CIGNA and Medco Office Hours Ongoing Workshops Employee Discounts Contact Information Last Day of School June 16 Father s Day June 19 Next, complete your wellness services by June 30, All services must be competed between the dates of July 1, 2010 through June 30, Services include the required Health Assessment. To complete the required Health Assessment, employees who are CIGNA members can go to You do not need to send the completion page if you complete the CIGNA Health Assessment. Non-CIGNA members can complete the Real Age Health Assessment at For a non-cigna member to qualify, print only the completion page and include it with your wellness passport. Last, select one of the two options you would like for your reward. Option 1 In September 2011, the $120 reward check will be sent to your address of record. This option is taxable and your reward will be included in your W-2. Option 2 In January 2012, the reward will be deposited into the LCPS Medical Flexible Benefit Plan. This option is tax free. After your passport is complete and your reward has been selected, return it to the Employee Benefits office no later than July 15, Independence Day July 4

2 JUNE 2011 PAGE 2 Flexible Benefit Plan Q&A Who is the new Flexible Benefit Plan Administrator? The LCPS Flexible Benefit Plan is administered by Compusys/Erisa Group, effective 1/1/11. How do I submit for reimbursement? Claims are sent to and processed by Compusys/ Erisa. They accept claims and receipts by mail, and fax. To print a claim form or to get additional information about claim submission please visit their website at or call them at What is a Flex Debit Card? One of the enhancements to our Medical Flexible Benefit Plan is the addition of a flex Debit Card. This card works similarly to a check card and allows you to pay for your medical expenses directly out of your flex account at the time of service. You may be required to submit supporting documentation after the fact. Wellness Reward and the Flexible Benefit Plan Employees who completed and turned in their Wellness Passport by the deadline may choose to receive their $120 reward in a Medical Flexible Benefit account. This enables the reward to be tax free. Employees do not have to be enrolled in the LCPS health insurance to participate in this method nor do they have to sign up for a Medical Flexible Benefit Plan to receive the reward. Employees who choose to receive the award in a Flexible Benefit Plan will have it automatically set up effective 1/1/12. What if I am not currently enrolled? Open Enrollment is October 15 - November 15 each year. Compusys/Erisa Group, Inc Hymeadow Drive, Building Austin, TX Phone: / Fax: flex@cserisa.com Business Hours: 8:45 AM 6:00 PM EST Revised Optional Group Life Insurance Rates Effective July 1, 2011 Minnesota Life recently announced revised optional group life insurance rates for three age brackets, which will be effective July 1, 2011: Age 30-34: Reduced from $0.07 to $0.06 Age 50-54: Reduced from $0.22 to $0.21 Age 55-59: Reduced from $0.43 to $0.40 Rates for all other age brackets remain the same. Rates for coverage for dependent children also remain unchanged from the last update. Family Sick Leave Bank Even though the Individual Sick Leave Bank was replaced by the Short-Term Disability Plan, the family Sick Leave Bank is still active. The Family Sick Leave Bank provides employees with access to additional sick leave days when a member of an employee s immediate family, specifically the employees spouse, child or parent, is incapacitated by long-term personal illness or injury. The first 30 working days of a family members illness or disability must be covered by the employees own accumulated leave or leave without pay. After the first 30 working days, a maximum of 30 additional days of leave can be drawn from the Family Sick Leave Bank. To enroll you must donate two of your sick leave days as a one-time donation. Simply complete the enrollment form found on the Employee Benefits webpage during Open Enrollment. Open Enrollment is October 15 - November 15 each year.

3 JUNE 2011 PAGE 3 Do you know how much money you can defer? Did you know you can be enrolled in both the 403(b) Elective Deferral Plan and the VRS 457 Elective Deferral Plan w/ ING simultaneously? You can also contribute the maximum IRS allowable amount to each plan. The 2011 IRS allowable contribution amount is $16, or $22, if you re over age 50. If you re in both plans, that is a total of $33, or $44, If you have any questions about the 403(b) with Lincoln Financial Group (LFG), you can work one-on-one with a Retirement Consultant (RC). If you have any questions about the VRS 457 Elective Deferral Plan with ING, you can work one-on-one with David Detamore, the ING representative that works with LCPS employees. Call or David at or david.detamore@us.ing.com. Reminder Important Payroll Information Going Green! No More Printed Pay Stubs Effective July 1, 2011 LCPS will no longer provide printed pay stubs. All pay stubs will be available through the E-Pay System. It is easy, convenient and the E-Pay system provides up to three years of pay history. The Payroll Division will be happy to assist employees with registering and viewing their pay stubs on-line. Please call the Payroll Division at with any questions. Maternity/Paternity Workshops Continue Over the Summer Are you expecting in the summer or fall? Are you just curious about how maternity/paternity leave works? Register and attend a maternity/paternity workshop over the summer! You can register online on the Employee Benefits webpage. All workshops are at the LCPS Administration Building. Do you have questions about a doctor's bill or how to make sure an upcoming procedure is approved? Or maybe you have questions about a prescription you are taking and it's alternatives. Representatives from CIGNA (Health Insurance) and Medco (Prescription Insurance) are available for appointments. Next available appointments are July 13th (and the 2nd Wednesday of every month thereafter) from 1:30-4:30 pm at the Administration Building. Appointments are required. Call to schedule an appointment. LFG Office Hours Office hours will continue through the summer. Do you have questions about your current 403(b) Elective Deferral Plan with LFG? Or maybe you have questions about how to plan for retirement? Virginia Retirement System (VRS) Member Benefit Profiles (MBP) are Available Your 2010 VRS Member Benefit Profile is available online. Go to Click on Member Log-In under the Members section. Log-in with your name and password. Then on the left hand side, click Member Benefit Profile (MBP) LCPS Total Compensation Statement Your 2010 LCPS Total Compensation Statement is available online.
