison Chart (also called Ask Benefits ) or view plan details for any of the plans by clicking on the plan names on this page. Waiving TriNet medical coverage? Scroll to the bottom of this page and check the box next to the statement I elect to waive medical coverage and enter the name of the insurance carrier and group number of your group health plan coverage. Keep in mind that to waive coverage for yourself for TriNet s 2014 benefits plan year, you ll need to show TriNet proof that you have other group coverage. Click Next when you re ready to move on to the next step. Notice that you can always go back to the previous page by clicking the Back button. 5 Step 5 Learn About the Health Savings Account (HSA), if applicable If you enroll in a medical high-deductible health plan (HDHP), and you want to participate in TriNet s HSA, you re responsible for enrolling in the HSA. The HSA plan year runs for the entire TriNet benefits plan year, and HSA elections don t roll over from one plan year to the next. During Open Enrollment, you must elect the amount you d like to contribute to an HSA for TriNet s 2014 benefits plan year. On this page, you ll find important information about TriNet s HSA and your responsibilities as an HSA account holder. Step 6 Choose HSA Contributions, if applicable Once you ve reviewed the information on TriNet s HSA, scroll to the bottom of the page and enter the annual amount you d like to contribute to an HSA.. If you don t want to contribute to an HSA, but your worksite employer contributes on your behalf, enter 0. If your worksite employer does not contribute to your HSA plan, and you don t wish to contribute, check the decline box. Click Next when you re ready to move on to your dental elections. 6 Step 7 Confirm or Choose Dental Coverage To confirm or choose your TriNet dental coverage, click the checkboxes next to the dependents you want to cover and then click the radio (round) button next to the TriNet dental plan name you want to elect. Note that the plan costs, along with the amounts your company and you will pay per pay period, will adjust based on the dependents you elect to cover. Your dental election is separate from your medical election, so you can elect TriNet dental coverage for dependents you don t cover under a TriNet medical plan, or vice versa. This allows you to match your TriNet benefits elections to your family s needs. Still not sure about which plan to select? Use TriNet s Dental Plan Comparison Chart or view plan details for any of the plans by clicking on the plan names on this page. Waiving TriNet dental coverage? Scroll to the bottom of this page and check the box next to the statement I elect to waive dental coverage. Click Next when you re ready to move on to your TriNet vision elections. 7 Step 8 Confirm or Choose Vision Coverage To confirm or choose your TriNet vision coverage, click the checkboxes next to the dependents you want to cover and then click the radio (round) button next to the TriNet vision plan name you want to elect. Note that the plan costs, along with the amounts your company and you will pay per pay period, will adjust based on the dependents you elect to cover. Your vision election is separate from your medical or dental elections, so you can elect TriNet vision coverage for dependents you don t cover under a TriNet medical or dental plan, or vice versa. This allows you to match your elections to your family s needs. Still not sure about which plan to elect? Use TriNet s Vision Plan Comparison Chart or view plan details for any of the plans by clicking on the plan names on this page. Waiving coverage? Scroll to the bottom of this page and check the box next to the statement I elect to waive vision coverage. Click Next when you re ready to review your medical, dental, and vision choices. Step 9 Medical/Dental/Vision Coverage Review This page is a great way to confirm that you ve elected the TriNet medical, dental, and vision plans you want at the coverage levels you need. Pay particular attention to the plan names, individuals covered, and per-pay-period costs. Make sure the plans you elect are affordable for you and your family. Want to make changes? Click on the Edit button next to the coverage you want to change. When you re comfortable with your medical, dental, and vision elections, click Next to move on to life and disability coverage. 8 Life and Disability Coverage Step 10 Confirm or Choose Beneficiaries It s important that you confirm or update your beneficiaries for the life and disability coverage you have through TriNet. To add beneficiaries, click on the Add a New Beneficiary button and enter the legal name, relationship to you, date of birth, and Social Security number of the beneficiary you d like to add. Click Next when you ve finished confirming or updating your beneficiaries. Step 11 Review Basic Life Insurance This page displays the company-paid basic life and basic accidental death and dismemberment (AD&D) insurance you receive through TriNet. This coverage level is important to consider when making your decisions on supplemental life and accidental death and dismemberment coverage (which are coming up). Click Next when you ve finished reviewing this coverage. Step 12 Confirm or Choose Basic Life and AD&D Beneficiaries It s important that you confirm or update your beneficiaries for the basic life and AD&D coverage you have through TriNet. To add beneficiaries, click on the Add a Beneficiary button and enter the legal name, relationship to you, date of birth, and Social Security number of the beneficiary you d like to add. You ll also be asked to confirm if you d like to add a contingent beneficiary. Click Next when you ve finished confirming or updating your beneficiaries. 9 Step 13 Confirm or Choose Supplemental AD&D To enroll in TriNet s supplemental AD&D coverage, click on the box next to supplemental AD&D and select the coverage level you want. Note that the per-pay-period cost will adjust to reflect your chosen coverage level. When you ve made your decision on supplemental AD&D, click Next to confirm or choose your beneficiaries for this coverage. Step 14 Confirm or Choose Supplemental AD&D Beneficiaries It s important that you confirm or update your beneficiaries for the supplemental AD&D coverage you may enroll in through TriNet. To add beneficiaries, click on the Add a New Beneficiary button and enter the legal name, relationship to you, date of birth, and Social Security number of the beneficiary you d like to add. You ll also be asked to confirm if you d like to add a contingent beneficiary. Click Next when you ve finished confirming or updating your beneficiaries. Step 15 Learn About Group Variable Universal Life You have the opportunity to elect either TriNet group variable universal life insurance (GVUL) or TriNet supplemental term life insurance coverage. You can t elect both. This page provides an overview of group variable universal life coverage as well as contact information for MetLife, if you d like to learn more about this option. When you ve finished reviewing this information, click Next to review your options for supplemental life coverage. BR14 Worksite Employee Open Enrollment Instructions 9

10 Step 16 Confirm or Choose Supplemental Life Insurance You have the opportunity to elect either TriNet group variable universal life insurance (GVUL) or TriNet supplemental term life insurance. You can t elect both. This is the election page for supplemental term life coverage. You can choose coverage for yourself, your spouse, and your child(ren)*. For Yourself: To enroll in TriNet supplemental term life insurance, click on the checkbox next to Earnings Voluntary Life and select the level of coverage you d like. To enroll in TriNet supplemental term life insurance for your spouse or child(ren), you will need to have entered your spouse and child(ren) as dependents on the Dependents page. For Your Spouse: To enroll in TriNet supplemental term life insurance for your spouse, click the checkbox next to Supplemental Life Spouse and highlight the level of coverage you d like*. For Your Child(ren): To enroll in TriNet supplemental term life insurance for your child(ren), click the checkbox next to Supplemental Life Child(ren). When you ve made your decision on supplemental life insurance, click Next to confirm or choose your beneficiaries for this coverage. *Statement of health may be required Step 17 Confirm or Choose Supplemental Life Beneficiaries It s important that you confirm or update your beneficiaries for the supplemental life coverage you elect through TriNet. To add beneficiaries, click on the Add a New Beneficiary button and enter the legal name, relationship to you, date of birth, and Social Security number of the beneficiary you d like to add. You ll also be asked to confirm if you d like to add a contingent beneficiary. Click Next when you ve finished confirming or updating your beneficiaries. 11 Step 18 Confirm or Choose Disability Coverage On this page, you ll have the opportunity to confirm or elect TriNet disability coverage. To elect a TriNet disability coverage option, click the radio (round) button next to the plan name. Click Next when you re ready to review your life and disability coverage. Step 19 Review Life and Disability Coverage This page provides an opportunity to confirm that you ve elected the TriNet life and disability options you want, at the coverage levels you need*. Pay particular attention to the plan names, beneficiaries, and per-pay-period costs. Want to make changes? Click on the Edit button next to the coverage you want to change. The insurance carrier may require a Statement of Health approval before your TriNet life and/or disability election takes effect. When you re comfortable with your life and disability elections, click Next to move on to flexible spending accounts (FSAs). *Statement of health may be required BR14 Worksite Employee Open Enrollment Instructions 11

12 Flexible Spending Accounts (FSAs) Step 20 Learn About FSAs TriNet offers two types of FSAs: health care and dependent day care. On this page, you ll find links to more information and planning tools to learn how TriNet s flexible spending accounts can help you reduce your taxable income while paying for qualified expenses. Step 21 Choose FSA Contributions Once you ve reviewed the benefits of TriNet s FSAs and which accounts may be best for you, scroll to the bottom of the page to make your FSA elections. To participate in a TriNet FSA, click the checkbox next to the account(s) in which you d like to participate and enter your annual FSA election amount. If you don t want to participate in either FSA, click the checkbox next to Decline: TriNet Flexible Spending Accounts. Click Next when you ve finalized your FSA participation decisions. 13 Review and Submit Step 22 Review Elections Carefully; Then Submit This is the final review page to confirm that you ve elected the TriNet medical, dental, vision, life, and disability plans you want at the coverage levels you need, as well as your FSA or HSA elections for TriNet s 2014 benefits plan year. Pay particular attention to the plan names, individuals covered, and per-payperiod cost for each plan. Want to make changes? Click on the Edit button next to the coverage you want to change. Don t forget to read the fine print. Confirm or change your address to receive your enrollment confirmation, review the important information about your elections on this page, click the checkbox to certify that you ve reviewed your benefits elections and the important information, and then click Submit Benefits Elections. You ll receive an confirmation of your elections shortly after you submit them. You can revisit the Open Enrollment wizard as often as you d like to review or update your elections throughout the Open Enrollment period. The last submission you make before the Open Enrollment wizard closes will be the one TriNet uses to process your new 2014 benefits plan elections. Remember, unless you have a qualified life status change, these elections will be effective for the entire 2014 benefits plan year. BR14 Worksite Employee Open Enrollment Instructions 13

14 Congratulations! You ve Enrolled Successfully Step 23 Review Your Confirmation Statement Your ed Confirmation Statement serves as your record of enrollment. If you go back into the Open Enrollment wizard and submit changes, you ll receive a new confirmation of your elections for TriNet s 2014 benefits plan year.
